The site plan has been revised to reconfigure the parking lot and raised islands which involves the relocation of four parking spaces to the front of the building and the provision of five employee parking spaces behind the building adjacent to the dumpster, and a reconfiguration of the required buffer area which involves the elimination of the board -on -board fence, the relocation of the tree plantings to wrap around the existing earth berm which was created for the stormwater management facility, and additional grading work adjacent to the Parsons property to assist with current drainage problems. It should be noted that the tree plantings along the earth berm have been revised to provide for a double row of evergreens as agreed by our department, Mr. Parsons, and yourself. The approved site plan calls for the construction of a new office/warehouse facility that will be 11,400 square feet, the creation of a new asphalt parking lot and maneuvering area that will contain curb and gutter, 25 regular parking spaces, two disabled parking spaces that are van accessible with appropriate ramps and signage, the striping of two loading spaces, raised islands with landscaping, and landscaping along the perimeter of the front parking lot, the provision of a concrete sidewalks to provide access from the parking lot to the building, a concrete dumpster pad that will be 120 square feet and completely enclosed with a six foot high board on board fence with a latched gate, the installation of two fire hydrants, the creation of a storm water detention basin that will be graded to provide for a six foot high berm, the planting of 66 evergreen trees and 33 deciduous trees to provide for a landscaped screen, and all appropriate erosion and sedimentation control measures. I have provided you with two copies of the approved site plan. This will allow you to provide the contractor with an on -site copy and keep a copy for your records. The following comments need to be addressed prior to final approval of this site plan: 1) Please provide your signature on the Professional Land Surveyor's Seal. 2) Please provide a signature and date block that reads Approved by the Zoning Administrator. 3) Please indicate the name and address of the developer. 4) Please revise the location map to indicate the name of the roads as indicated on the marked -up plan. 5) Please provide the use and zoning for all adjoining property owners, including those on the west side of Welltown Road. 6) Please verify the use of the fenced in area in the front yard setback. It appears that this is a pump station. 7) Please provide the Floor to Area Ratio for this site. 8) Please provide the height of the new structure and the existing structures. 9) Please provide the location, dimension and height of all existing or proposed signs on these properties. 10) Please provide the location, height, and specifications for any proposed outdoor lighting fixtures. 11) Please provide the location and specifications for the enclosure of any proposed outdoor storage area. 12) Please provide the location and specifications for enclosure of any proposed outdoor trash receptacle. 13) Please provide the location and width of all existing or new easements for the proposed utilities and drainage facilities. 14) Please provide the appropriate parking lot calculations for this use. These calculations should also reflect the required amount of disabled parking spaces. 15) The parking lot design is inadequate. Parking lots are required to have curb and gutter around the perimeter, which includes the parking lot aisles. Perimeter and interior parking lot landscaping is required as well. These spaces need to be provided in the front parking lot area, preferably in the middle of the proposed 13 parking spaces. This will allow for access to all proposed office areas. Appropriate disabled ramps and signage need to be provided as well. 16) The location of the two loading docks and loading areas is cause for concern. Based on this design, a truck will have to back into the front parking lot to leave the site. The building plans should be designed to have the loading dock doors and the loading spaces in the rear of the building. These loading spaces should be relocated to the area where the proposed disabled spaces are shown. All loading spaces need to be striped to provide for a 12' x 45' area. 17) The proposed fire hydrant needs to be protected by bollards if it is to remain in adjacent to the travel aisle. 18) Please provide the location and dimension of all required setback areas. 19) The proposed full buffer and screening needs to be modified. You are indicating a six foot high berm with a hedge. The requirements of a full screen include a six foot high berm, fence, wall, or hedge and three trees for each ten linear feet. Two-thirds of these trees need to be various species of evergreen and one-third deciduous. Therefore, the hedge on top of the berm is inadequate. Please revise this plan to show the location, number, and species of trees that will be used to accomplish this. The landscaping needs to be extended around the berm to the front of this site, as there are residences on the opposite side of Welltown Road. Provisions need to be made to screen the gap created by the spillway. 20) Please include the sewer line and pump station plans that are referenced on the first sheet. Please provide our department with the site plan review fee, the site plan application page, and all review agency comments. Once our department receives this information, a file will be created and this plan will be placed on the Planning Commission's Bimonthly Report. Our department will proceed with administrative approval of this site plan once all review agencies and the Planning Commission are satisfied. It appears all of our earlier review comments have been satisfactorily addressed. The plan is approved as submitted. However, due to the flatness of grade at the outfall of our existing 49"x33" C.M.P.A. culvert under Route 661, additional measures i.e. grading and/or ditch lining may be necessary to obtain positive drainage. A determination will be made by the VDOT Inspector during construction. A land use permit will need to be applied for to cover the construction of the commercial entrance and other right-of-way improvements prior to initiation of work. The permit is issued by this office and will require an inspection fee and surety bond coverage. A permit of this nature normally takes approximately thirty (30) days to issue once satisfactory application has been made. The dedication of proposed drainage easements and vacation of existing easement will need to be taken care of prior to completion of the permit. 