The checklist below indicates all the information that needs to be submitted as part of the site plan
application. Al required information must be submitted priorto the final approval of any site plan. The
Department of Planning and Development will review the application to ensure that it is complete. If
any portion is not included or complete, the site plan application will not be accepted and returned to
the applicant(s).
Site Plan Package
One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

The Frederick County Department of Planning and Development administratively approved the above -
referenced plan on May 23, 2003. The plan meets all requirements for development in the M 1 (Light Industrial)
Zoning District.. Below is a list of the primary improvements of this development, proposed for construction
in three phases:
• Two 12,000 square -foot buildings for use as shop/office facilities
• One 6,000 square -foot building for use as a shop/office facility
• Twelve 4,500 square -foot buildings for use as self -storage facilities
• 90 paved parking spaces, including five handicapped accessible
• Plantings to include 35 red maples, 53 white pines, 55 Japanese yew, nine little -leaf Lindens, and 64
Eastern red cedars
I am providing you with three copies of the approved site plan. Please forward these to the appropriate
representatives. Furtlicrmore, please advise the owner that a copy should be kept for future reference and an
approved copy must be kept on the construction site throughout the development process. Once site
development is complete, the owner should contact this office to schedule an on -site inspection. This is required
prior to the issuance of an occupancy permit by the Department of Building hnspections.

On Monday, May 12, 2003, you met with the writer for the purpose of inspecting a proposed
well site for approval as a public water supply source to serve the Clearbrook Storage and
]:rectors Plus, Inc. induntrial site in Frederick County_
In accordance with Paxt 1, Article 3, 12 VAC 5-590-280 of the Commonwealth of Virginia
Waterworks Regulations, this letter is to advise that the proposed well site, as approximately
located on the attached portion of the Inwood, West Virginia -Virginia geological survey reap
section, is tentatively approved by this office for construction of a Class II-B well. The well
must be cased an cement grouted to a depth of at least 50 feet below ground.
The following paragraphs apply to any well drilled on the above site.
The well must be located a minimum of 50 feet from public roadways, parking lots, property
lines, sewer lines, sewage gravity settling tanks, stormwater drainage facilities, flowing
streams, or naturally wet areas. The well site will need to be graded so that water flowing
from higher elevations does not directly cross the well site. The well site must be located
above the 100-year flood elevation..
This approval is valid for a ueriod of 12 mozxtkzs. If construction of the well teas not
commenced by May 13, 2004, a reinspection of the well site will be required. The well must
be located at least 50 feet inside the boundaries of a platted, dedicated well lot, and an all
weather access road, public or private, must be provided to the well lot.
During construction of the well, it: is requested that the Lexington Environmental
Engineering Field Office be notifxed of the date and approximate time that grouting of the

well will be accomplished. Please transmit this notification as soon as the information is
available to you so that an engineer from this office can be present, if possible.
Upon completion of the well, please forward a copy of the Water Well Completion Report to
this office as soon as possible. The completion, report will be evaluated to kelp determine the
potential for surface water influence based on parent material and. geological structure at the
selected well site. Based on this evaluation, you will be notified in writing as to the length of
the required yield and drawdown test and provided with a detailed list of the sampling
requixt!ments for the well.
Once the appropriate yield testing and sampling have been completed for the well, plans and
specifications for construction of the well and development of the well head must be
submitted to the Division for review in accordance with Part I, Article 3, 12 VAC 5-590-280,
Paragraphs 4, 5, and 6 of the Regulations. Included with dic submittal of plans and
specifications for the well must be copies of the Water Well Completion Report, recorded plat
of the well lot, recorded dedication document for the well lot stating that the lot will be used
only for waterworks appurtenances as long as the well thttreoD is utilized as a, source of
water for a public water supply, ing of the required, pumping test, and copies of all the
required sample results.
Upon receipt of all required information and documents, and after, approval of the plans and
specifications has been given, a construction permit will be issued by the State Health
Commissioner in accordance with Part 1, Article 3, 12 VAC 5-590-230 of the Regulations.
Construction of the facilities Shall not commence until receipt of the construction permit,
