1. THE CONTRACTOR SHALL FURNISH ALL MATERIALS, LABOR AND EQUIPMENT AND

NECESSARY FOR THE COMPLETE FACILITY, READY FOR USE. UNLESS OTHERWISE
NOTED, SPECIFICATIONS FOR ALL WORK TO BE IN ACCORDANCE WITH APPLICABLE
STANDARDS AND CONSTRUCTION SPECIFICATIONS OF THE WRGINIA DEPARTMENT OF
TRANSPORTATION, THE CITY OF MNCHESTER PUBLIC U77LI77ES AND PUBLIC WORKS
DEPT. AND THE CONTRACT DOCUMENTS.
2. THE ENGINEER HAS ATTEMPTED TO LOCATE EXISTING SUBSURFACE UTILITIES, HOWEVER,
SUCH MAY EXIST THAT ARE NOT SHOWN. THE CONTRACTOR SHALL EXERCISE CARE IN
THIS WORK SO AS TO AVOID DAMAGE TO ANY UTILITIES. ANY DAMAGE SHALL BE
THE CONTRACTOR'S RESPONSIBILITY. THE CONTRACTOR SHALL NOTIFY ALL UTILITY
COMPANIES 72 HOURS PRIOR TO CONSTRUCTION. CALL "MISS UTILITY" 1-800-552-7001.
3. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL CONSTRUCTION STAKEOUT. ALL
DIMENSIONS, ELEVATIONS AND LOCA77ONS SHALL BE VERIFIED IN THE FIELD BY
EACH CONTRACTOR PRIOR TO BEGINNING THE WORK. THE CITY PUBLIC WORKS DEPT.
OR THE ENGINEER WILL APPROVE ALL CUT SHEETS.
4. ANY UNUSUAL SUBSURFACE COND117ONS ENCOUNTERED DURING THE COURSE OF THE
WORK SHALL BE IMMEDIATELY BROUGHT TO THE ATTEN77ON OF THE ENGINEER AND/OR
THE OWNER.
5. ALL GRADING FOR IMPROVEMENTS TO BE CONFINED TO THE PROJECT AREA. ANY WORK
PERFORMED OFFSITE IS THE CONTRACTOR'S RESPONSIBILITY.
6. CONTRACTOR TO OBTAIN, AT HIS EXPENSE, ALL PERMITS REQUIRED FOR THIS WORK
AND SHALL FAITHFULLY ADHERE TO THE PERMIT CONDITIONS IMPOSED.
Z ALL UNPAVED AREAS OF THE SITE WHICH ARE DISTURBED BY THIS CONSTRUCTION
SHALL BE PROVIDED WITH 4" TOPSOIL, FERTILIZED, SEEDED, COMPACTED AND
MULCHED. SEED BED TO BE WATERED AND CARED FOR AS NECESSARY TO PROVIDE A
GOOD LAWN QUALITY GRASS COVER. AREAS NOT GRADED SHALL BE DISKED LEVELED
AND SEEDED TO ACHIEVE LAWN CONDITIONS.
8. WATER AND SEWER LINES ARE TO BE CONSTRUCTED IN ACCORDANCE WITH THESE PLANS
AND SPECIFICATIONS AND SHALL ADDITIONALLY MEET ALL REQUIREMENTS OF THE
CITY OF WINCHESTER. THE CONTRACTOR SHALL COORDINATE WITH THIS AGENCY AND
ARRANGE FOR 1NSPEC77ON AS NECESSARY. EXACT LOCATIONS FOR WATER AND SEWER
SERVICES TO BE DIRECTED BY THE CITY OF WINCHESTER INSPECTOR IN THE FIELD.
CONTRACTOR TO PROVIDE ASSISTANCE AS THE INSPECTOR MAY REQUIRE.
8A. CONTRACTOR SHALL PROVIDE THE PUBLIC U77LITIES DEPT. 24 HOUR NOTICE
PRIOR TO TAPPING EXISTING WATER MAINS.
8B. PROPOSED WATER MAINS WILL REQUIRE TESTING IN ACCORDANCE WITH PUBLIC
U71LI7Y SPECIFICATIONS. THE CONTRACTOR SHALL SCHEDULE THESE TESTS AND
PROVIDE ASSISTANCE AS NECESSARY.
8C. SHOULD MAINTENANCE OF WATER OR SEWER LINES WITHIN EXISTING EASEMENTS
REQUIRE DISTURBANCE OF PROPOSED IMPROVEMENTS, THE OWNER IS RESPONSIBLE
FOR REPAIR OF THOSE ITEMS.
