All work on Regency �Lokes Drive shall conform to the latest editions of the Vrginia Deportment of Transportation (VDOT) Rood and Briidge Specifications, and Standards, The Virginia Erosion and Sediment Control Regulations and any other state, federal or loco/ regulations applicable. In the event of conflict between any of these standards, specificotions or p1cros, the most stringent shall govern. 2. All construction on Reegency Lakes Drive shall comply with the latest U.S. Deportment of Labor Occupational Safety & Health Administration and KVSH Rules and Regulations. J. When working on VDOT right-of-way, all traffic control, whether permanent or temporary, shall be in accordance with the current editiion of vDOTs work area protection manual Furthermore, all traffic control /loggers must be certified in accordance with V/DOT's January, 1994 (or latest edition of ) Road & Bridge Specifications, Section 104.04-CC, 4. Design features relotlioq to construction or to regulation, control and safety of traffic moy be subject to change as deemed necessary by VDOT. 5. Prior to initiation of iwori, Contractor shall be responsible for acquiring all necessary VDOT land use permits for any work on VDOT right of -way. 6. if required by the /oc;ol VDOT Residency Office, a preconstruction conference must be arranged and held by the engineer ands or developer witch the attendance of the Contractor various County agencies, utility companies and VDOT prior to initiation of work. 7. Contractor shall notif* the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. V0071 will also require 48 hours notice for any inspection. 8. The Contractor will be responsible for maintaining adequate access to the project from the adjacent public roadway through construction and maintenance of a construction entrance in accordance with the Virginia Erosion & Sediment Control Handbook, Sec. 3.02. Furthermore, access to other properties affected by this project sholl be maintained through construction. 9. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. 10. AIi water and sewer lines within existing or proposed VDOT right-of-way ore to have minimum 36" cover and, when possible, to be installed under roadway drainage facilities. 11. Any unusual subsurface conditions encountered during the course of construction shall be immediately brought to the attention of the engineer and VDOT Work shall cease in that vicinity until an adequate design can be determined by the engineer and approved by VDOT. 12, All undercut areas a7nd borrow material shall be inspected and approved by VDOT Inspector prior to placement of fill. 1,J All roadway fill, bOs& subsurface material and bockfill of utility/storm sewer trenches shall be compacted in 6" lifts to 95X of theoireticol maximum density as determined by AASHTO T-99 Mehtod A, within plus or minus 2% of optimum moisture for the full) width of any dedicated street right-of-way. At the direction of VDOT Inspector density tests performed by a qualified independent` agency sholl be conducted as required in the VDOT Road & Bridge Specificitions. A copy of all tests shall be submitted to VDOT prior to final VOOT approval. 14. VDOT Standard CD rand LID underdroins shall be installed where indicated on these plans and further where determined necessary by VDOT /Inspector. 15. The installation of otny entrances and mailboxes within any dedicated street right-of-woy sh011 mee° VDOT minimum design stanndords and is the developer's responsibility. 16, If required by the local VDOT Residency Office, copies of all invoices for materials within any dedicated street right-of-way must be provided to the VDOT Inspector prior to acceptance of work. Unit and total prices moy be obscured. 17. Prior to acceptance by VDOT of any streets, any required street signage and` or pavement markings must be installed by the developer or; at VDOT's discretion, by VDOT on on account receivable basis. 18, VDOT approval of OpIs Site Plan expires 3 years from dote of opprovol. CONSTRUCTION NOTES GENERAL 1. The contractor shall furnish all materials, labor and equipment and perform all work including restoration for the completed instollotion of the futility systems and all other improvements shown or implied as necessary for the completed facility ready for use. Umless otherwise noted, specifications for a// work to be in accordance with applicable Standards and Construction Sjpecificotions of The virginio Deportment of Tronsportotion, Irginia Dept, of Health, Frederick County Sanitation Authority, and the contract documents. 