Contractor shall furnish all materials. labor and equipment required and shall perform all work including restoration for the completed. installation of utility systems and all other improvements shown or implied as necessary. Unless otherwise noted, specifications for all work will be in accordance with the applicable Standards and Contract Specifications of the Virginia Department of Transportation. Virginia Department of Health. County of Frederick, and the contract documents. 2. Any discrepencies found between the drawings and specifications, and site conditions or any inconsistencies or ambiguities in drawings or specifications shall be immediately reported to the engineer in writing, who shall promptly address such inconsistencies or ambiguities in writing. Work done by the contractor after his discovery of such discrepancies. inconsistencies or ambiguities, without clarification from the engineer of record, shall be done at the contractor's risk. 3. Contractor to verify the exact location of all existing utilities and notify the Engineer of any conflicts with existing and proposed utilities. Damage to existing utilities shall be the Contractor's responsibility. Forty-eight (48) hours prior to any excavation call "Miss Utility" 1-800-552-7001. 4. Contractor shall be responsible for all construction stakeout. All dimensions and elevations shown on the contract documents shall be field verified by the Contractor prior to beginning work. 5. Contractor shall obtain, at his expense, all permits required prior to starting work and shall adhere to the conditions imposed. 6. Any unusual subsurface conditions encountered during the course of the work shall be brought to the attention of the Geotechnical Engineer / Owner. 7. Access by emergency vehicles must be maintained at all times. Access to fire hydrants must not be impeded at any time by parked cars, construction materials. soil stockpiles. etc. 8. Refer to architectural drawings for exact building dimensions. CONSTRUCTION NOTES EARTHWORK NOTES: 1. All materials used for fill or back -fill shall be free of wood. roots, rocks, boulders or any other non -compactible soil type material. Unsatisfactory materials also include man-made fills and refuse debris derived from any source. 2. Satisfactory material for use as fill for public streets include materials classified in ASTM D-2487 as GW. GP, GM, GC, SW, SP, SM. SC. ML and CL groups. The moisture content shall be controlled within plus or minus 2 percentage points of optimum to facilitate compaction. Generally, unsatisfactory materials include materials classified in ASTM D-2487 as PT, CH. MH. OL. OH, and any soil too wet to facilitate compaction. CH and MH soils may be used subject to results of the Geotechnical Report and upon approval from VDOT. Soils shall have a minimum dry density of 92 lb/cu. ft. per ASTM D-698 and shall have a plasticity index less than 17. 3. Materials used to construct embankments for any purpose, back -fill around drainage structures or in utility trenches or any other depression requiring fill or back -fill shall be compacted to 95% of maximum density as determined by the standard proctor test as set out in ASTM Standard D-698. The contractor shall, prior to any operations involving filling or back -filling, submit the results of the proctor test together with a certification that the soil tested is representative of the materials to be used on the project. Tests shall be conducted by a certified materials testing laboratory and the certifications made by a licensed professional engineer representing the laboratory. 4. Compaction of fill material under building slabs shall be based upon recommendations of soils engineer after completion of standard proctor test and shall meet bearing requirements of architect for buildings. The contractor shall be responsible for testing. 5. Embankment fill and trench back -fill shall be placed in lifts at a maximum uncompacted depth of 8 inches and 6 inches, respectively. Density tests shall be conducted at the following minimum frequencies: (a) embankments for roads, streets, dams, etc.: one test per lift per 10,000 square feet of lift. (b) back -fill around structures and in trenches: one test per lift per 500 linear feet of trench. 6. Changes or revisions to these construction plans shall not be made without approval by the owner, engineer and all regulatory agencies. VDOT GENERAL NOTES: 1. All work on this project shall conform to the latest editions of the Virginia Department of Transportation (VDOT) Road and Bridge Specifications. and Standards. The Virginia Erosion and Sediment Control Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards. specifications or plans, the most stringent shall govern. 2. All construction shall comply with the latest U.S. Department of Labor Occupational Safety and Health Administration and VOSH Rules and Regulations. 3. When working on VDOT right of way, all traffic control, whether permanent or temporary. shall be in accordance with the current edition of VDOT's work area protection manual. Furthermore, all traffic control floggers must be certified in accordance with VDOT's January, 1994 (or latest edition of) Road and Bridge Specifications, Section 104.04-C. 4. Design features relating to construction or to regulation, control and safety of traffic may be subject to change as deemed necessary by VDOT. 5. Prior to initiation of work. Contractor shall be responsible for acquiring all necessary VDOT land use permits for any work on VDOT right of way. 6. If required by the local VDOT Residency Office, a preconstruction conference must be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. 7. Contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT will also require 48 hours notice for any inspection. 8. The contractor will be responsible for maintaining adequate access to the project from the adjacent public roadway through construction and maintenance of a construction entrance in accordance with the Virginia Erosion and Sediment Control Handbook, Sec. 3.02. S-2: The plan approving authority must be notified one week rior to the pre -construction conference, one week prior to the ommencement of land disturbing activity, and one week prior o the final inspection. Refer to architectural drawings for exact building dimensions. CONSTRUCTION NOTES EARTHWORK NOTES: 1. All materials used for fill or back -fill shall be free of wood, roots, rocks, boulders or any other non -compactible soil type material. Unsatisfactory materials also include man-made fills and refuse debris derived from any source. 