The CONTRACTOR shall be responsible for determining the exact location of all utilities before commencing work and for any damages which occur by the failure to locate and preserve these utilities. If the CONTRACTOR should encounter utilities other than those shown on these plans, the CONTRACTOR shall immediately notify the engineer and take the necessary and proper steps to protect the facility and assure the continuance of service. 2 The CONTRACTOR shall have approved construction drawings prior to the start of construction. At least one copy of approved plans and revisions shall be kept on -site at oil times. 3 Cut and fill slopes shall not exceed 2-horizontol to 1-verticol unless otherwise called for on the plans 4 Controlled Fills A. Controlled compaction shall occur in all fill sections for pavement, trenches for utilities and in other areas as designated on the plans. B. Controlled fills shall be compacted to 95% density as determined by methods per standard proctor ASTM D 698. Density shall be verified by a qualified soils engineer C. Controlled fills shall be compacted in 8 inch Ilfts (loose thickness) to the specified density, beginning from the existing ground surface, unless otherwise approved in writing by o qualified soils engineer D. The surfaces directly beneath areas to receive controlled fill are to be cleared, grubbed, and stripped of topsoil The area is then to be scarified and compacted to a depth of 6 inches to the same density as the controlled fill to be placed thereon. E Fill material shall be taken from approved borrow areas. It shot( be clean soil, free of roots, woody vegetation, oversized stones, rocks, sandy soil, or other objectionable material. 5. Blasting is not permitted within 25 feet of existing utilities without owner permission 6 Storm sewer and pavement materials called for on the plans refer to standard VDOT designations 7. BENCHMARK three nails in wood power pole adjacent to the existing parking lot - elevation = 101,26. 8. All radii indicate edge of grovel, edge of pavement or face of curb as applicable 9. Fire lanes shall be delineated as directed by the Frederick County Fire Marshal. 10. The CONTRACTOR shall obtain permission from adjoining property owners prior to conducting any off site construction activities. 11. The CONTRACTOR shall coordinate the relocation of the existing utilities with the appropriate utility company as necessary. 12. All site and building lighting shall be installed to avoid spill -over onto adjoining properties. 13 Remove trees, shrubs, grass and other vegetation, improvements, or obstructions as required to permit installation of new construction. Preserve existing vegetation as called for on the plans. Remove similar items elsewhere on site or premises as specifically indicated. 'Removal' includes digging out and off -site disposing of stumps and roots. Cut minor roots and branches of trees indicated to remain in a clean and careful manner, where such roots and branches obstruct installation of new construction. 14. Topsoil is defined as friable cloy loam surface soil found in a depth of not less than 4 inches. Satisfactory topsoil is reasonably free of subsoil, cloy lumps, stones, and other objects over 2 inches in diameter, and without weeds, roots, and other objectionable material. Strip topsoil to whatever depths encountered in o manner to prevent intermingling with underlying subsoil or other objectionable material. Stockpile topsoil in storage piles in areas indicated or directed. Construct storage piles to provide free drainage of surface water. Cover storage piles, if required, to prevent wind erosion 15. Clear site of trees, shrubs and other vegetation, except for those indicated to be preserved. Completely remove stumps, roots and other debris protruding through ground surface. Fill depressions caused by clearing and grubbing operations with satisfactory soil material, unless further excavation or earthwork is indicated. 16. Burning of excess materials will not be permitted on the property. 17 Remove waste materials and unsuitable soil from Owner's property. 18. The CONTRACTOR shall obtain a Land Disturbance Permit from the Frederick County Public Works Department prior to initiating any site work. Environmental Features Wetlands - None Flood plains - None Sinkholes - No sinkholes have been identified on this site Steep Slopes - None Woodlands -No woodlands have been identified on this site. Woodlands are generally defined by the Frederick County Code as including largely mature trees in groves greater than 1/4 acre. The wooded area of the site contains immature second growth deciduous vegetation with some scattered larger trees. 11. Storm water management is provided by a detention facility constructed on the property of Cutshaw Enterprises, Inc. See the report by Pointer -Lewis, PLC titled 'STORM WATER MANAGEMENT REPORT'; Allied Wood Products, dated July 6, 1998. EROSION AND SILTATION CONTROL NARRATIVE Project Description The developer proposes to build a church addition on a 2,12 acre parcel. The total area of disturbance on the parcel is approximately 25,000 square feet. Storm water runoff will discharge overland to the northeast and to on existing storm water detention pond. Site grading will include the importation of fill material to bring the site up to the proposed grades. Existing Site Conditions The site is currently occupied by a church. Slopes range from 1.0 % to approximately 5.0%, Adjacent Areas The site is bordered on the east by the Route 11 right-of-way. To the south is a residential lot and Allied Wood Products. The area to the north and west is the commercial Off -site Areas With the exception of borrow operations, no off site disturbance or construction activities are anticipated Critical Areas None are identified. Storm Water Discharge -Adequate Channel The runoff from the site will generally be sheet flow. Erosion and Sediment Control Measures 3.05 Silt fence will be installed in one location downstream from the construction area as a First measure of construction. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shall be made immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should be removed) after each storm event They must be removed when deposits reach approximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.31 Temporary seeding in accordance with the E & S Std & Spec 3.31 will be applied to the topsoil stock pile and all areas which will not be brought to final grade within 30 days. Embankment or excavated slopes denuded for a period of greater than 30 days shall be temporarily seeded and mulched. To coincide with the anticipated construction dates, the seeding will consist of 50 pounds per acre of German Millet. All temporary seeding areas will be mulched in accordance with the schedule included herein. 3.32 All disturbed areas will be stabilized by permanent seeding in accordance with the schedule included herein. Permanent stobilization shall be applied to areas that are to be left dormant for more than one year. During construction of the project, soil stockpiles shall be stabilized or protected with sediment trapping measures. The contractor is responsible for the temporary protection and permanent stabilization of all soil stockpiles on site as well as soil intentionally transported from the project site. EROSION AND SILTATION CONTROL NARRATIVE Project Description The developer proposes to build a church addition on a 2.12 acre parcel The total area of disturbance on the parcel is approximately 25,000 square feet. Storm water runoff will discharge overland to the northeast and to on existing storm water detention pond. Site grading will include the importation of fill material to bring the site up to the proposed gra des. Existing Site Conditions The site is currently occupied by a church. Slopes range from 1.0 % to approximately 5.0%. Adjacent Areas The site is bordered on the east by the Route 11 right-of-way. To the south is a residential lot and Allied Wood Products. The area to the north and west is the commercial. Off -site Areas With the exception of borrow operations, no off site disturbance or construction activities are anticipated. Critical Areas None are identified. Storm Water Discharge- Adequate Channel The runoff from the site will generally be sheet flow. Erosion and Sediment Control Measures 3.05 Silt fence will be installed in one location downstream from the construction area as a first measure of construction. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shot[ be modle immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should Ice remioved after each storm event. They must be removed when deposits reach approximately one-half the height of the barrier.
