Permit No. 2o69378 with an effective date of August 21, 2002. This permit is your authorization from the State
Health Commissioner to operate the public waterworks set- ing the Redland United Methodist Church, located
in Frederick Count\', in accordance with Part II of the WC1tC1-W07-L,; Regulations. You Will note that the permit
indicates that this water supply has a design capacity of 24,48o gallons per day. This permitted design capacity
is the maximum system capacity based on Part I I I of the Regulations and shall not be exceeded.
This amended waterworks operation permit replaces and nullifies Permit No. 2o69378 dated June 6, 1997;
therefore, it is requested that Operation Permit No. 2o69378 dated June 6, 1997 either be destroyed and this
office advised of its destruction or returned to this office.

The source of supply for this system consists of a drilled well, two 12o-gallon hydropneuniatic tanks,
three Zoo -gallon chlorine contact tanks, chlorination facilities, and ion exchange softening facilities.
Tile well is located approximately' 27o feet south of the church sanctuary. The well is drilled to a depth of
400 feet, cased to a depth of 105 feet with 6 25-inch steel casing, and is cement grouted to a depth Of 105
feet. The extended well casing terminates approximately 12 inches above a 6-foot by 6-foot by 6-inch
concrete slab and is equipped with a pitless adapter, sanitary seal, screened casing vent, sample tap, and
drawdown gauge. Water is pumped from the well by means of a 5 lip, 28 gpnl at 338 feet TDH
submersible pump through a flow regulating orifice plate and ion exchange Nvater softener into three
Zoo -gallon chlorine contact tanks. From the chlorine contact tanks, water enters the distribution
system. Two 12o-gallon hydropneuniatic tanks maintain system pressure and control well pump
operations. The extended well casing is unprotected while the hydropneumatic tanks, chlorine contact
tanks, water softener, chlorination equipment, and exposed discharge piping are located in the
mechanical room on the east side of the church sanetuai7•. Reported well Meld, based on a 48-hou1' Meld
and drawdown test, is 54 gpm. Total water production is metered.
Treatment facilities consist of chlorination and ion exchange softening. As water is pumped from the
well, it flows through a single, 16-inch diameter, ion exchange water softener and has a chlorine solution
injected into the softener effluent prior to entering the chlorine contact tanks. The chlorine feed system
comprises an 18 gpd at 150 psi metering pump and 35-9a11011 solution tank. The water softener has a
rated capacity of 17 gpni, an exchange capacity of 6o kilograins, and is provided N%ith influent and
effluent sample taps. Softener backwash is time clock controlled while the chlorine metering pump
operation is synchronized with the well pump.

The system design capacity is storage limited; however, since this is a nontransient noncommunity water
system, excess source capacity has been substituted for storage capacity. System design capacity will,
therefore, be 24,48o gpd (17 gpnl x 1440 minutes/day).
Note is The well pump is rated at 28 gpnl; however, the well is equipped with an orifice plate flow
controller that restricts potable flow to 17 gpm.

Dear Mr. Cannon:
The Frederick County Planning Department administratively approved Site Plan #023-95 for
Redland United Methodist Church on September 6, 1995. The approved site plan calls for the
construction of a new church facility that is 11,800 square feet, the creation of a new asphalt
parking lot and maneuvering area that will contain curb and gutter, 77 regular parking spaces,
four disabled parking spaces that are van accessible with appropriate ramps and signage, raised
islands, parking lot lighting, and concrete sidewalks to provide access from the parking lot to
the facility, the creation of a children's play area that is protected by a chain link fence that is
four feet in height, -and all appropriate erosion and sedimentation control measures.
I have provided you with two copies of the approved site plan. This will allow you to provide
the contractor with an approved site plan to maintain on -site and to maintain an approved copy
for your records. Please contact this office to schedule an on -site inspection once all
improvements are complete. Please contact me if I may answer any questions regarding this
letter.

