Dear Mr. Artz:
The Frederick County Department of Planning and Development administratively approved the above -
referenced site plan on September 30, 1999. This site plan, for an addition to an existing church, meets
all County requirements for development within the RA (Rural Areas) Zoning District. Specifically, a
384-square-foot classroom will be added to the rear of the structure. In addition, the existing pit privy
will be replaced with a water tight vault privy under direction of the Frederick County Health
Department.
I have provided you with several copies of the approved site plan. Please forward these approved plans
to the owner and advise the owner to keep an approved copy for their files. Furthermore, the owner
should ensure that an approved copy is maintained on the construction site throughout the development
process. Once site development has been completed, the owner should contact this office to schedule
an on -site inspection.

Dear Mr. Artz:
I have had the opportunity to review the above referenced site plan submitted to our department on
September 8, 1999. The following comments should be addressed and included in a revision of this
site plan. The submitted plan is also marked with these comments.
Please state the name, address, and phone number of the property owner on the site plan.
2 Please provide a note under the approval box reading "Site plan shall be valid for five years
from date of approval."
3. Please provide the use, zoning, and property identification number (PIN) for all adjoining
properties. This includes the properties located across Glengary and Green Spring Roads.
Please indicate the use for each property (i.e. residential, vacant, agriculture). The zoning for
all properties is RA (Rural Areas).
4. Please provide the location and dimensions of all required setbacks applicable to the subject
parcel.
5. Please provide the height of all buildings and structures.
6. Please provide the ground floor area and total floor area of all buildings with FAR
calculations.
7. Please provide the posted speed limit on Glengary and Green Spring Roads.
8. Please provide the dimensions of the existing gravel driveway.

9. Please provide the locations of all utilities and specify all easements associated with these
utilities.
10. Please specify the location and nature of all fire lanes, fire hydrants, and other facilities
necessary to meet Fire Code requirements.
11. Please show the location and general nature of the existing parking situation.
12. If required by the County Engineer, please show existing and finished contour lines. In
addition, show any natural features relevant to the site. Please provide information regarding
steep slopes, woodlands, flood plains, wetlands, sinkholes, or other environmental features.
If no such elements are present, please provide a note to this effect on the site plan.
13. If required by the County Engineer, please provide a stormwater management plan for the
site.
14. If required by the County Engineer, please provide an erosion and sediment control plan for
use during the development of the site.
After you have revised the site plan, please resubmit one copy so that I may verify the information
contained on the plan. I will then need all approved review agency comment sheets and five copies
of the final site plan for approval. Comment, sheets are required from the following agencies: the
Virginia Department of Transportation, the Frederick County Health Department, the Frederick
County Sanitation Authority, the Frederick County Inspections Department, the Frederick County
Engineer, and the Frederick County Fire Marshal.

A review of the submitted site plan has been completed. Based on tlis review and an on -site
inspection, we offer the following comments:
• The existing gravel entrance will need to be re -graded in order to obtain proper drainage. On -
site drainage is causing rutting through the entrance drive. This can be solved by improved
ditcling on both sides of the drive.
• On -site trimning of roadside vegetation is recommended to improve sight distance.
• Future sight plan submissions should show entrance (existing) details including drainage flow,
pipes, grades, pavement, etc.
• This entrance is currently adequate for present membership. However, VDOT will expect the
entrance to be upgraded to meet our minimum requirements should any future improvements
be designed and implemented.

I have the reviewed the submitted site plan and the Health Department has no objections
to the proposed construction if the following conditions are met:
40 The owner must submit the attached repair application (no fee) to the health
department. A repair permit would then have to be issued to replace the existing pit
privy with a water tight vault privy.
If at such time the church develops a water supply and the water is connected to the
church building, the health department would at that time require that an approved
means of sewage disposal be installed at the property.
0 If the OHW is a continuous line owned by a Utility Company, the tank would have to
be located out of their right of way or the Utility Company would have to approve the
plotted tank location.
