application. All required information must be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is
complete. If any portion is not included or complete, the site plan application will not be accepted
and returned to the applicant(s).

1. One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

Dear Bruce:
This letter is to inform you that the Frederick County Planning Department administratively approved
site plan #037-96 on April 9, 1997. The approved site plan consists of a 3,250-square-foot
veterinary office. There will be 14 parking spaces (one of which shall be van accessible) provided
with a bituminous concrete surface treatment, and curb and gutter. A full landscape screen will be
provided along the northeastern property line to buffer the business from the adjacent residential
properties. Additional landscaping will be provided in the form of a three-foot high evergreen hedge
along the parking area, and six evergreen trees planted throughout the site.
Attached are two copies of the approved plan. This will enable you to provide your contractor with
an approved copy, and maintain a copy for your records. Please have the contractor contact this
office to schedule an on -site inspection once all improvements are complete.

RE: Preliminary Site Plan Review -- Frederick Veterinary Hospital
Site Plan 9037-96
Dear Steve:
I have reviewed the above referenced site plan, submitted to my office August 21, 1996. The
following comments will need to be addressed in order for this office to proceed with the site plan
approval process. Please provide:
1) A listing of the conditions placed on the site as a result of a conditional use permit.
You did provide conditions on the submitted site plan, but the conditions on the site
plan are not the same as stated on the approved conditional use permit;
2) Location of sidewalks and pedestrian ways. Also provide the type of surface and
dimensions;
3) The location, height, and dimensions of all signs. The location of the proposed
monument sign must satisfy the minimum setback of 10 feet from the road right-of-
way;
4) Location, height, and specifications of outdoor lighting fixtures. Such fixtures should
be designed so as to minimize light being shed onto adjoining properties;
5) Screening required to enclose the trash receptacle. Dimensions, type of wood fencing
materials, and latching system should be provided;
6) Posted speed limits of existing adjacent roads;

7) Location of all entrances on adjoining roads within 200 feet of the proposed entrance;
8) Location of all utilities, including sewer and water lines with the size of lines, mains,
and laterals;
9) Location and width of all easements, including access, utility, and drainage
easements;
10) Location and nature of fire lanes, fire hydrants, and all other facilities necessary to
meet Fire Code requirements;
11) Calculations describing the required number of parking spaces. My calculations
indicate that 13 spaces are necessary;
12) Location and dimensions of all parking spaces,. driveways, parking aisles, curbing
and .other features to be used. The entire parking facility (asphalt surface) should
contain curb and gutter. Additionally, 9-foot wide landscaped islands should be
located at the end of all parking bays;
13) Location and dimension of all disabled parking spaces and ramps. Indicate the
location and dimensions for the curb ramp and accessible signage;
14) Landscaping plan describing location and types of plants to be used. Indicate the
plant species and minimum size at time of planting: Deciduous trees should have
minimum calipers of 2.5 inches; evergreen trees should be a minimum of 4-foot high;
parking area hedges should be 3-foot in height;
15) Location of required buffers and screening with cross sections or profiles. The
proposed opaque fence should be board -on -board, and positioned so that the
supporting posts are located on the veterinary hospital side of the fence. Also, the
landscape screening planting should be divided evenly between by the opaque fence.
Please revise your site plan to address these comments, and the comments provided to you by the
required review agencies. Enclosed is an red -lined copy of the submitted site plan.

You will also need to provide this office with the completed approved comment sheets from the

way. The permit is issued by this office and will require a minimal processing fee and
surety bond coverage. Once satisfactory application has been made a permit of this
nature normally takes approximately thirty days to process and issue.

As requested, we have reviewed your site plan dated August 14, 1996 for the
referenced project. Our comments may be found on the attached set of plans marked in
red and as follows:
1. Show entrance of 30' from face of gutter plan to face of gutter pan.
2. Entrance tapers should be 36' not 50', as shown.
3. Label entrance CG-11 not CG-91).
4. Entrance should be 12' from edge of pavement to face of gutter pan.
5. Stub sewer line out of Manhole #2 beyond the entrance pavement.
6. Replace existing 12" culverts south west of the entrance with 15" C.M.P.A.
7. Add standard EC-1 north notation off the ends of the CG-6 entrance gutter.
8. Need typical section of rebuilt shoulder and ditch line north east of the entrance.
Please revise and resubmit six copies for final approval. Should any changes be
deemed necessary, please design them to meet or exceed the above recommendations.

SITE PLAN CHECKLIST
The checklist below indicates all the information that needs to be submitted as part of the site plan
application. All required information must be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).
Site Plan Package
1. One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

Frederick County Sanitation Authority
Department of Planning and Development
Inspections Department
Frederick County Engineer (Pudic Works)
Frederick County Fire Marshal

2. One copy of the Site Plan application
3. Five Copies of the Final Site Plan for approval
4. One reproducible copy of the Site Plan (if required)
5. A 35mm slide of the Site Plan (if required)
