The road improvements shall include the following: Along the property frontage on U.S. Route 11, the applicant shall construct curb and gutter and storm sewer with a 12' or wider lane widening consistent with VDOT commercial entrance standards. The applicant shall dedicate an additional 14' of right of way along property frontage with U.S. Route 11. Two 38' or larger commercial entrances shall be installed in a ^.rirwed locations, one south and one north of the industrial access road intersection. (See 1 on the GDP). The southern ..,nimsrcial entrance shall be right in /out in accordance with VDOT requirements. b. The applicant shall construct an industrial access rood to VDOT standards with centerline intersection equal to the centerline intersection of Rest Church Rood and U.S. Route 11. The industrial access shall connect with Woodbine Rood. (See 1 on the GDP). The configuration and location of this connector road addition to the road system shall be adjusted to conform with actual proposed use of the site subject to Frederick County and VDOT approval at the site plan stop of development. The right of way width for this road shall be 80'. a The applicant shall modify the proposed VDOT stoplight from 3 way to 4 way Intimation. (See 3 on the GDP). d. The applicant shall enter into a signalization agreement invoMng partial cost shoring for the Woodbine Rood/Route 11 intersection. Establishment of the signalization agreement will correspond with development of the industrial (141) portion of the site, and will therefore occur prior to approval of the flat site pion for M1 development. The timing for ultimate signalizatkm of the intersection will be determined by VDOT. a The applicant agrees to provide a TIA for each industrial (M1) use at the tins of site pion. Additional sheet improvements suggested by the TIA will be negotiated with VDOT as a part of the land use permit. js ,y.eS_ n Fe The development of the subject property, and the submission of any Master Development Plan shall include the following landscape design features provided in these proffers and as illustrotad on the Generalized Development Plan dated March 8, 2004 which is attached hereto and incorporated herein by reference: A twenty foot (20') landscaped green strip along the U.S. Route 11 frontage portion of the sits. (See 5 on the GDP). Features within the landscaped green strip include a low berm (30' or lees) with suitable low profile landscaping that will not conflict with sight distance at entrances. On Site Development The applicant expects the industrial zoned portion of the site to develop with a minimum of 1 and maximum of 8 end users depending on marketing conditions at the time of sale. Covenants and restrictions of record shall control the standards of construction to include: a. Facade materials of building facing street to be concrete masonry (CMU) brick. architectural block, dry vit or other simulated stucco. real or simulated wood or gloss. b. All building within the development on the property shall be constructed using compatible architectural style and materials. and signage for such buildings shall be of a similar style and materials. c. M building within the property shall be developed as o cohesive entity, ensuring that building placement, architectural treatment, parking lot lighting, landscaping. trash disposal, vehicular and pedestrian circulation and other development elements work together functionally and aesthetically. Any discrepancies found between the drawings and site conditions or any inconsistencies or ambiguities in drawings shall be immediately reported to the engineer in writing, who shall promptly address such inconsistencies or ambiguities in writing. Work done by the contractor after his discovery of such discrepancies, inconsistencies or ambiguities, without clarification from the engineer of record, shall be done at the contractor's risk. 2. Contractor shall furnish all materials, labor and equipment required and shall perform all work including restoration for the completed installation of utility systems and all other improvements shown or implied as necessary. Unless otherwise noted, specifications for all work shall be in accordance with the applicable Standards and Contract Specifications of the Virginia Department of Transportation, Virginia Department of Health, County of Frederick, Frederick County Sanitation Authority, and the contract documents. 3. Contractor to verify the exact location of all existing utilities and notify the Engineer of any conflicts with existing and proposed utilities. Damage to existing utilities shall be the Contractor's responsibility. Forty -eight (48) hours prior to any excavation call "Miss Utility" 1 -800- 552 -7001. 4. Contractor shall be responsible for all construction stakeout. All dimensions and elevations shown on the contract documents shall be field verified by the Contractor prior to beginning work. 5. Contractor shall obtain, at his expense, all permits required prior to starting work and shall adhere to the conditions imposed. 