At this time, administrative approval cannot be granted. This site plan is denied until the issues in this letter, as well as all issues of the other review agencies, have been adequately addressed. Please review Staffs comments listed below and then prepare a revised site plan which adequately addresses each concern. Review Comments ✓ 1. Special Limited Power of Attorney Form. Please provide Special Limited Power forms for each property owner. This form should be notarized and completed to grant the applicant (you) with authorization to represent them for this site plan. 2. Project Information. Revise the following on the coversheet: • Waterline — remove the BRL's as the waterlines will be underground. • Booster Pump Station — indicate if this parcel has been created (if not the site plan cannot be approved until the plat is recorded); the zoning of this property is B2 not RA; the setbacks shown on the coversheet don't match what is shown on sheet C401 (provide the minimum shown on the plat as well as the actual); provide the height of the pump station structure. \• Water Tank — provide only the PIN for the tank; provide the actual setbacks for the structure; provide only the acreage for the tank parcel. 3. ROW Easement. On sheets C202 and C401 provide a legal reference for the ROW. 4. Light Detail. Ensure the light detail for the booster station shown on sheet C421 complies with the outdoor lighting requirements. Proposed Landscaping. The landscaping plan for the tank and the pump station are listed as "proposed". This is your authorization from the State Health Commissioner to construct additions to the subject waterworks in accordance with the approved documents. Any deviations from the approved documents affecting capacity, hydraulic conditions, operating units, the functioning of the treatment processes, or the water quality delivered, must be approved by this Office before any such changes are made. Revised plans and specifications shall be submitted to the Lexington Field Office in time to permit review and approval before construction. Upon completion of construction, the owner shall submit a statement signed by a professional engineer licensed in Virginia certifying that the work was completed in accordance with the approved documents. Upon receipt of this statement, satisfactory results of bacteriological analyses, and final inspection by an Office representative, the State Health Commissioner will amend the Authority's waterworks operation permit in accordance with the Regulations.
