1. Contractor shall furnish all materials, labor and equipment required
and shall perform all work including restoration for the completed
installation of utility systems and all other improvements shown or
implied as necessary. Unless otherwise noted. specifications for all
work will be in accordance with the applicable Standards and Contract
Specifications of the Virginia Department of Transportation, Virginia
Department of Health, County of Frederick, and the contract documents.
2. Contractor to verify the exact location of all existing utilities and
notify the Engineer of any conflicts with existing and proposed
utilities. Damage to existing utilities shall be the Contractor's
responsibility.
3. Contractor shall be responsible for all construction stakeout. All
dimensions and elevations shown on the contract documents shall be
field verified by the Contractor prior to beginning work.
4. Contractor shall obtain. at his expense, all permits required prior to
starting work and shall adhere to the conditions imposed.
5. Contractor is responsible for compliance with the requirements of
SEC. 59.1-406. ET SEQ. Of the Code of Virginia (Overhead High Voltage
Line Safety Act) .
6. Any unusual subsurface conditions encountered during the course
of the work shall be brought to the attention of the Engineer.
7. Access by emergency vehicles must be maintained at all times. Access
to fire hydrants must not be impeded at any time by parked cars.
construction materials, soil stockpiles. etc.
8. Topographic and planimetric information shown on this plan was
developed from a field run survey performed by Traid Engineering, Inc.
dated December 3. 1998. Boundary information was taken from
available records.
CONSTRUCTION NOTES
EARTHWORK NOTES:
1. Erosion and sediment control measures shall be in place prior to
performing any significant earth disturbing activities.
2. Fill material placed below structures, controlled fill slopes, and the
top one (1) foot of pavement subgrade shall be compacted to at least
98% of the laboratory maximum dry density as determined by the
Standard Proctor method (ASTM D 698). Fill placed below the top
one (1) foot of the pavement subgrade, and all other areas shall be
compacted to at least 95% of the maximum dry density as determined
by the same standard. The moisture content of the soil should be at
or within three (3) percentage points of the optimum moisture content.
Compaction of the subgrade shall be certified by a professional
engineer or technician certified in the Commonwealth of Virginia.
3. Changes or revisions to these construction plans shall not be made
unless first approved by the appropriate agency having jurisdiction
over the change.
4. Traffic control signs shall comply with the standards of the Virginia
Department of Transportation.
5. For erosion and sediment control notes see the erosion and sediment
control detail sheet.
ROAD NOTES:
1. Subgrades on cut shall be compacted for a minimum of one (1) foot
in depth. Subgrade in fill shall meet the compaction requirements stated
above at all levels.
2. Areas of wet or soft material under roads or buildings shall be
partially over -excavated and replaced with suitable material.
3. Gravel used in base courses shall be constructed on a compacted
subgrade free of organic matter. Gravel shall be VDOT type 21-B and
shall be placed and compacted in a moist condition so that the fines
remain mixed with the coarser material.
4. Asphaltic concrete. where applicable, shall be applied to the prepared
gravel subbase following the application of a liquid asphalt prime coat.
Asphalt shall consist of a plant -mixed bituminous material rolled smooth.
The asphalt surface shall be smooth, seamless and of a uniform texture.
Asphalt material shall comply with the minimum specifications required
by the VDOT Road and Bridge Specifications.
5. The elevation of new pavement shall match the elevation of the
existing pavement at all intersections.
GENERAL ENTRANCE NOTES
1. All work on this project shall conform to the latest editions of the
Virginia Department of Transportation (VDOT) Road and Bridge Specifications,
and Standards, The Virginia Erosion and Sediment Control Regulations and
any other state, federal or local regulations applicable. In the event of
conflict between any of these standards. specifications or plans, the most
stringent shall govern.
2. All construction shall comply with the latest U.S. Department of Labor
Occupational Safety &, Health Administration and VOSH Rules & Regulations.
3. When working on VDOT right of way, all traffic control, whether
permanent or temporary, shall be in accordance with the current edition of
VDOT's work area protection manual. Furthermore, all traffic control flaggers
must be certified in accordance with VDOT's January, 1994 (or latest edition of)
Road & Bridge Specifications, Section 104.04-C.
4. Design features relating to construction or to regulation, control and
safety of traffic may be subject to change as deemed necessary by VDOT.
5. Prior to initiation of work. Contractor shall be responsible for acquiring
all necessary VDOT land use permits for any work on VDOT right of way.
6. If required by the local VDOT Residency Office, a preconstruction
conference must be arranged and held by the engineer and/or developer with
the attendance of the contractor. various County agencies. utility companies
and VDOT prior to initiation of work.
7. Contractor shall notify the local VDOT Residency Office when work is to
begin or cease for any undetermined length of time. VDOT will also require 48
hours notice for any inspection.
