At this time, administrative approval cannot be granted. This site plan will not be approved until the issues in this letter, as well as all issues of the other review agencies, have been adequately addressed. Please review Staff s comments listed below and then prepare a revised site plan which adequately addresses each concern. Review Comments: 1) Power of Attorney. A signed and notarized "Special Limited Power of Attorney" form (from the Site Plan Application package) should be provided to complete the site plan project file prior to site plan approval. 2) Site Plan Reference Number. Add the Site Plan (SP) #12-19, to the cover sheet. 3) Parcel Identification Number: Update the parcel identification number (PIN) for the subject property to 64-1-A1. 4) Minor Site Plan. The following elements should be added to the plan set as follows: • Title: Update the title to `Blue Ridge Hospice — Flag Pole & Parking Area Addition" in the title block. • Current Zoning: Add "Current Zoning: B2 (General Business)" • Magisterial District: Add "Magisterial District Shawnee" • Parking Setbacks: Add "Front Setback — 10-feet; Side/Rear Setback — 5- 107 North Kent Street, Suite 202 9 Winchester, Virginia 22601-5000 • • Re: SP #12-19, Blue Ridge Hospice — Flag Pole & Parking Area Addition April 8, 2019 Page 2 feet". It does not appear that the parking area is setback at least S feet from the side property line with Wincrest Drive as presently shown on the plans. The location of the parking should be amended as appropriate to meet the required side setback. • Previous Approvals: Add a note referencing previously approved site plans for the above subject property as follows: "Previous Approvals: Site Plan(s) #03-87 & #39-93. • General Notes: Add the following statement under Notes: "No modifications are proposed to existing buildings, site lighting and/or signage." • Flag Pole Height: Add a not specifying the proposed height of the flag pole. 5) Parking Area. Parking areas require a minimum 6-inch curb and gutter and parking areas and walkways must be concrete or asphalt when located within the County's Sewer and Water Service Area. (Frederick County Zoning Ordinance § 165- 202.01(D)(1) & (3)(c). Concrete wheel stops may be substituted for curb and gutter when approved by the Director of Public Works. Remove reference to "gravel" for walkways and parking areas under Notes. Please submit two (2) copies of the plan an agency review request cover letter to the Department of Public Works (County Engineer) if it is the intent of the designer to utilize concrete wheel stops in lieu of curb and gutter. Label the individual parking stalls dimensions. The minimum width is 9-feet and the minimum depth is 20-feet (Frederick County Zoning Ordinance § 165- 202.01(D)(9)(a)). 6) Landscaping. One (1) shade tree is required per every 10 parking spaces (Frederick County Zoning Ordinance § 165-202.01(D)(13)(a)). The addition of three (3) parking spaces requires one (1) shade tree for this project. Depict the location of the proposed shade tree and note the tree species and that the tree will have a minimum 2-inch caliper at time of planting. 7) Agency Approvals. Comment sheets (approvals) are required from the following agencies: the Frederick County Inspections Department, the County Engineer (Public Works, see comment above) and the Frederick County Fire Marshal. After you have revised the minor site plan, please resubmit one (1) copy so that Staff may verify the information contained on the plan addresses the above comments. Staff will also need all approved review agency comment sheets and at least five (5) hard copies of the final plan for approval and a digital (PDF) copy for the project file.
