The site plan is approved for improvements to 1160 Millwood Pike which is located in the Shawnee Magisterial District. Required site improvements consist of the following: • Construction/expansion of a parking lot with a total of 72 spaces (including four handicap accessible), including sidewalks, landscaped islands, striping, and handicap access. • One loading space and one enclosed dumpster area. • Elimination of the two existing entrances on Route 50 and construction of a new entrance. • A 4' concrete sidewalk along Route 50. • Landscaping as shown on sheet 5 of the site plan. As you are aware, this property is within the planned right-of-way for the relocation of Route 522. This plan was discussed at the Frederick County Planning Commissions' May 2, 2007 meeting and the Board of Supervisors' May 23, 2007 meeting. I am providing you with five (5) copies of the approved site plan. Please forward these copies to the appropriate representative(s). Perkins, Planner DATE: May 14, 2007 RE: Discussion — Blue Fox Billiards, Bar & Grill Site Plan At the May 2, 2007 Board of Supervisors meeting, staff will present the site plan for the Blue Fox Billiards, Bar & Grill for review. This site plan is being presented to the Board of Supervisors due to its implications on the planned relocation of Rollte 522. The site is located at the corner of Route 50 East (Millwood Pike) and Ryco Lane and is within the Shawnee Magisterial District; this site is the location of the old Harley Davidson Shop. hl an effort to keep the Board of Supervisors apprised of improvements that may affect planned road improvements, site plans and subdivisions that impact planned rights -of -way will be presented to the Planning Commission and Board of Supervisors for their review. A diagram of the site plan has been provided by the applicant and has been included in your agenda along with a sketch provided by VDOT that shows the proposed alignment for Route 522. Action on the site plan is not required; this site plan is being brought for informational purposes only. The Planning Commission reviewed this site plan at their May 2, 2007 meeting along with a waiver request for Zoning District Buffer along the back property line. Tile Planning Commission had minimal discussion on the site plan and approved the applicant's waiver request; action from the Board of Supervisors is not required oil the waiver request. This letter is to inform you of the approval of said plans as received by the Frederick County Health Department. It also serves to notify you that before issuing a food service permit for the facility, the following must be completed; 1. A complete set of floor plans is needed for completion of the plan review. Our department already has a copy of the site plans and needs a layout of the kitchen and other facilities regarding food prep and storage. 2. Water supply and sewage information ensuring that the establishment is hooked up to public water and sewage. 3. Information regarding floor, wall, and ceiling material. 4. A copy of the menu. 5. Equipment model numbers and Spec sheets for the equipment "Commercial equipment." 6. Insect and rodent control information. 7. Ensure that all equipment hooked to the water line has proper air gap and backflow prevention where needed. 8. A complete list of food suppliers and vendors must be submitted (this may be given to the Environmental Health Specialist at time of construction inspection). 9, Information explaining trash and or grease disposal method. 10. A member of our office must conduct a construction inspection sometime during the final stages of construction of the restaurant. A representative from this office may stop by to make sure things are going according to plan. 11. At least one week before opening the establishment, a member of our office needs to perform a pre -opening inspection.