9. TEMPORARY TERMINATION OF SEWER LINES SHALL BE PLUGGED WATERTIGHT. THE
END OF THE LINE SHALL BE MARKED WITH A TREATED 2 INCH X 4 INCH POST
EXTENDING AT LEAST 18 INCHES ABOVE THE GROUND. THE TOP 6 INCHES SHALL
BE PAINTED GREEN, AND THE CUT DEPTH TO THE LINE INVERT SHALL BE SHOWN.
10. DESIGN FEATURES RELATING TO CONSTRUC71ON OR TO REGULATION, CONTROL AND
SAFETY OF TRAFFIC MAY BE SUBJECT TO CHANGE AS DEEMED NECESSARY BY THE
CITY OF WINCHESTER.
11. AFTER REMOVAL OF ALL TOPSOIL, THE SUBGRADE SHOULD BE PROOFROLLED WITH APPROVED
CONSTRUCTION EQUIPMENT PRIOR TO FILL REPLACEMENT. ALL SUBGRADE AREAS SHALL
BE STABLE PRIOR TO FILL PLACEMENT. ALL FILL AND BASE MATERIAL SHALL BE
COMPACTED TO 95% OF THE MAXIMUM DRY DENSITY AS DETERMINED BY
ASTM D 698. (STANDARD PROCTOR). LANDSCAPED AREAS SHALL BE COMPACTED TO
903' OF THE MAXIMUM DRY DENSITY AS DETERMINED BY THE SAME STANDARD.
COMPACTION TESTING SHALL BE PERFORMED BY A QUALIFIED GEOTECHNICAL ENGINEER
REGISTERED IN THE COMMONWEALTH OF VIRGINIA OR A QUALIFIED REPRESENTATIVE
UNDER DIRECT SUPERVISION OF THE ENGINEER.
12. ACCESS FOR EMERGENCY VEHICLES MUST BE MAINTAINED AT ALL TIMES. TEMPORARY
STREET SIGNS MUST BE POSTED AS CONSTRUCTION BEGINS. ACCESS TO FIRE
HYDRANTS IS NOT TO BE IMPEDED AT ANY TIME BY CONSTRUCTION MATERIALS, PARKED
CARS ETC..
13. MANHOLE RIMS, VALVE BOXES, ETC. TO BE ADJUSTED TO MATCH PROPOSED ELEVA7IONS
SHOWN IN THESE PLANS.
14. IF BLAS77NG IS REQUIRED THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING THE
NECESSARY FIRE DEPARTMENT PERMITS.
15. THIS PROJECT ENTAILS CONSTRUCTION OVER AND IN CLOSE PROXIMITY TO EXISTING
GAS MAIN. THE CONTRACTOR WILL NOTIFY SHENANDOAH GAS COMPANY (PHONE NO.
869-1111) BEFORE EXCAVATING NEAR THIS GAS MAIN.
16. TV INSPEC77ON OF ALL PLASTIC PIPE MUST BE PERFORMED PRIOR TO SITE
ACCEPTANCE BY CITY OF WINCHESTER.
EROSION AND SEDIMENT CONTROL NOTES:
1. All work shall conform to the standards and specifications and other
criteria adopted by the City of Winchester unless a variance has been
granted in writing.
2. No work shall commence on site until a Land Disturbance Permit is
issued by the City of Winchester.
3. The Land Disturbance Permit must be kept on site at all times and shown
on request.
4. A copy of approved Erosion and Sediment Control Plan must be kept on site
at all times and shown on request.. .
5. If the contractor fails to produce the Land Disturbance Permit and the
approved Erosion and Sediment Control Plan, a Stop Work Order will be
placed on job until such time that the Permit and the Plan are secured
on the job site.
6. The City must be notified when the work commences and when the project
is completed.
7. Other work (grading, excavating, construction) on the project shall not
commence until the appropriate erosion and sediment controls are in
place as specified on the plan.
8. Contactor is responsible for any and all damages to any other conservation
measures already in place as a result of work covered by this permit.
9. Contractor shall maintain the conservation measures in satisfactory
operating condition until final, permanent stabilization is achieved.
10. Contractor shall submit a monthly monitor report to the City indicating
the condition of the erosion and sediment controls and any corrective
actions taken during the reporting period. The report shall run on the
calendar month and must be recieved no later than the 5th day of the
following month. Copies of the report form may be obtained at the
address below.
11. All denuded areas on site shall be stabilized within 7 days of final
grading with permanent vegatation or a protective ground cover suitable -
for the time of the year.
12. All denuded areas that may not be at final grade but will remain dormant
for longer than 30 days must recieve temporary soil stabilization.
13. Permanent stabilization must be applied to all denuded areas that are
to be left dormant for more than one year.