6. Any unusual subsurface conditions encountered during the course of the work shall be brought to the attention of the Geotechnical Engineer Owner. 7. Access by emergency vehicles must be maintained at all times. Access to fire hydrants must not be impeded at any time by parked cars, construction materials, soil stockpiles, etc. 8. Refer to architectural drawings for exact building dimensions. CONSTRUCTION NOTES: EARTHWORK NOTES: 1. Erosion and sediment control measures shall be in place prior to performing any significant earth disturbing activities. This plan is incomplete without the Erosion Control Narrative and all addendums. A preconstruction conference shall be held prior to the issuance of the land disturbing permit. The contractor shall arrange the meeting with the County E SC Administrator. 2. All materials used for fill or backfill shall be free of wood, roots, rocks, boulders or any other non compactible soil type material. Unsatisfactory materials also include man -made fills and refuse debris derived from any source. 3. Satisfactory material for use as fill for public streets include materials classified in ASTM D -2487 as GW, GP, GM, GC, SW, SP, SM, SC, ML and CL groups. The moisture content shall be controlled within plus or minus 3 percentage points of optimum to facilitate compaction. Generally, unsatisfactory materials include materials classified in ASTM D -2487 as PT, CH, MH, OL, OH, and any soil too wet to facilitate compaction. CH and MH soils may be used subject to results of the Geotechnicat Report and upon approval from VDOT. 4. Materials used to construct embankment for any purpose, backfill around drainage structures or in utility trenches or any other depression requiring fill or backfilt shall be compacted in strict accordance with the requirements outlined in the Geotechnicat Report. All materials proposed for use as fill shall be approved by the geotechnical engineer prior to placement as controlled fill, and representative samples shall be obtained one week prior to placement of that material to allow time for completion of the necessary laboratory tests. Tests shall be conducted by a certified materials testing laboratory and the certifications made by a licensed professional engineer representing the laboratory. 5. All fill material compacted by heavy compaction equipment shall be placed in maximum 12 -inch loose lifts. All fill material compacted by hand- operated tampers or light compaction equipment shall be placed in maximum 6 -inch loose lifts. 6. Fill material placed below and extending ten(10) feet beyond the foundations for the structure shall be compacted to at least 100 percent of the laboratory maximum dry density as determined by the Standard Proctor method (ASTM D 698). Fill placed within the top one (1) foot of pavement subgrade areas shall also be compacted to 100 percent of the maximum dry density as determined by ASTM D 698. Fill placed below the top one (1) foot of pavement subgrade areas shall be compacted to not less than 95 percent of the maximum dry density as determined by ASTM D 698. The moisture content of the soils shall be at or within three (3) percentage points of the optimum moisture content. 7. Field density tests shall be performed in accordance with ASTMD 2922 (nuclear method). A minimum of three field density tests shall be performed for each lift of fill placed or a minimum of every 2,500 square feet of fill placed to confirm the required soil compaction. Trench backfill shall be tested at a frequency of not less than one test per lift per 250 lineal feet of trench. 8. Compaction tests for street pavement structure shall be made in cut and fill areas at the minimum frequencies prescribed by VDOT. 9. Changes or revisions to these construction plans shall not be made without approval by the owner, engineer and all applicable regulatory agencies. ROAD NOTES: 1. Subgrade on cut shall be compacted for a minimum of one (1) foot in depth. Subgrade in fill shall meet the compaction requirements stated above at all levels. 2. Areas of wet or soft material under roads or buildings shall be partially over- excavated and replaced with suitable material as directed by the Geotechnical Engineer. 3. Gravel used in base courses shall be constructed on a compacted subgrade free of organic matter. Base course aggregate shall be VDOT type 21 -B and shall be placed and compacted in a moist condition so that the fines remain mixed with the coarser material. 4. Asphaltic concrete, where applicable, shall be applied to the prepared gravel subbase following the application of a liquid asphalt prime coat. Asphalt shall consist of a plant -mixed bituminous material rolled smooth. The asphalt surface shall be smooth, seamless and of a uniform texture. Asphalt material shall comply with the minimum specifications required by the VDOT Road and Bridge Specifications. 