SITE PLAN CHECKLIST
The checklist below indicates all the information that needs to be submitted as part of the site plan
application. All required information must be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).
Site Plan Package
1. One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

As requested, we have reviewed your site plan dated October 30, 1996 for the
referenced project. Our comments may be found on the attached set of plans marked in
red and as follows:
1. The entrance radii is to be shown as 25'.
2. The entrance curb & gutter is to be CG-6. A CG-6 detail needs to be added to the
plan sheet.
3. Standard VDOT Notes are to be added to the plan sheet.
4. The R.C.P. entrance culvert is to have mitered ends.
5. The dimensions are to be added to the entrance detail.
Please revise and resubmit six (6) copies for final approval. Should any changes be
deemed necessary, please design them to meet or exceed the above recommendations.

On May 2, 1997 I performed a final inspection of the commercial entrance at the
referenced project. Based on this inspection I found the entrance paved and completed.
Our records do not indicate that inspections were scheduled. Since the work is now
complete, we will require an independent testing laboratory take core samples. These
samples are necessary to verify 21-B aggregate depth and the thickness of base and
surface asphalts.
This entrance permit will not be finalized until we are satisfied the core sample results
indicate the entrance was installed in accordance with the approved plans.

maintenance of one commercial entrance at the referenced location.
Please ensure a copy of the approved permit assembly is kept at the job site whenever working
within the Route 1328 right-of-way. I would also like to remind you of the need to erect and
maintain proper traffic control in accordance with our Virginia Work Area Protection Manual.
Mr. Steve Melnikoff of this office has been discussing recently with Mr. Mike Carroll unfinished
permit work by your company. In particular, the F.C.S.A. water and sewer project on Route 522
South (Land Use Permit #855-03061). This permit was issued to you on 07/28/95. We feel you
have had ample time to complete this work. Therefore, the enclosed permit will be the last one
issued to your company until the aforementioned Land Use Permit #855-03061 has been
satisfactorily completed.
Please give Mr. Steve Melnikoff a call at (540) 722-3460 at least 48 hours in advance of the start
of construction to arrange for any necessary inspections.

addressed. Please advise the owner of our approval.
A land use permit may now be applied for to cover work within the Route 1328 right-
of-way. The permit is issued by this office and will require a minimal processing fee
and surety bond coverage. Once satisfactory application has been made, a permit of
this nature will normally take approximately thirty days to process and issue.

Dear Mr. McIlvaine:
This letter is to inform you that the Frederick County Planning Department administratively
approved site plan #053-96 on January 30, 1997. ]'his approval allows for a building permit to be
issued for the entire project.
As discussed with Mr. Mark Smith of Greenway Engineering, and Mr. Bob Childres of the Virginia
Department of Transportation, this approved site plan may not address all VDOT concerns. in order
for the contractor to begin construction, Mr. Smith, Mr. Childres, and Frederick County have agreed
that the issuance of the Certificate of Occupancy will be contingent upon satisfying; all VDOT
concerns. Any necessary changes to the site, as a result of VDO]' requirements, will be the sole
responsibility of the contractor. Additionally, it will be necessary to revise the site plan to reflect
any changes to the site.
This approved site plan consists of a 140,880 square foot building addition and a total of 20 parking
space on site (one of which is van accessible I-IDCP). All driving areas will be paved (asphalt and
concrete as noted). Four trees will be provided adjacent to the parking lot: 2 red maple; 2 Aristocrat
pear. Upon completion of this addition, this site will contain a 201,217.5 square foot warehouse.

I have reviewed the above referenced site plan, submitted to my office November 22, 1996. The
following comments will need to be addressed in order for this office to proceed with the site plan
approval process. Please provide:
1) Location, height, and specification of outdoor lighting fixtures. Are any building
mounted lamps proposed?
2) The use of the proposed building addition.
3) Dimensions of the parking spaces and the handicap accessible space.
4) Landscape calculations that proactive the "rounding up" method. If 2.25 trees are
required for the parking perimeter, then 3 trees should be planted. Also, if 0.90 trees
are required for the parking interior, then 1 tree should be planted.
I have enclosed a red -lined copy of the submitted site plan, on which are additional comments that
should be addressed.

I conducted a second review of the above referenced site plan, submitted to my office December 10,
1996. 1 have one additional comment that should be addressed.
• Please provide a detail of the surface treatment proposed for the parking lot. In my review
of the submitted site plan, I do not believe the treatment type has been indicated.
Please revise your site plan to address this comment and the comments provided to you by the
required review agencies.
You will need to provide this office with the compl,;ted approved comment sheets from the
following review agencies: Virginia Department of Transportation, Sanitation Authority, Inspections
Department, County Engineer, Fire Marshal, and this department. Once all comments have been
addressed, resubmit two copies of the plan to this office for a final review. If the plan adequately
addresses all comments, I will request five final copies and recommend this plan for approval.
