This is a one year permit to be reviewed and renewed annually by the staff, Planning Commission, and the Board of Supervisors. 2. If the use, occupancy, or ownership of the property changes, this Conditional Use Permit shall expire and a new Conditional Use Permit will be required. 3. The administrator shall approve the site layout. 4. All necessary permits shall be obtained from the appropriate State agencies before the food processing operation is started. REVIEW EVALUATIONS: Highway Department - There is an existing commercial entrance, however, it has not been maintained and we feel both entrance ways and the section between the existing edge of pavement and the face of curb between these entrances should be repaved with a minimum to two inches of S5 plant mix. Middletown Fire Company - No comment. Health Department - No objections. Inspection Department - Building must be brought into compliance with the Code requirements for the new use. A Certificate of Occupancy must be obtained before occupancy as grocery store. Planning and Zoning - Satisfactory; recommend that the applicant work with the staff on site details'.
