All work on this project shall conform to the latest editions of the Virginia Department of Transportation (VDOT) Road and Bridge Specifications, and Standards, The Virginia Erosion and Sediment Control Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards, specifications or plans, the most stringent shall govern. V2. All construction shall comply with the latest U.S. Department of Labor Occupational Safety & Health Administration and VOSH Rules & Regulations. V3. When working on VDOT right of way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition of VDOT's work area protection manual. EROSION AND SILTATION CONTROL NARRATIVE Project Description The developer proposes to build a dental office on approximately 35,000 square feet of an existing 2.93 acre parcel The total area of disturbance on the parcel is approximately 0.50 acres. Storm water runotf will discharge to an existing detention facility via existing drainageways. Site grading will include the importation of fill material to bring the site Lip to the proposed grades. Existinci Site Conditions The site is currently vacant. Slopes range from 1.0 %, to approximately 12.0%. diacent Areas The site is bordered on the north by the Home Plate recreational development. To the east is the Warrior Drive right-of-way. The area to the west is residential land. The land to the south is the remainder of the parent parcel which is currently vacant. Off -site Areas With the exception of borrow operations, no off site disturbance or conStrUCtion activities are anticipated. Critical Areas The are no critical areas identified on this site. Erosion and Sediment Control Measures 102 A 12'w by 70'1 temporary stone constrLiCtion entrance will be constructed frorn Warrior Drive. The entrance shall be maintained in a condition which will prevent tracking or flow of mud onto the public rights -of -way. This may require periodic top dressing with additional stone or the washing arid reworking of existing stone as conditions demand and repair and/or cleanout of any StrLICtUres used to trap sediment. All materials spilled, dropped, washed, or, tracked from vehicles onto roadways or into storin drains must be removed immediately The use of water trucks to remove materials dropped, washed, or tracked onto roadways will not be permitted under any circumstances. 3.05 Silt fence will be installed in selected locations downstrearn from the construction area as a first measure of construction. Silt fences shall be inspected after each rainfall arid at least daily during prolonged rainfall Any required repairs shall be inade immediately Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should be removed after each storm event. They must be removed when deposits reach approximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade arid stabilized. 3.31 Temporary seeding in accordance with the E & S Std & Spec 3.31 will be applied to the topsoil stock pile arid all areas which will not be brought to final grade within 30 clays. Embankment or excavated slopes denuded for a period of greater than 30 days shall be temporarily seeded and mulched. To coincide with the anticipated construction dates, the seeding will consist of 50 pounds per acre of German Millet. All perimeter sediment control devices shall be erected prior to any land disturbing activities and shall remain in place until the site is fully stabilized 3. The contractor shall perform overlot graiding to provide positive drainage and preclude ponding of water. All off site grading and construction is to be done with the property owner's consent. 4. Cut and fill slopes shall be graded at a miaxiMUm of 2 horizontal to 1 vertical unless otherwise specified on the plans. 5. The limits of clearing and grading are restricted to the limits of the parcel. The CONTRACTOR shall be responsible for determining the exact location of all utilities before commencing work and for any damages which occur by the failure to locate and preserve these utilities. If the CONTRACTOR should encounter utilities other than those shown on these plans, the CONTRACTOR shall immediately notify the engineer arid take the necessary and proper steps to protect the facility and assure the continuance of service. 2. The CONTRACTOR shall have approved construction drawings prior to the start of construction. At least one copy of approved plans and revisions shall be kept on -site at all times 3. Cut and fill slopes shall riot exceed 2-horizontal to 1-verlical unless otherwise called for on the plans, 4. Controlled Fills� A. Controlled compaction shall occur in all till sections for pavernent, trenches for utilities and in other areas as designated on the plans, B Controlled tills shall be compacted to 95% density as determined by methods per standard proctor ASTM D 698. Density shall be verified by a qualified soils engineer. C. Controlled fills shall be compacted in 8 inch lifts (loose thickness) to the specified density, beginning from the existing ground Surface, unless othierwise approved in writing by a qualified soils engineer. D. The Surfaces directly beneath areas to receive controlled fill are to be cleared, grubbed, and stripped of topsoil. The area is then to be scarified anid COMpacted to a depth of 6 inches to the same density as the Controlled fill to be placed thereon. E Fill material shall be taken from a1pproved borrow areas. It shall be clean soil, free of roots, woody vegetation, oversized stories, rocks, sandy soil, or other objectionable material. 5. Blasting is not permitted within 25 feet of existing utilities without owner permission. 6. Pavement materials called for on the plans refer to standard VDOT designations. 7 BENCHMARK: corner of existing concrete slab on the property ot the Horne Plate - elevation = 746.95. 8 All radii indicate edge of gravel, edge of pavement or face of curb as applicable, 9. Fire lanes shall be delineated as directed by the Frederick County Fire Marshal. 10. The CONTRACTOR shall obtain permission from adjoining properly owners prior to conducting any off site construction activities. 11 The CONTRACTOR shall coordinate the relocation of the existing utilities with the appropriate utility company as necessary. 12. All site arid building lighting shall be Installed to avoid spill -over onto adjoining properties. 13. Remove trees, shrubs, grass and other vegetation, improvements, or obstructions as required to permit installation of new construction. Remove similar items elsewhere on site or premises as specifically indicated. "Removal" includes digging out and off -site disposing of stumps and roots. Cut minor roots and branches of ti-ees indicated to rernain in a clean and careful mariner, where such roots arid branches obstruct installation of new construction. 14 Topsoil is defined as friable clay loam surface soil found in a depth of riot less than 4 inches. Satisfactory topsoil is reasonably free Of Subsoil, clay lumps, stones, and other objects over 2 inches in diameter, and without weeds, roots, and other objectionable material. Construct storage piles to provide free drainage of surface water. Cover storage piles, if required, to prevent wind erosion. 15. FREDERICK COUNTY SANITATION AUTHORITY NOTES 1 All sanitary sewer arid water supply constrUIC111011 shall conform to the current standards of the Virginia Department of Health and the Frederick County Sanitation Authority (the "authority"). 2, Water service will be provided by an extension of the Frederi(A County Sanitation Authority water main located adjacent to the Warrior Drive right-of-way. 3. The CONTRACTOR shall coordinate with, and arrange for inspection by, the Authority 4. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's inspector. 5. The Sanitation Authority shall install all water and sewer services that Connect to existing lines, The location of the tap and the termination point of the lateral are to be coordinated with the Authority's assistant engineer. An application for services Must be made and fees paid. 6. The Authority shall tap all existing water lines. They will furnish the materials for and make the tap. The CONTRACTOR must make application for and pay the cost of this work. 7. The Sanitation Authority maintenance department is responsible for the installation of water meters. Before a permanent meter is set, they require: 1) the frame arid cover to be properly aligned with its meter box and yoke bar ' 2) the frame arid cover to be set to the final grade of the lot-, 3) the distance between the top of the cover arid the yoke bar be between 18 and 21 inches, 4) all components of the meter box assembly to be in proper working order, It is the developer's responsibility to have the meter box assembly installed correctly.
