application_ The Department of Planning & Development will review the application to ensure that it is
complete prior to accepting it. If any portion of the application is not complete, it will be returned to the
applicant(s).
(1) One (1) set of approved comment sheets are required from each relevant review
agency prior to final approval of a site plan. It is recommended that applicants

The Frederick County Department of Planning and Development administratively
approved the above -referenced site plan on October 18, 2006. The site plan is approved
for the construction of addition to the current facility, in the Stonewall Magisterial
District. This construction includes the following:
Approximately 34,000 square feet of nursing home (addition of 68 beds).
The site plan meets all requirements for development in the RP (Residential
Performance) Zoning District. The expansion of this use is covered within the approved
Conditional Use Permit. All relevant review agencies have approved the project at this
time. I am providing you with five (5) copies of the approved site plan. Please forward
these copies to the appropriate representative(s). Furthermore, advise the owner(s) that a
copy should be kept for future reference, and an approved copy must be kept on the
construction site throughout the development process. Once site development is
complete, the owner(s) should contact this office to schedule an on -site inspection. Do

We have received your revised site plan dated September 22, 20o6 for the referenced project.
It appears all of our earlier review comments have been satisfactorily addressed. Please advise
the owner of our approval. Also please provide five (5) sets of approved construction
plans "itli signed seal for VDOT distribution.
The appropriate land use permit may now be applied for to cover work within the Route 7
right-of-way. The permit is issued by this office and will require a minimum processing fee,
surety bond coverage, and the salary & expenses of a State assigned Inspector. A copy of
any/all recorded plats of dedication for drainage easements or additional right-of-way required
for implementation of this proposed project should be provided to VDOT prior to issuance of
any land use permits. Dedication should be made to the Commonwealth of Virginia.
Once satisfactory application has been made, a permit will normally take approximately thirty
days to process and issue. Should you have any questions, do not hesitate to call.

Dear Mr. Painter:
Planning Staff has reviewed the above -referenced site plan to determine if administrative
approval can be granted. At this time, administrative approval cannot be granted. This
site plan is denied until the issues in this letter, as well as all issues of the other review
agencies, have been adequately addressed. Please review Staff s comments listed below
and then prepare a revised site plan which adequately addresses each concern.
Review Comments:
1. Required Signature. There needs to be a signature space for the Frederick
County Zoning Administrator.
2. Setbacks. The setbacks on the cover sheet need to be revised to reflect what is
provided on the site plan.
3. Conditional Use Permit (CUP). A revised CUP is required prior to approval of
this site plan.
4. Adjoining Properties. Provide all adjoining lot information, including those
across right-of-ways.
5. Handicap Ramp. A handicap ramp must be placed near the two handicap spaces
on the southern side of the property.
6. Easements. Any easements located on the property need legal references.
7. Eastern Entrance. The driving lane to the south of this entrance has been
reduced to 12' from a 22' lane, which would only allow for one-way traffic. A stop
sign also needs to be displayed for exiting the site.
8. Sidewalks. Sidewalks with curb and gutter are required along Berryville Pike
(Route 7).
9. Southern Entrance. Crosswalks need to be placed across the entrance.
10. Building Restriction Lines (BRL). Within the Landscaping Plan, the BRL's for
the site are incorrect.

11. Landscaping. Please include the proper requirements for landscaping on the site
plan (Section 165-36).
After you have revised the site plan, please resubmit one copy so that I may verify the
information contained on the plan. I will need all approved review agency comment
sheets and at least five copies of the final plan for final administrative approval.
Comment sheets are required from the following agencies: the Frederick County
Inspections Deparhnent, the Frederick County Department of Public YVorks (County
Engineer), Frederick County Sanitation Authority, Virginia Department of
Transportation, and the Frederick County Fire Marshal.