2. The engineer has ottempted to locate existing subsurface utilities; however such moy exist that ore not shown. The contractor shall exercise core in this work so as to avoid dcmoge to any utilities. Any damage shall be the contractor's responsibility. The Contractor sholl notify all utility companies 72 hours prior to construction. Coll "Miss Utility" 1-800-257-7777. J. The contractor sholll be responsible for a// construction stakeout. All dimensions, elevations and locations shall be field verified by each contractor prior to beginning the work. 4. Any unusual subsurtfoce conditions encountered during the course of the work shall be immediately brought to the attention of the Engineer cnd� or tMe Owner. 5. A// grading for imprrovements to be confined to the project area. Any work performed offsite is the Contractor's responsibility. 6. Contractor to obtciin, at his expense, all permits required of this work and shall faithfully adhere to the permit conditions imposedl. 7. All fill, base and subsurface material shall be compacted to 95X of theoretical maximum density as determined by AASHO-T-99 reth�od A, within plus or minus 29 of optimum moisture for the full width of any dedicated right-of-woy. Compaction in otheer areas to be 90% Independent compaction test results moy be required for verification at direction of I/DOT Inspector. 8. Cutsheets shall be submitted to the Engineer for approval prior to construction. All cutsheets should show existing ground elevations lot centerline of utility, structure or road, and offset hub elevation. 9. /t is the contractor's responsibi/ity to meet compliance requirements with SEC 59. 1-406, ET SEO. of the Code of Virginia (Overhead High Voltage Lines Safety Act) 10. Areas which are subject to standing water due to lake levels shall be stabilized with "Quarry Waste" aggregate to secure a stable subbase lorior to building fill. Proof rolling will be required. TROL 11�OTemrpJoraarry and permanent erosion control structures and methods to be in accordance with the Vrginic Erosr'on Control Guidelines and local ordinances and to be utilized during the construction period. 72, All erosion control structures shown or as required during construction shall be continuously maintained throughout the period in which areas ore denuded. Areas shown to be surfaced ore to have stone base installed at the earliest possible dote after estoblishing grade. No area is to remain denuded for more than 30 calendar days when construction is not in progress. 73. Contractor to clean-up and restore disturbed areas immediate/y upon completion of pipe -laying in the affected area. 14. All unpaved areas of the site which ore disturbed by this construction shall be provided with 4" topsoil, fertilized, seeded, compacted and miulched. Seedbed to be watered and cored for as necessary to provide a good lawn quality gross cover. Areas not 9roded shall be disced, leveled and seeded to achieve /own conditions. 15. Vegetation Plan: All' gross areas to be seeded with o mixture of Marion Kentucky Bluegrass at a rote of 2-3 /bs per 1,000 sf and Anuuo/ Rye at a rate of 1 /b per 1,000 sf. New/v seeded areas shall be mulched at a rate of 70- 720 lbs. per 1,000 sf. Adequate watering and fertilization ore required to aid the emergence of new gross. FRE and RESCUE 76. Burning of land clearing debris requires a permit from the Fre Morshol's Office. Burning of construction debris on site is prohibited. 17. Access for emer9erncy vehicles must be maintained at a// times. Temporary street signs must be posted as construction begins. Access to fire hydtronts is not to be impeded at anytime by construction materials, parked cars, etc. FREDERCK COUNTY 8AN1TATION AUTHORfTY 18. Water and sewer /lines ore to be constructed in accordance with these plans and specification and shall additionally meet all requirements o)f the Frederick County Sanitation Authority. The contractor shall coordinate with this agency and arrange for inspection by the Sanitation Authority as necessary. 19. Exact locations for sewer and water services to be directed by the Frederick County Sanitation Authority Inspector in the field. Contractor to provide assistance as the Inspector moy require in this effort. 20. The Sanitation Authpority maintenance dept. is responsible for the installation of water meters. Before a permanent meter /s set, they require 1) the frame and cover to be properly aligned with its meter box and yoke bar, 2)the frame and cover to be set to the final grade of the lot, 3)the distance between the top of the cover and yoke bar be between 18 and 21 inches, and 4) a// components of the meter box ossembly to be in proper working order. It is Developer's Contractor's responsibility to hove the meter box ossembly installed correctly. 27. The Frederick County Sanitation Authority shall install al/ water and sewer services that connect to existing lines The location of tihe top and the termination point of the lateral ore to be coordinated with the Authority's ossistont engineer. 22. The Frederick County Sanitation Authority she/l top o// existing waterlines. They wit/ furnish the materials for and make the top.