1 would like to call to your attention the design changes made to the entrance configuration on these plans has changed the original utility layout. The original utility plans for the project we approved and subsequent revised plans recently submitted with the developer's land use permit application do not match these entrance plans. The utility plans should be revised to prevent any costly delays and/or changes during construction. We will need four sets of the revised utility plans to issue the necessary land use permit. Also, the ditch profile should be extended southward to show when and how the proposed ditch bottom ties in to the existing ground line. 3.) The ditch slope from our 49"x33" pipe to the northeast is very flat. To make sure ponding will not be a problem you will need to show some spot elevations downstream of the 695 contour from your proposed 21" basin outfall pipe to the north. Our comments may be found on the enclosed plans marked in red and as follows: A half typical section of the entrance detailing a pavement design of 8" Type 21-B Aggregate, 3" Type BM-2 Asphalt Concrete, and 2" Type SM-2A Asphalt Concrete will need to be added to the plan. 2. The proposed 10' entrance offset will need to be increased to 12'. This may affect the entrance culvert location. 3. The entrance culvert should be extended to the ends of the entrance curb radii. Also, with the elevations given it does not appear adequate cover can be obtained over the pipes. Perhaps elliptical pipe may want to be considered. Regardless if minimum cover cannot be obtained, Class IV Reinforced Concrete Pipe will be require. 4. We realize from previous conversations with your office due to the flatness of grade along Route 661 you are not proposing to tie the future entrance curb and gutter together along the roadway. However, completion of the planned future phases according to this plan will relocate the roadway shoulder and ditch off the Route 661 right-of-way. While the open ditch will collect runoff from Route 661's pavement and shoulder and discharge to the on site detention basin, the plan makes no provisions for roadway ditch and/or shoulder maintenance. - Connecting the entrance curb and gutter along Route 661 together with appropriately located drop inlets with necessary maintenance easements or right-of-way will allow VDOT control of the roadway surface water and maintenance of the system. We have no problem with the open ditch concept for this initial phase. However, serious consideration will need to be given to a storm drainage system on future phases. The proposed elevations given seem adequate to provide drop inlets with appropriately sized pipe outfalls to a storm sewer system. Due to the flatness of grade a profile of the proposed culverts and ditch will be required. 6. The existing 20'x50' drainage easement at the outfall of our existing 49"x33" pipe under Route 661 will need to be vacated or quitclaimed since the drainage pattern is being changed. Additional easement at the location shown will need to be provided. 7. A statement absolving the Department of any maintenance responsibility or liability for the proposed on site detention basin will need to be added to the plan. 8. During future ,'uses of the project the existing commercial entrances on the south side of the property will need to be upgraded to current VDOT standards. 9. The limited access right-of-way will need to be clearly defined and the following note added to the plan: "Absolutely no work associated with this project is to encroach upon the I-81 limited access right-of-way without first obtaining written permission from the Virginia Department of Transportation." 10. The proposed entrance/drainage design and our aforementioned comments may conflict with the previously approved utility plan for this project. 11. Other miscellaneous items as marked will need to be addressed. Please revise and resubmit three copies for further review. Kiracofe Fire Marshal On March 4, 1997, I had the opportunity to visit the K& J building site located at 360 Welltown Road. During my inspection, I noticed the fire hydrant located at the front right side of the building is facing the parking lot. This will not allow for fire company access. The approved site plan shows the fire hydrant closer to the front curb and therefore would allow fire company access without any impact on parking. I have outlined two possible options to correct the problem. Option one, move the fire hydrant closer to the front curb or sacrifice the two parking spots directly in front of the fire hydrant. This option would require the appropriate stripes and signage indicating Fire Lane No Parking. The approved plan showed a required hydrant at the rear portion of the building. This fire hydrant was not installed as outlined on the approved site plan. Apparently there was an existing fire hydrant at the corner of the property that was not shown on the approved plan. In order for this fire hydrant to be used it will need to be moved to the edge of the parking lot. Impact protection will have to be installed around the fire hydrant. Concrete bollards will provide adequate protection. I have also listed minor building corrections on my inspection form with L&L builders. It is my understanding they will be responsible for making the required changes. Also, an"as built" site plan revision will be required.