5. Roadwork within existing or planned VDOT right of way shall conform to the latest editions of the Virginia Department of Highways Road and Bridge Specifications and Standards and Virginia E SC Regulations. VDOT General Notes vi. All work on this project shall conform to the current editions of and latest revisions to the Virginia Department of Transportation (VDOT) Road and Bridge Specifications and Standards, the Virginia Erosion and Sediment Control Regulations, and any other applicable state, federal or local regulations. In case of a discrepancy or conflict between the Standards or Specifications and Regulations, the most stringent shall govern. V2. All construction shall comply with the latest U.S. Department of Labor. Occupational Safety and Health Administration (OSHA), and Virginia Occupational Safety Health (VOSH) Rules and Regulations. V3. When working within VDOT right -of -way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition of VDOT's Work Area Protection Manual. Furthermore. all traffic control flaggers must be certified in accordance with Section 104.04(c) of the VDOT Road and Bridge Specifications. V4 The developer shall be responsible for relocating, at his expense, any and all utilities, including traffic signal poles, junction boxes, controllers, etc., owned; by VDOT or private public utility companies. It is the sole responsibility of the developer to locate and identify utility facilities or items that may be in conflict with the proposed construction activity. VDOT approval of these plans does not indemnify the developer from this responsibility. V5. Design features relating to field construction, regulations, and control or safety of traffic may be subject to change as deemed necessary by VDOT. Any additiona I expense incurred as a result of any field revision shall be the responsibility of the developer. V6. Prior to initiation of work, the contractor shall be responsible for acquiring all necessary VDOT land use permits for any work within VDOT right -of -way. V7. If required by the local VDOT Residency Office, a pre- construction conference shall be arranged and held by the engineer and /or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. V8. The contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT requires and shall receive 48 hours advance notice prior to any required or requested inspection. V9. The contractor shall be responsible for maintaining adequate access to the project from the adjacent public roadway via a construction entrance that is constructed and maintained in accordance with Section 3.02 of the Virginia Erosion and Sediment Control Handbook. Furthermore, access to other properties affected by this project shall be maintained through construction. The Developer shall have, within the limits of the project, an employee certified by the Virginia Department of Conservation and Recreation (VDCR) in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures on a continuous basis for proper installation and operation. Deficiencies shall be promptly rectified. V10. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. v11. All water and sewer Hines within existing or proposed VDOT right -of -way shall have a minimum thirty -six (36) inches cover and when possible shall be installed under roadway drainage facilities at conflict points. V12. Any unusual subsurface conditions (e.g., unsuitable soils, springs, sinkholes, voids, caves, etc.) encountered during the course of construction shall be immediately brought to the attention of the engineer and VDOT. Work shall cease in that vicinity until an adequate design can be determined by the engineer and approved by VDOT. V13. All fill areas, borrow nnaterial and undercut areas shall be inspected and approved by a VDOT representative prior to placement and fill. Where CBR testing is required, a VDOT representative shall be present to insure the sample obtained is representative of the location. When soil samples are submitted to private laboratories for testing, the samples shall be clearly identified and labeled as belonging to a project to be acceptedl by VDOT and that testing shall be performed in accordance with all applicable VDOT standards and procedures. V14. All roadway fill, base, subgrade material, and backfill in utility /storm sewer trenches shall be compacted in six (6) inch lilts to 95% of theoretical maximum density as determined by AASHTO T -99 Method A, within plus or minus 2% of optimum moisture for the full width of any dedicated street right -of -way. At the direction of VDOT, density tests shall be performed by a qualified independent agency in accordance with VDOT Road and Bridge Specifications. Certified copies of test reports shall be submitted to VDOT doily, unless specified otherwise. V15. VDOT Standard CD and UD underdrains shall be installed where indicated on these plans and /or as specified by VDOT. V16. The installation of any entrances and mailboxes within any dedicated street right -of -way shall meet VDOT minimum design standards and is the