GENERAL CONSTRUCTION NOTES
1. The contractor shall furnish all materials, labor and equipment and perform all work including restoration for
the completed installation of the utility systems and all other improvements shown or implied as necessary for the
completed facility ready for use. Unless otherwise noted, specifications for all work to be in accordance with
applicable Standards and Construction Specifications of the The Virginia Department of Transportation. and the
contract documents.
2. The engineer has attempted to locate existing subsurface utilities; however, such may exist that are not shown.
The contractor shall exercise care in this work so as to avoid damage to any utilities. Any damage shall be the
contractor's responsibility. The contractor shall notify all utility companies 72 hours prior to construction. Call
Miss Utility" 1-800-257-7777,
3. The contractor shall be responsible for all construction stakeout. All dimensions. elevations and locations shall
be verified in the field by each contractor prior to beginning the work. The Engineer will approve all cut sheets.
4. Any unusual subsurface conditions encountered during the course of the work shall be immediately brought to the
attention of the Engineer and/or the Owner.
5. All grading for improvements to be confined to the project area. Any work performed offsite is the Contractor's
responsibility.
6. Contractor to obtain, at his expense. all permits required of this work and shall faithfully adhere to the permit
conditions imposed.
7. Temporary and permanent erosion control structures and methods to be in accordance with the Virginia Erosion
Control Guidelines and local ordinances and to be utilized during the construction period.
8. All erosion control structures shown or as required during construction shall be continuously maintained
throughout the period in which areas are denuded. Areas shown to be surfaced are to have stone base installed at the
earliest possible date after establishing grade. No area is to remain denuded for more than 30 calendar days when
construction is not in progress.
9. All unpaved areas of the site which are disturbed by this construction shall be provided with 4" topsoil,
fertilized, seeded, compacted and mulched. Seed bed to be watered and cared for as necessary to provide a good lawn
quality grass cover. Areas not graded shall be disced, leveled and seeded to achieve lawn conditions.
10. Contractor to clean up and restore disturbed areas immediately upon completion of pipe -laying in the affected
area.
11. Design features relating to construction or to regulation, control and safety of traffic may be subject to change as
deemed necessary by V.D.O.T.
12. All fill, base and subsurface material shall be compacd to 95% of theoretical maximum density as determined
by AASHO T-99 method A, within plus or minus 2% of o imum moisture for the full width of any dedicated right-
of-way. Compaction in other areas to be 90%.
13. Vegetation Plan: All grass areas to be seeded with a mixture of Marion Kentucky Bluegrass at a rate of 2 3 lbs.
per 1.000 square feet and Annual Rye at a rate of 1 lb. per 1,000 square feet. Newly seeded areas shall be mulched at
a rate of 70 120 lbs. per 1,000 square feel. Adequate watering and fertilization are required to aid the emergence
of new grass.
14. Cutsheets shall be submitted to the Engineer for approval prior to constructioh. All cutsheels should show
existing ground elevations at centerline of utility, structure or road, and offset hub elevation.

Conditional Use Permit #006-90 was approved for a recreational area
including golf, driving range, batting cages, mini golf, arcade and snack
bar.
This conditional use permit is approved with the following conditions
1. This permit is to be reviewed and renewed annually by the
staff, the Planning Commission and the Board of Supervisors.
Additional conditions may be attached if additional impacts
2. If the use, occupancy or ownership of the property changes,
this permit shall expire.
3. A formal site plan must be submitted to address parking,
lighting, buffering and all other normal site plan requirements.
4. All RA (Rural Areas) Zoning District setback requirements
must be maintained.
5. The applicant shall comply with all items listed in the

Conditional Use Permit #006-90 was approved for a recreational area
including golf, driving range, batting cages, mini golf, arcade and snack
bar.
This conditional use permit is approved with the following conditions
1. This permit is to be reviewed and renewed annually by the
staff, the Planning Commission and the Board of Supervisors.
Additional conditions may be attached if additional impacts
occur.
2. If the use, occupancy or ownership of the property changes,
this permit shall expire.
3. A formal site plan must be submitted to address parking,
lighting, buffering and all other normal site plan requirements.
4. All RA (Rural Areas) Zoning District setback requirements
must be maintained.
5. The. applicant shall comply with all items listed in the
"specifications foresight" listing attached to the application.
6. Lighting must be directed away from U.S. Route 11 and the
adjoining residential areas.
7. If parking lot is approved to be covered with 21A stone, it
must be upgraded to ordinance requirements, if this property is
rezoned in the future.
8. No ABC license to be applied for at this establishment.
91. Hours of operation will not exceed 11:00 p.m.
10. All Health Department requirements must be met.
11. Landscaping will be provided in the setback zone fronting
on Route 11.
