Hi Jason. Attached is the letter of credit (LOC) format which we accept
for bonds. This LOC form should be printed on the lending institution's
letterhead - fill in the blanks to specify the project's name. Because
the original bond was established in 1999, it is appropriate to increase
the amount to $25,000 to cover inflation. Therefore, please submit the
new LOC in the amount of $25,000. Thank you.

WHEREAS, Adams Family Partnership is the owner of that certain property
lying in Shawnee Magisterial District, Frederick County, Virginia, as more particularly
shown on that certain plat of Elliott Ritchie, Jr., Land Surveyor, dated November 30,
1999, known as Final Plat of a Division of a Portion of the Land of Adams Family
Limited Partnership, a copy of which is attached hereto and by this reference made a
part hereof; and
WHEREAS, said property is a portion of the land conveyed to the party of the
first part by Deed dated May 23, 1994 and recorded in the Clerk's Office of the Circuit
Court for Frederick County, Virginia in Deed Book 826, at Page 479; and
WHEREAS, said subdivision has been approved in accordance with the laws of
the Commonwealth of Virginia and the ordinances of Frederick County, Virginia; and
WHEREAS, the party of the first part desires to dedicate that certain parcel of
land designated as "Fern Court" to the party of the second part, as shown on said plat.
NOW, THEREFORE, the parties of the first part do hereby dedicate to the party
of the second part for public use that certain parcel of land designated as "Fern Court"
as more particularly shown and described on the plat attached hereto. The party of the

Upon review of the subject road plan, we offer the following comments:
1. Prior to commencing construction of the road, it will be necessary to obtain a land
disturbance permit from the Public Works Department.
2. As stated in your erosion and sediment control narrative, the proper length of a
construction entrance shall be 70 feet. However, your detail indicates a length of
50 feet.
These comments are minor in nature. Therefore, we recommend approval of the subject
road plan.
We request that you submit a copy of the final revised plan for our file. A more thorough
review of each parcel will take place at the time of the full site plan submittal

This is to acknowledge receipt of your revised plans dated June 21, 1999 for the
referenced project. The plans appear satisfactory and are approved. Please advise the
developer accordingly. Please provide seven sets of approved construction
plans with signed seal for VDOT distribution.
I offer the following comments:
• A preconstruction conference be held by the engineer and/or developer with the
attendance of the contractor, various County agencies and VDOT prior to initiation
of work.
• Materials used and method of construction shall apply to current observed VDOT
Road & Bridge Specifications applicable during construction of this development.
• Our review and comments are general in nature. Should conditions in the field exist
such that additional measures are warranted, such measures shall be completed to
the satisfaction of the Department prior to inclusion into the Secondary Road
System.
• Attached is a copy of the minimum requirements and information needed prior to
acceptance of subdivision streets into the Secondary System. This is the
responsibility of the developer.
• All drainage is to be carried within the right-of-way in ditch lines or gutters along the
street to a pipe or drainage easement.
• The contractor shall notify VDOT when work is to begin or cease for any
undetermined length of time. VDOT will also require 48 hours notice for inspections.

• The appropriate land use permits shall be obtained before any work is performed on
the State's right-of-way. The permit is issued by this office and will require a
minimum processing fee, surety bond coverage, and the salary & expenses of a

A VDOT review has been completed on the preliminary plan dated 05/11/99
(received 05/18/99) for the referenced project. Our comments are as follows and as
shown in red on the enclosed subdivision checklist and plan sheets:
Sheet 2 of 6
1. The dimensions for the right turn lane and taper for Route 645 (Airport Road)
eastbound vehicles entering the proposed subdivision street were established and
graded into place when VDOT Project #0645-034-212, C501 was constructed. A copy
of Plan Sheets 5 & 6 of 12 are attached for your use. The exit taper on the east side of
the subdivision street should be 12'x48'.
2. The subdivision street should have a 36' width beginning at the radii return and
extending 150' south then tapered 100' to the normal 24' shown on the typical section.
A pavement striping plan and turn arrow should be provided at the turn lane locations
(see VDOT General Note #17).
3. Potential CG-12 ramp locations should be shown in some format to supplement the
general note covering this item. A CG-11 standard VDOT entrance should be shown.
4. Right and left turn arrows should be provided as appropriate in striping plan. CG-7
curb should be installed on the radii and also along the right turn lane as shown.
5. The additional pavement added by others (not inspected by VDOT) approximately 25'
in length along the entrance should be removed and replaced with the pavement
structure proposed for the entrance. VDOT Inspector will monitor this item, a note
indicating the removal/replacement should be added to the plan sheet.

6. See Plan Sheets 5 & 6 of 12 for the Route 645 project. The plan calls for a 20' required
drainage easement alonglapproximately parallel to Route 645. If this easement has
been dedicated it could/should be utilized for site culvert at the entrance street if
possible.

7. The proposed culvert should be placed at low point on vertical curve to allow
placement of drainage structure if/when CG is placed on subdivision. Street culvert
should be RCP with ES-1's each end.
8. The proposed sleeves crossing the roadway should be placed that 36" minimum cover
is provided from the bottom of the roadway ditch, portions of the parallel 8" PVC water
line should be placed so that 36" minimum cover is provided from the shoulder break
point.
9. Please note several errors appear at the vertical curve data on Station 13+25.

11.Additional VDOT detail sketches should be added: CG-7; CG-11; CG-12; CD-2 and
ES-1 (remove ES-2).
If VDOT trenching detail is shown on plan detail sheet, it should be current VDOT detail
which is: Subsequent Special Provisions and Figure 1, Cut Replacement in Roads with
Asphalt Concrete Base and Surface (copy attached).
As indicated earlier, the red comments on the subdivision checklist should be removed
and applied to the plan sheets.
When all the above comments have been addressed, please provide one set of revised
plans for VDOT review. If there are any questions, please feel free to call.

referenced project. The plans appear satisfactory and are approved. Please advise the
developer accordingly. Please provide seven seta of ,approved construction
plans with signed seal for VDOT distribution,
I offer the following comments:
• A preconstructlon conference be held by the engineer and/or developer with the
attendance of the contractor, various County agencies and VDOT prior to initiation
of work.
• Materials used and method of construction shall apply to current observed VDOT
Road & Bridge Specifications applicable during construction of this development.
• Our review and comments are general in nature. Should conditions in the field exist
such that additional measures are warranted,, such measures shall be completed to
the satisfaction of the Department prior to Inclusion into the Secondary Road
System.
• Attached is a copy of the minimum requirements and Information needed prior to
acceptance of subdivision streets Into the Secondary System. This Is the
responsibility of the developer.
• All drainage Is to be carried within the right -of --way In ditch lines or gutters along the
street to a pipe or drainage easement.
• The contractor shall notify VDOT when work is to begin or cease for any
undetermined length of time. VDOT will also require 48 hours no *p for inspections.

• The appropriate land use permits shall be obtained before any work is performed on
the State's right-of-way. The permit is issued by this office and will require a
minimum processing fee, surety bond coverage, and the salary & expenses of a

Upon review of the subject road plan, we offer the following comments:
1. Prior to commencing construction of the road, it will be necessary to obtain a land
disturbance permit from the Public Works Department.
2. As stated in your erosion and sediment control narrative, the proper length of a
construction entrance shall be 70 feet. However, your detail indicates a length of
50 feet.
These comments are minor in nature. Therefore, we recommend approval of the subject
road plan.
We request that you submit a copy of the final revised plan for our file. A more thorough
review of each parcel will take place at the time of the full site plan submittal.