14. The Land Disturbance Permit may be revoked, should the City determine
that the project is not in compliance with the conditions of the
approved plan.
15. All questions regarding Erosion and Sediment Contol must be directed

GENERAL CONSTRUCTION NOTES: FOR
1. THE CONTRACTOR SHALL FURNISH ALL MATERIALS, LABOR AND EQUIPMENT AND

NOTED, SPECIFICATIONS FOR ALL WORK TO BE IN ACCORDANCE WITH APPLICABLE

2. THE ENGINEER HAS ATTEMPTED TO LOCATE EXISTING SUBSURFACE UTILITIES, HOWEVER,
SUCH MAY EXIST THAT ARE NOT SHOWN. THE CONTRACTOR SHALL EXERCISE CARE IN
THIS WORK SO AS TO AVOID DAMAGE TO ANY UTILITIES. ANY DAMAGE SHALL BE
THE CONTRACTOR'S RESPONSIBILITY. THE CONTRACTOR SHALL NOTIFY ALL UTILITY
COMPANIES 72 HOURS PRIOR TO CONSTRUCTION. CALL "MISS UTILITY" 1-800-552-7001.
3. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL CONSTRUCTION STAKEOUT. ALL
DIMENSIONS, ELEVATIONS AND LOCATIONS SHALL BE VERIFIED IN THE FIELD BY
EACH CONTRACTOR PRIOR TO BEGINNING THE WORK. THE CITY PUBLIC WORKS DEPT.
OR THE ENGINEER WILL APPROVE ALL CUT SHEETS.
4. ANY UNUSUAL SUBSURFACE CONDITIONS ENCOUNTERED DURING THE COURSE OF THE
WORK SHALL BE IMMEDIATELY BROUGHT TO THE ATTENTION OF THE ENGINEER AND/OR
THE OWNER.
5. ALL GRADING FOR IMPROVEMENTS TO BE CONFINED TO THE PROJECT AREA. ANY WORK
PERFORMED OFFSITE IS THE CONTRACTOR'S RESPONSIBILITY.
6. CONTRACTOR TO OBTAIN, AT HIS EXPENSE, ALL PERMITS REQUIRED FOR THIS WORK
AND SHALL FAITHFULLY ADHERE TO THE PERMIT CONDITIONS IMPOSED.
7. ALL UNPAVED AREAS OF THE SITE WHICH ARE DISTURBED BY THIS CONSTRUC7ION
SHALL BE PROVIDED WITH 4" TOPSOIL, FERTILIZED, SEEDED, COMPACTED AND
MULCHED. SEED BED TO BE WATERED AND CARED FOR AS NECESSARY TO PROVIDE A
GOOD LAWN QUALITY GRASS COVER. AREAS NOT GRADED SHALL BE DISKED LEVELED
AND SEEDED TO ACHIEVE LAWN CONDITIONS.
8. WATER AND SEWER LINES ARE TO BE CONSTRUCTED IN ACCORDANCE WITH THESE PLANS
AND SPECIFICATIONS AND SHALL ADDITIONALLY MEET ALL REQUIREMENTS OF THE
CITY OF WINCHESTER. THE CONTRACTOR SHALL COORDINATE WITH THIS AGENCY AND
ARRANGE FOR INSPECTION AS NECESSARY. EXACT LOCA7IONS FOR WATER AND SEWER
SERVICES TO BE DIRECTED BY THE CITY OF WINCHESTER INSPECTOR IN THE FIELD.
CONTRACTOR TO PROVIDE ASSISTANCE AS THE INSPECTOR MAY REQUIRE.
8A. CONTRACTOR SHALL PROVIDE THE PUBLIC UTILITIES DEPT. 24 HOUR NOTICE
PRIOR TO TAPPING EXISTING WATER MAINS.
8B. PROPOSED WATER MAINS WILL REQUIRE TESTING IN ACCORDANCE WITH PUBLIC
UTILITY SPECIFICATIONS. THE CONTRACTOR SHALL SCHEDULE THESE TESTS AND
PROVIDE ASSISTANCE AS NECESSARY.
8C. SHOULD MAINTENANCE OF WATER OR SEWER LINES WITHIN EXISTING EASEMENTS
REQUIRE DISTURBANCE OF PROPOSED IMPROVEMENTS, THE OWNER IS RESPONSIBLE
FOR REPAIR OF THOSE ITEMS.
