Any discrepancies found between the drawings and site conditions or any inconsistencies or ambiguities in drawings shall be immediately reported to the engineer in writing, who shall promptly address such inconsistencies or ambiguities in writing. Work done by the contractor after his discovery of such discrepancies, inconsistencies or ambiguities, without clarification from the engineer of record, shall be done at the contractor's risk. 2. Contractor shall furnish all materials, labor and equipment required and shall perform all work including restoration for the completed installation of utility systems and all other improvements shown or implied as necessary. Unless otherwise noted, specifications for all work shall be in accordance with the applicable Standards and Contract Specifications of the Virginia Department of Transportation, Virginia Department of Health, County of Frederick, Frederick County Sanitation Authority, and the contract documents. 3. Contractor to verify the exact location of all existing utilities and notify the Engineer of any conflicts with existing and proposed utilities. Damage to existingutilities shall be the Contractor's responsibility. Forty-eight (48) hours prior to any excavation call "Miss Utility" 1-800-552-7001. 4. Contractor shall be responsible for all construction stakeout. All dimensions and elevations shown on the contract documents shall be field verified by the Contractor prior to beginning work. 5. Contractor shall obtain, at his expense, all permits required prior to starting work and shall adhere to the conditions imposed. 6. Any unusual subsurface conditions encountered during the course of the work shall be brought to the attention of the Geotechnical Engineer / Owner. 7. Access by emergency vehicles must be maintained at all times. Access to fire hydrants must not be impeded at any time by parked cars, construction materials, soil stockpiles, etc. 8. Refer to architectural drawings for exact building dimensions. CONSTRUCTION NOTES: EARTHWORK NOTES: 1. Erosion and sediment control measures shall be in place prior to performing any significant earth disturbing activities. This plan is incomplete without the Erosion Control Narrative and all addendums. A preconstruction conference shall be held prior to the issuance of the land disturbing permit. The contractor shall arrange the meeting with the County E & SC Administrator. 2. All materials used for fill or back -fill shall be free of wood, roots, rocks, boulders or any other non -compactible soil type material. Unsatisfactory materials also include man-made fills and refuse debris derived from any source. 3. Satisfactory material for use as fill for public streets include materials classified in ASTM D-2487 as GW, GP, GM, GC, SW, SP, SM, SC, ML and CL groups. The moisture content shall be controlled within plus or minus 2 percentage points of optimum to facilitate compaction. Generally, unsatisfactory materials include materials classified in ASTM D-2487 as PT, CH, MH, OL, OH, and any soil too wet to facilitate compaction. CH and MH soils may be used subject to results of the Geotechnical Report and upon approval from VDOT. Soils shall have a minimum dry density of 92 lb/cu. ft. per ASTM D-698 and shall have a plasticity index less than 17. 4. Materials used to construct embankments for any purpose, back -fill around drainage structures or in utility trenches or any other depression requiring fill or back -fill shall be compacted to 95% of maximum density as determined by the standard proctor test as set out in ASTM Standard D-698. The contractor shall, prior to any operations involving filling or back -filling, submit the results of the proctor test together with a certification that the soil tested is representative of the materials to be used on the project. Tests shall be conducted by a certified materials testing laboratory and the certifications made by a licensed professional engineer representing the laboratory. 5. Compaction of fill material under building slabs shall be based upon recommendations of soils engineer after completion of standard proctor test and shall meet bearing requirements of architect for buildings. The contractor shall be responsible for testing. 6. Embankment fill and trench back -fill shall be placed in lifts at a maximum uncompacted depth of 8 inches and 6 inches, respectively. Density tests shall be conducted at the following minimum frequencies: (a) embankments for roads, streets, dams, etc.: one test per lift per 10,000 square feet of lift. (b) back -fill around structures and in trenches: one test per lift per 500 lineal feet of trench. 7. Compaction tests for street pavement structure shall be made in cut and fill areas at the following minimum frequencies: (a) Sub -grade: one test per lane per 500 lineal feet. (b) Stone base: one test per lane per 6" compacted lift per 500 lineal feet. (c) Hot asphaltic concrete: one test per lane per lift per 500 lineal feet. 8. Changes or revisions to these construction plans shall not be made without approval by the owner, engineer and all applicable regulatory agencies. ROAD NOTES: 1. Subgrades on cut shall be compacted for a minimum of one (1) foot in depth. Subgrade in fill shall meet the compaction requirements stated above at all levels. 2. Areas of wet or soft material under roads or buildings shall be partially over -excavated and replaced with suitable material as directed by a Geotechnical Engineer. 