All required information must be submitted prior to the final approval of any site plan. The Department of Planning and Development will review the application to ensure that it is complete. Provide concrete wheel stops for all parking spaces. Provide directional signaaeand directional arrows on pavement to clarify intended vehicle travel around the site. 3. Box 41 Winchester, Virginia 22604 Dear Mr. Lesman: Plans and specifications for a proposed sewage treatment works, located in Frederick County, as prepared by Painter -Lewis, P.L.C., Winchester, Virginia, have been received by this Department. The plans and specifications consist of five sheets titled "Unitarian Universalist Church, Valley Pike (U.S. Route ii), Back Creek Magisterial District, Frederick County, Virginia" and sealed by the engineer on November 14, 2002. A General Permit (VPDES Permit No. VAG401333) has been issued for the design flow of less than i,000 gpd by the Virginia Department of Environmental Quality (DEQ). The proposed treatment works consists of a septic tank, aerated dosing siphon, evapotranspiration bed, chlorination, dechlorination, and post -aeration, with a discharge to an unnamed tributary to West Run. The evaluation of the project design has been confined to technical requirements and design criteria as stipulated in the Commonwealth of Virginia Sewerage Regulations (12 VAC 5-580), together with currently accepted standards of practice and best professional judgment. It is recommended that a water meter be used on the potable supply to estimate flow to the sewage treatment works. In accordance with the Code of Virginia 1950, as amended, Title 62.1, Chapter 3.1, Article 4, Section 62.1- 44.19, Paragraph 3, and 'Tale 32.1, Chapter 6, Article 1, Section 32.1-164, this letter constitutes permission to construct the subject facility based on your application for a discharging system, request for a construction permit, and the aforementioned plans and specifications, which have been found to be technically adequate. This construction permit remains valid for a period of time concurrent with the existing General Permit. Enclosed for your use is one set of engineering drawings with a Virginia Department of Health approval sticker. It should be noted that a system designed for this property in 1992 had certain constraints levied to protect a nearby spring used as a drinking water supply (see attached letter by Dr. Robert B. Stroube dated April 10,1992). The proposed treatment works is designed to evapotranspirate the effluent flow and essentially not discharge. The engineer has designed the system to produce an effluent quality of less than 10 milligrams per liter for the parameters of Biochemical Oxygen Demand (five day) and suspended solids. It is required that semi-annual samples be collected, if the system discharges, and the results submitted to this office for evaluation. It is to be understood that, if the treatment works does not meet these performance criteria, the owner shall address the deficiencies through an upgrade of the treatment processes. Upon completion of construction, the owner shall submit to this office a statement signed by a licensed professional engineer or contractor that the system was installed and constructed in accordance with the approved plans and specifications.
