Smith Route 1, Box 1985 Berryville, Virginia 22611 Dear Mr. Smith: This letter is to confirm the Frederick County Board of Supervisors' action at their meeting of March 9, 1988: Approval of Conditional Use Permit Request #004-88 of Joseph C. Smith for an adult convalescent home for 25 residents. This property is located east of Route 7, one mile east of town. The first house east of Paramount Pest Control, and one house west of Shenandoah Computer, in the Stonewall Magisterial District. With the following conditions: 1. This is a one-year permit to be reviewed and renewed annually by the staff, the Planning Commission, and the Board of Supervisors. 2. If the use, occupancy, or ownership of the property changes, this conditional use permit shall expire and a new conditional use permit will be required. 3. Proposed structure is approximately 32' x 100'. REVIEW EVALUATIONS: VA Dept. of Highways & Transportation - The property is currently being served by an approved commercial entrance which will presently serve proposed expansion. Health Department - No objection, City water and sewer connections. Sanitation Authority - Proposed home currently has water and sewer directly available to the site. Greenwood Fire Company - No comment. Inspections Department - All buildings shall comply with Section 307 (Use Group I, Institutional Buildings) of the 1984 BOCA Basic National Building Code. Planning and Zoning - No objections. A convalescent/nursing home is permitted in the RP (Residential Performance) zoning district with a conditional use permit. A site plan is recommended for this type of use. STAFF RECOMMENDATIONS: Approval, with the following conditions: 1. This is a one-year permit to be reviewed and renewed annually by the staff, Planning Commission, and the Board of Supervisors. 2. If the use, occupancy, or ownership of the property changes, this conditional use permit will expire and a new conditional use permit will be required. 3.