It appears all of our earlier review comments have been satisfactorily addressed. Please
advise the owner of our approval.
A land use permit may now be applied for to cover work within the Route 693 & 522 rights
of way. The permit will require a minimal processing fee and surety bond coverage. A
permit of this nature normally takes approximately 30 days to process and issue once
satisfactory application has been made. Therefore, I would suggest application be made
as soon as practicable.

Dear Mr. Westermeyer:
As requested, we have reviewed the referenced pro*ect's site plan dated June 7, 1995. Our comments
may be found on the enclosed plans marked in red and as follows:
1.) The entrance width on Route 693 will need to be increased to a minimum 30'.
2.) The.half typical section of the proposed entrance with pavement design noted will need to be
incorporated into the plan. Also, there is some confusion over the pavement design for the
access roadway shown on Sheet 5-C.
3.) To improve the life of the entrance culvert, we suggest it be asphalt coated. Also the ends of
the pipe should be mitered to conform to the fill slope.
4.) The drainage calculations provided indicated that Culvert No. 3 is to be installed under the
Route 522 entrance. However, no entrance is shown within the Route 522 right-of-way.
5.) During earlier discussions you indicated additional grading on the Route 522 right-of-way may
be desirable to eliminate existing tractor trailer parking in front of the church property. If the
church still intends to do so, this grading will need to be shown on the plan.
6.) Other miscellaneous items as marked will need to be addressed.
Please revise and resubmit six (6) copies for final approval. Should any changes be deemed
necessary, please design them to meet or exceed the above recommendations.

In accordance with your request of June 2, 1995 you are given permission to install a temporary
construction entrance at the reference location. This letter is to be considered your permit to do so. The
following provisions will be required:
Work area protection signage in accordance with the attached while working on the right-of-way will
be required.
All work on the right-of-way is to be confined to the temporary entrance location only.
A minimum 6" depth of VDOT #1 (2-3 inch stone) course aggregate by 30' in width will be required.
It is to be placed from the edge of pavement and extended back onto private property far enough to
prevent the carrying of mud onto the roadway.
Positive drainage is to be maintained within the right-of-way at all times.
Any disturbed areas of the right-of-way or damages to the pavement structure adjacent to this entrance
will be repaired by you to our satisfaction.
This permit is being issued for 180 ,la.,s. Aland use perr_r.it must be applied for within the allotted tiz-ac.
All work is to be completed in accordance with the enclosed Virginia Erosion & Sediment Control
Handbook, Specification 3.02.
The Department of Transportation will not be held liable for suit should such result from this operation nor
is it granting permission to grade on the property of others or disturb underground utility lines. If the above

I have had an opportunity to review the above referenced site plan, received by our department
on August 1, 1995. The majority of the comments from my June 26, 1995 review have been
addressed; however, there are a few minor comments that need to be addressed to satisfy the
requirements of the Planning Department. Please include the following information on this site
plan:
1) Please revise the signature block to read Zoning Administrator. (Sheet C-1)
2) Please revise the information provided under #5 of the General Notes to remove 19-A-
13A, as this parcel has been consolidated into parcel 19-A-13B. (Sheet C-1)
3) Please revise the parcel boundary information on sheet C-2 and sheet C-3 to remove the
boundary line and note that were incorporated during the consolidation of the two
properties. Thank you for providing our department a copy of the approved, recorded
consolidation plat.
4) In conjunction with comment number 3, please also remove the notes of sheet C-2 and
sheet C-3 that read "Parcel A" and "Parcel B".
5) Please provide a description of ]low the outdoor trash area will be enclosed.
6) Please provide the name Cross Junction Road for Route 693. (Sheets C-2 & C-3)
Please provide our department with approved review agency comment sheets from the agencies
specified in my June 26, 1995, and five copies of the final site plan that address the comments