3. Gravel used in base courses shall be constructed on a compacted subgrade free of organic matter. Gravel shall be VDOT type 21-B and shall be placed and compacted in a moist condition so that the fines remain mixed with the coarser material. 4. Asphaltic concrete, where applicable, shall be applied to the prepared gravel subbase following the application of a liquid asphalt prime coat. Asphalt shall consist of a plant -mixed bituminous material rolled smooth. The asphalt surface shall be smooth, seamless and of a uniform texture. Asphalt material shall comply with the minimum specifications required by the VDOT Road and Bridge Specifications. 5. Roadwork within existing or planned VDOT right of way shall conform to the latest editions of the Virginia Department of Highways Road and Bridge Specifications and Standards and Virginia E & SC Regulations. VDOT GENERAL NOTES: 1. All work on this project shall conform to the latest editions of the Virginia Department of Transportation (VDOT) Road and Bridge Specifications, and Standards, The Virginia Erosion and Sediment Control Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards, specifications or plans, the most stringent shall govern. 2. All construction shall comply with the latest U.S. Department of Labor Occupational Safety and Health Administration and VOSH Rules and Regulations. 3. When working on VDOT right of way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition of VDOT's work area protection manual. Furthermore, all traffic control flaggers must be certified in accordance with VDOT's January, 2001 (or latest edition of) Road and Bridge Specifications, Section 104.104-C. 4. Design features relating to construction or to regulation, control and safety of traffic may be subject to change as deemed necessary by VDOT. 5. Prior to initiation of work, Contractor shall be responsible for acquiring all necessary VDOT land use permits for any work on VDOT right of way. 6. If required by the local VDOT Residency Office, a preconstruction conference must be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. 7. Contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT will also require 48 hours notice for any inspection. 8. The contractor will be responsible for maintaining adequate access to the project from the adjacent public roadway through construction and maintenance of a construction entrance in accordance with the Virginia Erosion and Sediment Control Handbook, Sec. 3.02. Furthermore, access to other properties affected by this project shall be maintained through construction. 9. Site ADT Trip Generation 68 FCSA CONSTRUCTION NOTES: 1. Water and sewer lines are to be constructed in accordance with these plans. The lines shall additionally meet all requirements of the (Frederick County Sanitation Authority. 2. The contractor shall coordinate with, and arrange for inspection by, the Authority. 3. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's inspector. 4. The Sanitation Authority's maintenance department is resonsible for the installation of water meters through 2" in size. Before a permanent meter is set, they require: 1) the frame and cover to be properly aligned with its meter box and yoke bar; 2) the frame and cover to be set to the final grade of the lot; 3) the distance between the top of the cover and the yoke bar to be between 18 and 21 inches; and 4) all components of the meter box assembly to be in proper working order. It is the developer's/contractor's responsibility to have the meter box assembly installed correctly. 5. The contractor shall install water and sewer services that connect to existing lines. a. A water service (and force main sewer service) requires the installation of a saddle with corporation stop and making the tap. b. A gravity sewer service requires the installaton of a wye, 18" long stub pieces, and 15" long, full circle, stainless steel repair clamps. c. The FCSA inspector assigned to the project shall be present for the installation of each connection. 6. When an existing water main is tapped for the installation of a new line: a. The Authority shall furnish all labor and materials to make the tap. b. The developer/contractor must: (1) make application for the tap, (2) pay deposit (before work begins); (3) mark the location of the tap; (4) make final payment when the work is completed. Erosion and sediment control measures shall be in place prior to performing any significant earth disturbing activities. This plan is incomplete without the Erosion Control Narrative and all addendums. A preconstruction conference shall be held prior to the issuance of the land disturbing permit. The contractor shall arrange the meeting with the County E & SC Administrator. 2. All materials used for fill or back -fill shall be free of wood, roots, rocks, boulders or any other non -compactible soil type material. Unsatisfactory materials also include man-made fills and refuse debris derived from any source. 3. Satisfactory material for use as fill for public streets include materials classified In ASTM D-2487 as GW, GP, GM, GC, SW, SP, SM, SC, ML and CL groups. The moisture content shall be controlled within plus or minus 2 percentage points of optimum to facilitate compaction. Generally, unsatisfactory materials include materials classified in ASTM D-2487 as PT, CH, MH, OL, OH, and any soil too wet to facilitate compaction. CH and MH soils may be used subject to results of the Geotechnical Report and upon approval from VDOT. Soils shall have a minimum dry density of 92 lb/cu. ft. per ASTM D-698 and shall have a plasticity index less than 17. 