9. TEMPORARY TERMINATION OF SEWER LINES SHALL BE PLUGGED WATERTIGHT. THE
END OF THE LINE SHALL BE MARKED WITH A TREATED 2 INCH X 4 INCH POST
EXTENDING AT LEAST 18 INCHES ABOVE THE GROUND. THE TOP 6 INCHES SHALL
BE PAINTED GREEN, AND THE CUT DEPTH TO THE LINE INVERT SHALL BE SHOWN.
10. DESIGN FEATURES RELATING TO CONSTRUCTION OR TO REGULATION, CONTROL AND
SAFETY OF TRAFFIC MAY BE SUBJECT TO CHANGE AS DEEMED NECESSARY BY THE
CITY OF WINCHESTER.
11. AFTER REMOVAL OF ALL TOPSOIL, THE SUBGRADE SHOULD BE PROOFROLLED WITH APPROVED
CONSTRUCTION EQUIPMENT PRIOR TO FILL REPLACEMENT. ALL SUBGRADE AREAS SHALL
BE STABLE PRIOR TO FILL PLACEMENT. ALL FILL AND BASE MATERIAL SHALL BE
COMPACTED TO 95% OF THE MAXIMUM DRY DENSITY AS DETERMINED BY
ASTM D 698 (STANDARD PROCTOR). LANDSCAPED AREAS SHALL BE COMPACTED TO
90' OF THE MAXIMUM DRY DENSITY AS DETERMINED BY THE SAME STANDARD.
COMPACTION TESTING SHALL BE PERFORMED BY A QUALIFIED GEOTECHNICAL ENGINEER
REGISTERED IN THE COMMONWEALTH OF VIRGINIA OR A QUALIFIED REPRESENTATIVE
UNDER DIRECT SUPERVISION OF THE ENGINEER.
12. ACCESS FOR EMERGENCY VEHICLES MUST BE MAINTAINED AT ALL TIMES. TEMPORARY
STREET SIGNS MUST BE POSTED AS CONSTRUCTION BEGINS. ACCESS TO FIRE
HYDRANTS IS NOT TO BE IMPEDED AT ANY TIME BY CONSTRUCTION MATERIALS, PARKED
CARS ETC..
13. MANHOLE RIMS, VALVE BOXES, ETC. TO BE ADJUSTED TO MATCH PROPOSED ELEVATIONS
SHOWN IN THESE PLANS.
14. IF BLASTING IS REQUIRED THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING THE
NECESSARY FIRE DEPARTMENT PERMITS.
15. THIS PROJECT ENTAILS CONSTRUCTION OVER AND IN CLOSE PROXIMITY TO EXISTING
GAS MAIN. THE CONTRACTOR WILL NOTIFY SHENANDOAH GAS COMPANY (PHONE NO.
869-1111) BEFORE EXCAVATING NEAR THIS GAS MAIN.
16. TV INSPECTION OF ALL PLASTIC PIPE MUST BE PERFORMED PRIOR TO SITE
ACCEPTANCE BY CITY OF WINCHESTER.
EROSION AND SEDIMENT CONTROL NOTES:
1. All work shall conform to the standards and specifications and other
criteria adopted by the City of Winchester unless a variance has been
granted in writing.
2. No work shall commence on site until a Land Disturbance Permit is
issued by the City of Winchester.
3. The Land Disturbance Permit must be kept on site at all times and shown
on request.
4. A copy of approved Erosion and Sediment Control Plan must be kept on site
at all times and shown on request.
5. If the contractor fails to produce the Land Disturbance Permit and the
approved Erosion and Sediment Control Plan, a Stop Work Order will be
placed on job until such time that the Permit and the Plan are secured
on the job site.
6. The City must be notified when the work commences and when the project
is completed.
7. Other work (grading, excavating, construction) on the project shall not
commence until the appropriate erosion and sediment controls are in
place as specified on the plan.
8. Contactor is responsible for any and all damages to any other conservation
measures already in place as a result of work covered by this permit.
9. Contractor shall maintain the conservation measures in satisfactory
operating condition until final, permanent stabilization is achieved.
10. Contractor shall submit a monthly monitor report to the City indicating
the condition of the erosion and sediment controls and any corrective
actions taken during the reporting period. The report shall run on the
calendar month and must be recieved no later than the 5th day of the
following month. Copies of the report form may be obtained at the
address below.
11. All denuded areas on site shall be stabilized within 7 days of final
grading with permanent vegatation or a protective ground cover suitable
for the time of the year.
12. All denuded areas that may not be at final grade but will remain dormant
for longer than 30 days must recieve temporary soil stabilization.
13. Permanent stabilization must be applied to all denuded areas that are
to be left dormant for more than one year.
14. The Land Disturbance Permit may be revoked, should the City determine
that the project is not in compliance with the conditions of the
approved plan.
