Box 897 New Market, VA 22844 Dear Mr. Reinhart The subject waterworks was required to have a Level 2 assessment completed per the Notice of Level 2 Assessment Required, dated December 11, 2020, Enclosed is a copy of the'assessment completed by Deborah Hoy, Environmental Health Compliance Specialist, of this Office. The Level 2 assessment was required due to an exceedance of the E. coli Primary Maximum Contaminant Level (PMCL). Refer to the Notice of Level 2 Assessment Required and Notice of Alleged Violation dated December 11, 2020 for details regarding the events which triggered the assessment requirement. Although a potential cause for the contamination was not determined, the well is located in karst geology which is subject to increased contamination of ground water. The results of 20 Most Probable Number (MPN) bacteriological raw water samples collected during May 2018 determined that disinfection treatment with contact time was required. A Waterworks Construction Permit was approved on December 3, 2018. However, the required disinfection treatment has not been installed; therefore, the cause of the contamination may be due to generally poor ground water quality in the area, as determined in 2018, REQUIRED ACTION Review the assessment, paying particular attention to the "elements of concern" and the enclosed schedule of corrective actions. If you are in agreement with the assessment and the schedule of corrective actions, sign, date and return page 9 of the assessment and the schedule of corrective actions form to this Office no later than January 20, 2021. INTERIM ACTION REQUIREMENTS Based on the Level 2 assessment findings, the following interim actions must be taken: 1, Ensure that the ODW prepared Boil Water/Do Not Drink Advisory is posted in the restroom facility until informed otherwise. This notice was emailed to you on December It, 2020, and was posted by Winchester Battlefields Visitor Center staff on December 12, 2020. 2. Continue to keep the Winchester Battlefields Visitor Center waterworks closed until all required Corrective Actions listed below have been completed, inspected, and approved by the ODW -r Culpeper Field Office. On December 18, 2020, the Winchester Battlefields Visitor Center waterworks waterlines were depressurized to prevent freezing and the Visitor Center was voluntarily closed to the public. In addition, it was mentioned during the assessment that the adjacent property with the mobile home is owned by the Foundation. This property has a well in front of the home that is no longer in use and reportedly contaminated. It is recommended that the well also be officially abandoned. I agree to fully implement the above interim actions and corrective actions to rectify the elements of concern that may have led to the E. coli MCL violation. I will notify the Virginia Department of Health, Office of Drinking Water, in writing, within 1 business day upon completion of each corrective action. There has been noted possible sight distance issues that need to be addressed along with this installation. For additional guidance and details please see IIM-TE-384 which allows for the installation of these facilities if at least a Condition B is met (See attachment A Section 7.4) which has already been determined by the design engineer. Along with the crosswalk, Traffic Engineering recommends installing advanced pedestrian signage in advance of the crosswalk location to alert oncoming traffic and at the crosswalk install a "Pedestrian Actuated Rectangular Rapid Flashing Beacons" structure with necessary signage, etc. Once designed if you would like Traffic Engineer to provide a review, please forward to this office and we will be happy to look over the details of the proposed equipment, layout and other aspects of the proposed accommodation. Attached please find the Interim Approval by the FHWA for the installation of RRFB's at uncontrolled marked crosswalk locations for your use. There had recently been a removal of the approval of these facilities, however, the allowance has been reinstated under certain conditions. The referenced plan is acceptable provided the above items are included with the final plan for signature site. A maintenance agreement and VDOT Land Use Permit are necessary prior to installation.