4. Materials used to construct embankments for any purpose, back -fill around drainage structures or in utility trenches or any other depression requiring fill or back -fill shall be compacted to 95% of maximum density as determined by the standard proctor test as set out in ASTM Standard D-698. The contractor shall, prior to any operations involving filling or back -filling, submit the results of the proctor test together with a certification that the soil tested is representative of the materials to be used on the project. Tests shall be conducted by a certified materials testing laboratory and the certifications made by a licensed professional engineer representing the laboratory. 5. Compaction of fill material under building slabs shall be based upon recommendations of soils engineer after completion of standard proctor test and shall meet bearing requirements of architect for buildings. The contractor shall be responsible for testing. 6. Embankment fill and trench back -fill shall be placed in lifts at a maximum uncompacted depth of 8 inches and 6 inches, respectively. Density tests shall be conducted at the following minimum frequencies: (a) embankments for roads, streets, dams, etc.: one test per lift per 10,000 square feet of lift. (b) back -fill around structures and in trenches: one test per lift per 500 lineal feet of trench. 7. Compaction tests for street pavement structure shall be made in cut and fill areas at the following minimum frequencies: (a) Sub -grade: one test per lane per 500 lineal feet. (b) Stone base: one test per lane per 6" compacted lift per 500 lineal feet. (c) Hot asphaltic concrete: one test per lane per lift per 500 lineal feet. 8. Changes or revisions to these construction plans shall not be made without approval by the owner, engineer and all applicable regulatory agencies. ROAD NOTES: 1. Subgrades on cut shall be compacted for a minimum of one (1) foot in depth. Subgrade in fill shall meet the compaction requirements stated above at all levels. 2. Areas of wet or soft material under roads or buildings shall be partially over -excavated and replaced with suitable material as directed by a Geotechnical Engineer. 3. Gravel used in base courses shall be constructed on a compacted subgrade free of organic matter. Gravel shall be VDOT type 21-B and shall be placed and compacted in a moist condition so that the fines remain mixed with the coarser material. 4. Asphaltic concrete, where applicable, shall be applied to the prepared gravel subbase following the application of a liquid asphalt prime coat. Asphalt shall consist of a plant -mixed bituminous material rolled smooth. The asphalt surface shall be smooth, seamless and of a uniform texture. Asphalt material shall comply with the minimum specifications required by the VDOT Road and Bridge Specifications. 5. Roadwork within existing or planned VDOT right of way shall conform to the latest editions of the Virginia Department of Highways Road and Bridge Specifications and Standards and Virginia E & SC Regulations. VDOT GENERAL NOTES: 1. All work on this project shall conform to the latest editions of the Virginia Department of Transportation (VDOT) Road and Bridge Specifications, and Standards, The Virginia Erosion and Sediment Control Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards, specifications or plans, the most stringent shall govern. 2. All construction shall comply with the latest U.S. Department of Labor Occupational Safety and Health Administration and VOSH Rules and Regulations. 3. When working on VDOT right of way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition of VDOT's work area protection manual. Furthermore, all traffic control flaggers must be certified in accordance with VDOT's January, 2001 (or latest edition of) Road and Bridge Specifications, Section 104.04-C. 4. Design features relating to construction or to regulation, control and safety of traffic may be subject to change as deemed necessary by VDOT. 5. Prior to initiation of work, Contractor shall be responsible for acquiring all necessary VDOT land use permits for any work on VDOT right of way. 6. If required by the local VDOT Residency Office, a preconstruction conference must be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. 7. Contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT will also require 48 hours notice for any inspection. 8. The contractor will be responsible for maiintaining adequate access to the project from the adjacent public roadway through construction and maintenance of a construction entrance in accordance with the Virginia Erosion and Sediment Control Handbook, Sec. 3.02. Water and sewer lines are to be constructed in accordance with these plans. The lines shall additionally meet all requirements of the Frederick County Sanitation Authority. 2. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's inspector. 4. The Sanitation Authority's maintenance department is resonsible for the installation of water meters through 2" in size. Before a permanent meter is set, they require: 1) the frame and cover to be properly aligned with its meter box and yoke bar; 2) the frame and cover to be set to the final grade of the lot; 3) the distance between the top of the cover and the yoke bar to be between 18 and 21 inches; and 4) all components of the meter box assembly to be in proper working order.
