As discussed over the phone and email an owner's signature block has been added to the cover sheet in order to prevent the necessity of' tile limited power of attorney. 2. A signature block has been added to the cover sheet for the Zoning Administrator as requested. 3. The requested notes have becn added to the cover sheet. 4. The project scope has been revised to include installing the new exit onto Zachary Ann Road and the relocated truck scale. 5. The approved site plan number has been added to the cover sheet. 6. Per our phone conversation no striping is proposed. 7. A new island has been shown as requested in the bay adjacent to the pump/canopy addition. The permit is issued by this office and will require a minimum processing fee, surety bond coverage, and the salary & expenses of a State assigned Inspector.
