Dear Randy:
The Frederick County Department of Planning and Development administratively approved the
above - referenced site plan on July 5, 2011. The site plan is approved for the construction of a
new 120 foot lattice type telecommunications tower located at 2042 Martinsburg Pike in the
Stonewall Magisterial District. Required improvements include the following:
• New 120 foot lattice -type telecommunications tower.
• Expanded enclosed compound, including a new 12' x 30' equipment building.
• Relocated landscaping.
• Construction of a five foot sidewalk along Martinsburg Pike.
The site plan meets all requirements for development in the RP (Residential Performance) Zoning
District and meets the conditions of Conditional Use Permit 905-10. All relevant review agencies
have approved the project at this time. I am providing you with five (5) copies of the approved
site plan. Please forward these copies to the appropriate representative(s). Furthermore, advise
the owner(s) that a copy should be kept for future reference, and an approved copy must be kept
on the construction site throughout the development process. Once site development is complete,
the owners) should contact this office to schedule an on -site inspection. Do not hesitate to
contact me should you have any questions or concerns.

Dear Randy:
Planning Staff has reviewed the above - referenced site plan to determine if administrative
approval can be granted. At this time, administrative approval cannot be granted. This site
plan is denied until the issues in this letter, as well as all issues of the other review agencies,
have been adequately addressed. Please review Staff's comments listed below and then
prepare a revised site plan which adequately addresses each concern.
Review Comments:
I . Special Limited Power of Attorney Form. Please have the property owner sign the
attached Special Limited Power of Attorney Form prior to further review or approval.
This form should be notarized and completed to grant the applicant (you) with
authorization to represent them for this site plan.
2. Lot Consolidation. The lot consolidation needs to be completed before the site plan
can be approved.
3. Site Information. On the coversheet under the site information, revise the use from
residential to telecommunication facility, revise the note under condition 9, reference
the previously approved site plan ( 940 -08), and provide the tower height and type.
4. Tower Construction Type. Please note that this site plan cannot be approved as a
lattice style tower unless the Board of Supervisors approves the waiver request
(scheduled for November 10`h).
5. Shed Relocation. A location for the shed proposed for relocation (adjacent to the
existing AT &T building) needs to be provided.
6. Existing Structures. A note needs to be provided that the existing residential
structures will be removed prior to a final occupancy being provided. The existing
gravel driveway and parking areas also need to be removed.
7. Sidewalk. A sidewalk is required along the Martinsburg Pike frontage of this site.
8. Approved Plans. The improvements from the previously approved site plan need to
be shown on this plan (parking and landscaping).

After you have revised the site plan, please resubmit one copy so that 1 may verify the
information contained on the plan. 1 will need all approved review agency comment sheets
and at least five copies of the final plan for final administrative approval.
