Z Please give me a call at (540) 535 -1832 when the pipe has been delivered at the
site. Soil moisture conditions will govern when the culvert is installed.
If rock is encountered which requires drilling and/or blasting, the entrance is to he
graded by you at no cost to the Virginia Department of Transportation.
You will be required to place crushed stone from the right -of -way to the edge of
the roadway.
All graded slopes are to be 1 44:1 or flatter, You will be required to topsoil and
reseed all disturbed turf on the State's right -of -way.
Z You will be required to flatten the slopes in a uniform manner and restore them to
the satisfaction of the Virginia Department of Transportation.
Z The ditch line will have to be restored.
Provisions will be made for siltation control within the State's right -of -way.

should this land parcel subdivide or develop commercially, this private entrance
permit will be invalid. The applicant wilt be required to apply for a new permit
which fits the circumstances appropriately.
Asphalt and/or concrete applications to entrance must be inspected by VDOT and
approved prior to installation In connecting to State roadway. When scheduling asphalt
and/or concrete installations, please notitr the VDOT Area Headquarters at the telephone
number listed below in advance for Inspection.
FINAL INSPECTION REOUIREMENTS: Upon use/occupancy of the property
served by the private entrance described under this permit. the petmittec shall
contact the Area Superintendent's Office to request an inspection. Applkant Is
required to present original permit at time of Inspection.
Other: Pipe size required to be 30' of 12" (minimum). Pipe should be either concrete or
metal.
This permit does not grant permission to grade on the property of others or disturb in anyway
utility poles or underground lines. Attached is a copy of a sketch of our standard private entrance
for your information.
Should you have any questions during construction and upon completion, please notify me at
(540) 535 -1832 so 1 may inspect this work.