Dear Mr. Wyatt:
I have incorporated the comments that your office has made on the Preliminary Site Plan, and I
am resubmitting them for final approval. Following is a brief description of how each comment
was addressed.
1) The phone number for the owner/developer was included with his address on sheet C-1.
2) All spelling and spacing errors have been corrected as noted on sheet C-1.
3) The USGS Gore Quadrangle Map on sheet C-1 has been labeled as the Location Map.
4) The adjoining property information has been changed to the names that were recorded on
the consolidation plat dated 7/17/95. This information I believe is current.
5) A copy of the recorded consolidated plat is attached to this letter.
6) The only area specifically designated for recreation is the children's play area noted on
sheet C-3. A fence has been added to increase safety for the children.
7) The only outdoor storage area included in this project will be for the trash receptacles.
This area has been added and designated on sheet C-3.
8) See comment number 7 above.
9) A detail of the lighting fixture has been added to sheet C-5.
10) The names of the adjoining roads have been added to both sheets C-2, and C-3.
11) The speed limit of 55 mph has been added to the State Route 522 designation. The speed
limit on the Cross Junction Road (St. Rt. 693) is not posted.

12) Location of easements have been designated where required on sheet C-2.
13) The slopes of the proposed disabled parking spaces have been changed to a maximum of
2%. Grading has been adjusted to allow for this change.
14) The only landscape plantings in the project will be the reseeding of the area with grass.
In addition to your comments I have also incorporated other agencies comments that are reflected
in these drawings. Once these offices have given final approval to the site plan I will provide
your office with a copy of their approved agency comment sheets.

Dear Jeff:
The design documents prepared by Lewis and Associates adequately
addressed our concerns related to stormwater management and erosion
and sediment control. Consequently, we granted a land disturbance
permit based on the implementation of these documents.
However, after reviewing the plans prepared by Wayne
Westenmeyer, we offer the following comments:
1. The culvert designs and locations should be coordinated with
the design recommended by Lewis & Associates. i.e. Lewis
recommended an 18" RCP.
2. Refer to sheet C-3: The grading approved for the Lewis
design should be reflected on the Westenmeyer design.
3. Refer to Sheet C-5: The proposed pavement details appear to
be somewhat conservative for the anticipated church traffic.
We suggest that John Lewis evaluate these designs to
determine more realistic and cost effective alternatives.
This evaluation should also, be extended to a review of the
sidewalk details.

I've reviewed the changes to the Fire Lane configuration for Redland United Methodist
Church which you faxed to my office on 7/7/95. This design allows much better access
for Fire Dept. vehicles should the need ever arise to deploy a ladder truck to this
building.
Thank you for your efforts to make this site as accessible as possible for emergency
vehicles. We all hope our services are never required in an emergency situation, but
when they are, it is to everyones benefit to have a site that allows immedeate access to
all portions of the structure, including the roof.
I will be looking for the revised site plan in the near future so that final approval from the
Zoning Administrator can be obtained and your project can move forward. If there is
anything further my office can assist you with, please do not hesitate to contact us.

I have reviewed the site plan submitted for the new church
building to be located on Rt. 522 N. (North Frederick Pike). The
plan, as drawn, cannot be approved by the Fire and Rescue Dept. as
there is inadequate access to the north and south sides of the
building for a ladder truck.
When we met on April 26, 1995 to discuss your variance request
for height, there was agreement that automatic smoke ventilation
devices would be placed in the gable ends of the structure, and access
for a ladder truck on three sides of the structure. As shown, the
fire lanes are 100' from the building which exceeds the capabilities
of our truck.
In order to utilize our aerial device effectively, the centerline
of the fire lane must be a minimum of 20' and a maximum of 35, from
the face of the building.
If your parking configuration is to remain as shown, additional
fire lanes will be necessary for fire dept. use, constructed to
specifications for fire lanes, found in Frederick County Ordinance
Chapter 10. Basically this would require fire lanes 20, in width,
capable of supporting 60,000 pounds and posted with signage
designating the fire lane as a "No Parking" area.