2. Satisfactory material for use as fill for public streets include materials classified in ASTM D-2487 as GW. GP, GM, GC, SW, SP, SM, SC. ML and CL groups. The moisture content shall be controlled within plus or minus 2 percentage points of optimum to facilitate compaction. Generally. unsatisfactory materials include materials classified in ASTM D-2487 as PT. CH. MH. OL. OH, and any soil too wet to facilitate compaction. CH and MH soils may be used subject to results of the Geotechnical Report and upon approval from VDOT. Soils shall have a minimum dry density of 92 lb/cu. ft. per ASTM D-698 and shall have a plasticity index less than 17. 3. Materials used to construct embankments for any purpose, back -fill around drainage structures or in utility trenches or any other depression requiring fill or back -fill shall be compacted to 95% of maximum density as determined by the standard proctor test as set out in ASTM Standard D-698. The contractor shall, prior to any operations involving filling or back -filling, submit the results of the proctor test together with a certification that the soil tested is representative of the materials to be used on the project. Tests shall be conducted by a certified materials testing laboratory and the certifications made by a licensed professional engineer representing the laboratory. 4. Compaction of fill material under building slabs shall be based upon recommendations of soils engineer after completion of standard proctor test and shall meet bearing requirements of architect for buildings. The contractor shall be responsible for testing. 5. Embankment fill and trench back -fill shall be placed in lifts at a maximum uncompacted depth of 8 inches and 6 inches. respectively. Density tests shall be conducted at the following minimum frequencies: (a) embankments for roads, streets, dams, etc.: one test per lift per 10.000 square feet of I i ft. (b) back -fill around structures and in trenches: one test per lift per 500 linear feet of trench. 6. All materials used for fill or back -fill shall be free of wood, roots, rocks, boulders or any other non -compactible soil type material. Unsatisfactory materials also include man-made fills and refuse debris derived from any source. 2. Satisfactory material for use as fill for public streets include materials classified in ASTM D-2487 as GW. GP, GM, GC, SW, SP, SM, SC. ML and CL groups. The moisture content shall be controlled within plus or minus 2 percentage points of optimum to facilitate compaction. Generally, unsatisfactory materials include materials classified in ASTM D-2487 as PT. CH. MH. OL. OH. and any soil too wet to facilitate compaction. CH and MH soils may be used subject to results of the Geotechnical Report and upon approval from VDOT. Soils shall have a minimum dry density of 92 lb/cu. ft. per ASTM D-698 and shall have a plasticity index less than 17. 3. Materials used to construct embankments for any purpose, back -fill around drainage structures or in utility trenches or any other depression requiring fill or back -fill shall be compacted to 95% of maximum density as determined by the standard proctor test as set out in ASTM Standard D-698. The contractor shall. prior to any operations involving filling or back -filling, submit the results of the proctor test together with a certification that the soil tested is representative of the materials to be used on the project. Tests shall be conducted by a certified materials testing laboratory and the certifications made by a licensed professional engineer representing the laboratory. 4. Compaction of fill material under building slabs shall be based upon recommendations of soils engineer after completion of standard proctor test and shall meet bearing requirements of architect for buildings. The contractor shall be responsible for testing. 5. Embankment fill and trench back -fill shall be placed in lifts at a maximum uncompacted depth of 8 inches and 6 inches. respectively. Density tests shall be conducted at the following minimum frequencies: (a) embankments for roads, streets, dams, etc.: one test per lift per 10.000 square feet of I i ft. (b) back -fill around structures and in trenches: one test per lift per 500 linear feet of trench. 6. S-2: The plan approving authority must be notified one week rior to the pre -construction conference, one week prior to the ommencement of land disturbing activity. and one week prior o the final inspection. S-3: All erosion and sediment control measures are to be placed rior to or as the first step in clearing. S-4: A copy of the approved erosion and sediment control plan hall be maintained on the site at all times. S-5: Prior to commencing land disturbing activities in areas ther than indicated on these plans (including. but not limited o, off -site borrow or waste areas), the contractor shall submit supplementary erosion control plan to the owner for review nd approval by the plan approving authority. S-6: The contractor is responsible for installation of any dditional erosion control measures necessary to prevent erosion edimentation as determined by the plan approving authority. S-7: Site grading is to drain to the silt fences at all times uring land disturbing activities and during site development ntil final stabilization is achieved. S-8: The contractor shall inspect all erosion control measures ally. Any necessary repairs or cleanup to maintain the ffectiveness of the erosion control device shall be made by the lose of the day. The contractor will be responsible for applying for and obtaining any iecessary disturbance permits which may be required by Warren County >rior to the start of clearing and grading activities. ' The contractor shall stabilize all disturbed areas within 7 days after -he area is brought to final grade. i. Stabilization shall be by permanent seeding and mulching as follows: a. Small grain straw mulch shall be spread at the rate of 1.5 tons per acre. e. 10-10-10 fertilizer shall be spread at the rate of 1.000 lbs. per acre.