responsibility of the developer. V17. Prior to VDOT acceptance of any streets, all required street signage and /or pavement markings shall be installed by the developer or, at VDOT's discretion, by VDOT on an account receivable basis following the Manual On Uniform Traffic Control Devices. v18. The developer shall provide the VDOT Residency Office with a list of all material sources prior to the start of construc+• ;:m. Copies of all invoices for materials utilized within any dedicated street right -of -way must be provided to the local VDOT Residency OM:- prior to acceptance of the work. Unit and total prices may be obscured. V19. Aggregate base and subbase materials shall be placed on subgrade by means of a mechanical spreader. Density will be determined using the density control strip in accordance with Section 304 of the VDOT Road and Bridge Specifications and VTM -10. A ceroied compaction technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. In addition to checking stone depths, a VDOT representative shall be notified and given the opportunity to be present during the construction and testing of the density control strip. V20. Asphalt concrete pavements shall be placed in accordance with Section 315 of the VDOT Road and Bridge Specifications. Density shall be determined using the density control strip as specified in Section 315 and VIM-76. A certified compaction technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. A VDOT representati shall be notified and given the opportunity to be present during the construction and testing of the control strip. V21. In accordance with Section 302.03, the foundations for pipe culverts thirty -six (36) inches and larger shall be explore,a b.ILw the bottom of the excavation to dletermine the type and condition of the foundation. The contractor shall report findings of foundation exploration to the engineer and VDOT for approval prior to placing pipe. Foundation designs shall comply with VDOT Road and Bridge Standard PB -1. Where soft, yielding, or otherwise unsuitable foundation is encountered, the foundation design and /or need for foundation stabilization shall be determined by the engineer and approved by VDOT. V22. Approval of these plants shall expire three (3) years from the date of the approval letter. V23. VDOT Standard CG -12 Curb Ramps shall be installed where indicated on these plans and /or as specified by VDOT. V24. VDOT Standard Guardrail shall be installed where warranted and /or as proposed on these plans in accordance with VDOT's installation criteria. Final approval of the guardrail layout to be given by VDOT after grading is mostly complete. FCSA CONSTRUCTION NOTES: 1. The Authority's Water and Sewer Standards and Specifications are available at www.fcsa- water.com. 2. The water and sanitary sewer portion of these plans are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrange for inspection by, the Authority. 5. The contractor shall tie -in a new sewer line to an existing manhole by core drilling the manhole. 6. When an existing waiter main is tapped for the installation of a new line: a) The Authority shall) furnish all labor and material. b) The contractor must: 1. make application and pay deposit 2. mark the tap location. 3. coordinate installation with the engineering- assistant. 7. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's inspector. 8. The Authority's mainitenance department shall furnish and install all water meters through 2" in size. It is the contractor's responsibility to have the meter box assembly installed correctly. Before a permanent meter is installed: 1) The meter box„ with its frame and cover, must be properly aligned with the coppersetter. 2) the frame and cover to be set to final grade. 3) the distance between the top of the cover and the coppersetter shall be between 18 and 21 inches. 4) all components; of the meter box assembly shall be in proper working order. 9. For water and sewerr services that connect to existing lines: a) The location off the service lateral is to be coordinated with the Authority's Engineering Assistant. b) An Application ifor Service must be made and fees paid before work will begin. c) The Authority shhall furnish and install all 5/8 3/4" and 1" water services. d) For all 1 1/2"' and 2" water services, the Authority shall make the tap and bring the piping to the property line. The contractor shall furnish and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals. 10. Any water service (tine must have a backflow prevention assembly (double check valve or RPZ, as required.) The assembly must meet ASSE staindard No. 1015 or 1013. 11. All fire lines shall have a backflow prevention assembly (detector double check valve or RPZ, as required.) The assembly shall meet ASSE standard No. 1048 or 1047. Any exterior, privately owned fire line shall also have a fire service meter. This assembly shall be installed inmediately before the backflow prevention unit. Radio read remotes are required. 12.