8. The Contractor will be responsible for maintaining adequate access to the
project from the adjacent public roadway through construction and maintenance
of a construction entrance in accordance with the Virginia Erosion & Sediment
Control Handbook. Sec 3.02. Furthermore. access to other properties affected
by this project shall be maintained through construction.

GENERAL EROSION AND SEDIMENT CONTROL NOTES
ES-1: Unless otherwise indicated, all vegetative and structural erosion
and sediment control practices will be constructed and maintained
according to minimum standards and specifications of the Virginia
Erosion and Sediment Control Handbook and Virginia Regulations VR
625-02-00 Erosion and Sediment Control Regulations.
ES-2: The plan approving authority must be notified one week prior to
the pre -construction conference, one week prior to the commencement
of land disturbing activity, and one week prior to the final inspection.
ES-3: All erosion and sediment control measures are to be placed prior
to or as the first step in clearing.
ES-4: A copy of the approved erosion and sediment control plan shall
be maintained on the site at all times.
ES-5: Prior to commencing land disturbing activities in areas other than
indicated on these plans (including, but not limited to, off -site borrow or
waste areas). the contractor shall submit a supplementary erosion
control plan to the owner for review and approval by the plan approving
authority.
ES-6: The contractor is responsible for installation of any additional
erosion control measures necessary to prevent erosion sedimentation as
determined by the plan approving authority.
ES-7: Site grading is to drain to the silt fences or sediment trap at all
times during land disturbing activities and during site development until
final stabilization is achieved.
ES-8: During dewatering operations, water shall be pumped into sediment
traps.
ES-9: The contractor shall inspect all erosion control measures
daily. Any necessary repairs or cleanup to maintain the effectiveness of
the erosion control device shall be made by the close of the day. The
contractor shall be responsible for the ultimate removal of all temporary
sediment control devices.
CONSTRUCTION NOTES
1. The contractor will be responsible for applying for and obtaining any
necessary disturbance permits which may be required by Frederick
County prior to the start of clearing and grading activities.
2. The contractor shall stabilize all disturbed areas within 7 days after
the area is brought to final grade.
3. Stabilization shall be by permanent seeding and mulching as follows:
a. Scarify the top 1" to 2" of soil after final grades have been
achieved.
b. Add pulverized agricultural limestone at the rate of 2 tons per
acre.

GENERAL NOTES
1. Contractor shall furnish all materials, labor and equipment required
and shall perform all work including restoration for the completed
installation of utility systems and all other improvements shown or
implied as necessary. Unless otherwise noted. specifications for all
work will be in accordance with the applicable Standards and Contract
Specifications of the Virginia Department of Transportation, Virginia
Department of Health, County of Frederick, and the contract documents.
2. Contractor to verify the exact location of all existing utilities and
notify the Engineer of any conflicts with existing and proposed
utilities. Damage to,existing utilities shall be the Contractor's
responsibility.
3. Contractor shall be responsible for all construction stakeout. All
dimensions and elevations shown on the contract documents shall be
field verified by the Contractor prior to beginning work.
4. Contractor shall obtain, at his expense, all permits required prior to
starting work and shall adhere to the conditions imposed.
5. Contractor is responsible for compliance with the requirements of
SEC. 59.1-406, ET SEQ. Of the Code of Virginia (Overhead High Voltage
Line Safety Act) .
6. Any unusual subsurface conditions encountered during the course
of the work shall be brought to the attention of the Engineer.
7. Access by emergency vehicles must be maintained at all times. Access
to fire hydrants must not be impeded at any time by parked cars,
construction materials, soil stockpiles. etc.
8. Topographic and planimetric information shown on this plan was
developed from a field run survey performed by Traid Engineering, Inc.
dated December 3. 1998. Boundary information was taken from
available records.
CONSTRUCTION NOTES
EARTHWORK NOTES:
1. Erosion and sediment control measures shall be in place prior to
performing any significant earth disturbing activities.
2. Fill material placed below structures, controlled fill slopes, and the
top one (1) foot of pavement subgrade shall be compacted to at least
98% of the laboratory maximum dry density as determined by the
Standard Proctor method (ASTM D 698). Fill placed below the top
one (1) foot of the pavement subgrade, and all other areas shall be
compacted to at least 95% of the maximum dry density as determined
by the same standard. The moisture content of the soil should be at
or within three (3) percentage points of the optimum moisture content.
Compaction of the subgrade shall be certified by a professional
engineer or technician certified in the Commonwealth of Virginia.
3. Changes or revisions to these construction plans shall not be made
unless first approved by the appropriate agency having jurisdiction
over the change.
4. Traffic control signs shall comply with the standards of the Virginia
Department of Transportation.
5. For erosion and sediment control notes see the erosion and sediment
control detail sheet.
