The plans have been reviewed and the office issues approval of the plans as submitted. Before issuing a food establishment permit for the facility, the following must be completed: 1. A member of our office needs to conduct a construction inspection sometime during the final phase of the construction of the restaurant evaluating the facility in respect to the original approved design. Specifically the flow of operations, equipment requirements, design construction, plumbing, etc. will be assessed and any issues must be addressed. 2. At least one week before opening the restaurant, a member of our office must perform a pre - opening inspection. At this point the restaurant should be "ready" to open and evaluated as such. Any violations found must be corrected before a permit can be issued. 3. Our office must receive copy of Bonjangles' employee health policy. 4. Our office must receive copy of the Certificate of Occupancy as issued by the Frederick County Building Department. Refer to Sheet C-1.0: It is recommended that Marsh & Legge Land Surveyors, P.L.C. verify the location and invert of the "existing" 2 1 " pipe shown on sheet C-3.0. This verification shall be accomplished prior to the final site plan approval. 2. Refer to Sheet C-3.0: a. The storm sewer schedule should reference the Virginia Department of Transportation (VDOT) designations for the drop inlets (catch basins) and curb inlets. b. Revise the above schedule to indicate that CB-2 flows to C1-10 rather than C1-9 as shown. C. Include the "JB" designation and description in the grading plan legend. d. The legend referenced above indicates that a water quality insert is shown on sheet C-3.2. This detail is not included on sheet C-3.2. 3. Refer to Sheet C-4.0: a. Define HDC in the paving legend. b. Reference VDOT standards for type of curb and gutter. The concrete paving should be shown for the entire drive-thru area. Check the proposed inverts of the sanitary sewer entering the building and the possible conflict with the proposed storm sewer. 5. Refer to Sheet C-5.2: a. Provide a storm sewer profile to prevent conflicts with water and sewer lines. b. Show separation requirements between 2" water line and the proposed 12" storm sewer. 6. Purchase of Nutrient Credits: The letter provided by Winchester Management, Inc. dated October 27, 2016 indicates the availability of 1.78 nutrient credits. Provide this office with a final agreement indicating the purchase of these credits. This agreement will be required prior to issuance of a land disturbance/stormwater discharge permit.
