Unless otherwise noted, specificationsfor all work to be in accordance with applicable Virginia Department of Transportation Road and Bridge Standards, the Virginia Departmentof Environmental Quality Erosion and Sediment Control Handbook, and the Virginia Department of Transportation Road and BridgeSpecifications.2.All construction and methods shall comply with the latest U.S. Department of Labor, occupational safety and health administration (OSHA),and Virginia Occupational Safety & Health (VOSHA) rules and regulations.3.The contractor shall comply with title 56, chapter 10.3 et. seq. of the Code of Virginia (Underground Utility damage Prevention Act).4.The contractor shall comply with title 59.1, chapter 30 section 406, et. seq. of the Code of Virginia (overhead high voltage lines safety act).5.The placement of a construction trailer, fencing, parking, and staging areas shall be coordinated with and approved by the the propertyowner and developer.6.Access for emergency and utility service vehicles shall be maintained to the area surrounding the site.7.The Contractor shall be responsible for hiring a surveyor to provide construction surveying stake out.8.The Contractor shall be responsible for hiring a testing firm to provide all earthwork and compaction testing.9.This design is based on topographic mapping provided by Carmeuse10.The engineer has attempted to show all subsurface utilities, however, such may exist that are not shown. The contractor shall exercise care inthis work so as to avoid damage to any utilities. Any damage shall be the responsibility of the contractor. The contractor shall notify MissUtility of Virginia 72 hours prior to digging.11.All disturbed surfaces outside of the work area shall be restored to the pre-construction condition by the contractor at the contractor'sexpense.12.The project area falls within a VA Dept of Mines, Minerals and Energy permitted surface mine area. As such a local Land Disturbance Permit isnot required. All erosion & sediment control measures shall comply with DMME requirements.13.Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction.14.Any unusual subsurface conditions (e.g., unsuitable soils, springs, sinkholes, voids, caves, etc.) encountered during the course of constructionshall be immediately brought to the attention of the engineer. Work shall cease in that vicinity until an adequate design can be determinedby the engineer and approved.15.All surfaces and slopes shall provide positive drainage away from the project area.EARTHWORK NOTES1.All embankments (fill) placed on this project are to be controlled fills, and placement is to be observed and tested by the geotechnicalengineer.2.All excavation is unclassified.3.Materials - Suitable excavated material shall be used in forming the embankments. Materials classified as OL, OH, or Pt are not suitable foruse as controlled fill. The on-site high plasticity soils are generally suitable for re-use as controlled fill provided that proper drainage, grading,and sloping away from the structures is maintained both during and after construction. Blasted or “shot” rock can be utilized for fill providedthat the maximum particle size of the rock not exceed 12 inches and sufficient amounts of soil fines be mixed with the rock to fill in openvoids between the rock particles. The nesting of boulders in the embankment is prohibited. The material to be used in embankments shall befree of frozen or organic materials such as leaves, roots, grass, weeds, and all other material not consistent with construction of a stable,homogeneous fill. Embankments shall not be constructed on frozen ground.All proposed fill materials should be approved by the geotechnical engineer prior to placement as controlled fill, and representative samplesshould be obtained one week prior to placement of that material to allow time for completion of the necessary laboratory tests.4.Site Preparation - All vegetation and topsoil shall be removed from areas upon which embankment will be constructed. Topsoil shall bestockpiled as required by the E&S plan. Vegetation shall shall be converted to mulch and used for E&S control or removed frm the site by thecontractor. All sloping areas upon which fill is to be placed should be benched or “notched” so that a smooth interface between existingground and new fill will not be present. Each layer of fill should be benched into the existing ground a minimum of 3 feet horizontally and thedepth of one fill layer. Flat areas upon which fill is to be placed shall be deeply plowed to allow for bonding with the existing material. Thecontrolled fill slopes and embankments should be constructed at the designed 2H:1V slopes or flatter.5.Formation in Layers - Unless otherwise provided, embankments shall be constructed in successive layers no more than 6 inches thick, loosemeasurement. Where embankments are built by dumping from draglines, trucks, or other similar equipment, a bulldozer must be operatedconstantly to spread the material. These layers must be full width of embankment, each thoroughly compacted, built to the true slope, andnot widened with loose material from the top. When embankments are being constructed principally of rock, the depth of each layer shall becarefully distributed throughout the embankment, and the voids shall be filled with fine material to secure the maximum density. The mostsuitable material shall be reserved for top 2 feet of the embankment.Large stones with any dimension greater than six inches shall not be permitted within two (2) feet of the design subgrade. As theembankment is consolidated, the slopes shall be carefully dressed to the desired section and maintained to their proper height, dimensions,and shape until the work is accepted. When transporting material with rubber-tired equipment, care shall be taken to see that the trailingunits do not follow in the tracks of the preceding unit. At the end of each day's work the embankment shall be dressed to shed any waterthat might fall during the night.6.Density - Suitable compaction equipment shall be continuously operated while embankments are being constructed. All fill materialcompacted by heavy compaction equipment should be placed in maximum 6-inch loose lifts for the uppermost zones previously describedand 12-inch loose lifts for the deeper fill areas. In areas where larger rock is utilized, the size of the loose lift will be dependent on thequantity of rock within the overall fill matrix, and this can be evaluated by the geotechnical engineer at the time of actual placement. All fillmaterial compacted by hand- operated tampers or light compaction equipment should be placed in maximum 4-inch loose lifts. While work isprogressing in separate areas, approved compaction equipment shall be operated continuously in each embankment area.Prior to compaction, the moisture content of each layer should be adjusted, as necessary, to obtain the required moisture content to achievethe specified compaction level. Each layer should be compacted to the required percentage of maximum dry density. Fill should not beplaced on surfaces that are muddy or frozen, or have not been approved by testing and/or proof-rolling. Free water should be preventedfrom appearing on the surface during or subsequent to compaction operations. Soil material which is removed because it is too wet to permitproper compaction can be stockpiled, or spread and allowed to dry. Drying can be facilitated by discing or harrowing until the moisturecontent is reduced to an acceptable level. When the soil is too dry, water should be applied uniformly to the subgrade surface or to the layerto be compacted.Compaction of the embankments shall be to density of 95 percent of that obtained in a Standard Proctor Density Test, ASTM D-1557. Themoisture content of the soils should be within three (3) percentage points of the optimum moisture content. In areas where mixtures of shotrock and soil fill are placed, minimum passes with compaction equipment should be established during construction. All areas should beproof-rolled with approved equipment for acceptance of compaction by the geotechnical engineer where density tests cannot be reasonablyconducted. Material that does not contain adequate moisture to obtain specified density shall require the incorporation of additionalwater. Material containing an excess amount of moisture shall not be placed in an embankment until it has been allowed to dry to the designmoisture content.7.All blasting operations shall be performed in accordance with the Fire Marshall's requirements.
