The checklist below indicates all the information --that needs to be submitted as part of the site plan
application. All required information must. be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).

Dear Austin:
The Frederick County Planning Department administratively approved the above referenced site plan,
#049-98, on December 2, 1998. This site plan, for the construction of a 3,789-square-foot addition
to the existing church, meets all County requirements for development within the RA (Rural Areas)
Zoning District. The site plan also calls for the provision of twenty-one (21) additional parking spaces
bringing the total number of parking spaces to seventy-three (73). The addition brings the total
seating at the church to two hundred and twenty (220).
I have provided you with several copies of the approved site plan. Please ensure that two approved
copies are forwarded to the property owner, and advise them to keep an approved copy for their files
and maintain an approved copy on the construction site. Once the site development has been
completed, the owner should contact this office to schedule a final site inspection.

We have reviewed the preliminary site plan dated July 15, 1998 for the referenced project.
VDOT comments are as follows and as shown in red on the enclosed Plan Sheet 2 of 3:
1. The need for overlayment of the existing asphalt from Route 50 edge of pavement to the
right-of-way should be considered and provided if existing is raveling, peeling, etc.
2. A right turn taper 12' wide from the existing Route 50 pavement and 150' long tying back to
the existing edge of pavement is needed. The taper pavement should match existing
Route 50 pavement for depth and slope.
3. The existing guardrail along the taper should be relocated an appropriate distance to meet
the taper widening as shown.
4. A note should be added to the plan to absolve VDOT of liability and maintenance of the
stormwater management pond.
All miscellaneous notes in red should be addressed.
When all of the above comments have been addressed, please submit one set of revised plan to
us for review. If approvable, VDOT will request additional plans for distribution.

project. It appears all of our earlier review comments have been satisfactorily
addressed. Please advise the owner of our approval.
The appropriate land use permit may now be applied for to cover work within the
Route 50 right-of-way. The permit is issued by this office and will require a minimum
processing fee, surety bond coverage, and the salary & expenses of a State assigned
Inspector. Please provide seven sets of approved construction plans with the seal
signed for VDOT distribution.
Once satisfactory application has been made, a permit will normally take approximately
thirty days to process and issue. Should you have any questions, do not hesitate to
call.

Dear Austin:
I have had the opportunity to review the above referenced site plan. The following comments should
be addressed during the revision of this site plan:
1) Please note the variance approval number and provide a description of the variance on the site
plan. You may wish to copy the actual approval letter onto the site plan.
2) The parcels owned by the Hayfield Assembly of God Church should be consolidated in
conjunction with this site plan. This lot consolidation must occur prior to final site plan
approval. Please ensure that the property information and acreage on the final site plan are
consistent with the lot consolidation plat.
3) Please consider providing circulation around the rear of the building. Individuals using the
facility would then have the ability to maneuver around the site in a safe manner and would
not have to contend with the Route 50 traffic.
Please revise your site plan to reflect the comments specified in this letter and, as all approved review
agency comment sheets have been received, submit five copies of the final site plan for approval.
Final site plan approval may occur when the recorded lot consolidation plat has been received.

Dear Mark:
Upon review of the subject site plan, we offer the following comments:
Place a note on the plan that a land disturbance permit will be required prior to
commencing grading operations.
2. The stormwater calculations do not include a 100- year storm analysis. Provide
this analysis for our review. We recommend the placement of an emergency
spillway to pass the proposed 100-year storm without overtopping the
embankment. Provide a detail of the spillway and place a note on the plan that the
stormwater pond shall be installed as a first step in the grading process. Install a
rip -rap berm on the inlet side of the 18 inch culvert to contain erosion and silt on -
site. Also, the stormwater pond needs to be placed within a dedicated drainage
easement.
3. A construction entrance may be required to ensure that mud is not tracked onto
the State Route 50 or the right-of-way which runs through the church's parking lot
leading to the adjacent property owners. It is imperative that this area be free from
obstructions during the site development. Place a note on the site plan indicating
these requests.