ROAD NOTES:
1. Subgrades on cut shall be compacted for a minimum of one (1) foot
in depth. Subgrade in fill shall meet the compaction requirements stated
above at all levels.
2. Areas of wet or soft material under roads or buildings shall be
partially over -excavated and replaced with suitable material.
3. Gravel used in base courses shall be constructed on a compacted
subgrade free of organic matter. Gravel shall be VDOT type 21-B and
shall be placed and compacted in a moist condition so that the fines
remain mixed with the coarser material.
4. Asphaltic concrete, where applicable, shall be applied to the prepared
gravel subbase following the application of a liquid asphalt prime coat.
Asphalt shall consist of a plant -mixed bituminous material rolled smooth.
The asphalt surface shall be smooth, seamless and of a uniform texture.
Asphalt material shall comply with the minimum specifications required
by the VDOT Road and Bridge Specifications.
5. The elevation of new pavement shall match the elevation of the
existing pavement at all intersections.
GENERAL ENTRANCE NOTES
1. All work on this project shall conform to the latest editions of the
Virginia Department of Transportation (VDOT) Road and Bridge Specifications,
and Standards, The Virginia Erosion and Sediment Control Regulations and EXISTING
any other state. federal or local regulations applicable. In the event of - - - - - -
conflict between any of these standards, specifications or plans. the most
stringent shall govern.
2. All construction shall comply with the latest U.S. Department of Labor
Occupational Safety 8. Health Administration and VOSH Rules & Regulations.-------------
3. When working on VDOT right of way, all traffic control, whether
permanent or temporary, shall be in accordance with the current edition of
VDOT's work area protection manual. Furthermore, all traffic control flaggers
must be certified in accordance with VDOT's January, 1994 (or latest edition of
Road & Bridge Specifications. Section 104.04-C.

4. Design features relating to construction or to regulation, control and-------------
sof ety of traffic may be subject to change as deemed necessary by VDOT.
5. Prior to initiation of work. Contractor shall be responsible for acquiring
all necessary VDOT land use permits for any work on VDOT right of way.
6. If required by the local VDOT Residency Office, a preconstruction
conference must be arranged and held by the engineer and/or developer with -X x
the attendance of the contractor. various County agencies. utility companies
and VDOT prior to initiation of work. 0
7. Contractor shall notify the local VDOT Residency Office when work is to
begin or cease for any undetermined length of time. VDOT will also require 48
hours notice for any inspection.
8. The Contractor will be responsible for maintaining adequate access to the
project from the adjacent public roadway through construction and maintenance
of a construction entrance in accordance with the Virginia Erosion & Sediment
Control Handbook. Sec 3.02. Furthermore. access to other properties affected
by this project shall be maintained through construction.

GENERAL. EROSION AND SEDIMENT CONTROL NOTES
ES-1: Unless otherwise indicated, all vegetative and structural erosion
and sediment control practices will be constructed and maintained
according to minimum standards and specifications of the Virginia
Erosion and Sediment Control Handbook and Virginia Regulations VR
625-02-00 Erosion and Sediment Control Regulations.
ES-2: The plan approving authority must be notified one week prior to
the pre -construction conference. one week prior to the commencement
of land disturbing activity, and one week prior to the final inspection.
ES-3: All erosion and sediment control measures are to be placed prior
to or as the first step in clearing.
ES-4: A copy of the approved erosion and sediment control plan shall
be maintained on the site at all times.
ES-5: Prior to commencing land disturbing activities in areas other than
indicated on these plans (including, but not limited to. off -site borrow or
waste areas). the contractor shall submit a supplementary erosion
control plan to the owner for review and approval by the plan approving
authority.
ES-6: The contractor is responsible for installation of any additional
erosion control measures necessary to prevent erosion sedimentation as
determined by the plan approving authority.
ES-7: Site grading is to drain to the silt fences or sediment trap at all
times during land disturbing activities and during site development until
final stabilization is achieved.
ES-8: During dewatering operations, water shall be pumped into sediment
traps.
ES-9: The contractor shall inspect all erosion control measures
daily. Any necessary repairs or cleanup to maintain the effectiveness of
the erosion control device shall be made by the close of the day. The
contractor shall be responsible for the ultimate removal of all temporary
sediment control devices.
CONSTRUCTION NOTES
1. The contractor will be responsible for applying for and obtaining any
necessary disturbance permits which may be required by Frederick
County prior to the start of clearing and grading activities.
2. The contractor shall stabilize all disturbed areas within 7 days after
the area is brought to final grade.
3. Stabilization shall be by permanent seeding and mulching as follows:
a. Scarify the top 1" to 2" of soil after final grades have been
achieved.
b. Add pulverized agricultural limestone at the rate of 2 tons per
acre.
c. Seed with the following mixture:
