Enclosed is your approved Land Use Permit 9855-003625 to cover the reconstruction and
maintenance of one commercial entrance and turn lanes at the referenced location.
Please ensure a copy of the approved permit assembly is kept at the job site whenever working
within the Route 50 right-of-way. I would also like to remind you of the need to erect and
maintain proper traffic control in accordance with our Virginia Work Area Protection Manual.
For your information the account receivable number for this project is 0245 8531. Any inspection
costs we incur will be billed to you monthly.
As you are aware, we have agreed to furnish the 200' of 15" R.C.P. culvert for the left turn -lane
improvements. We will need at least thirty days notice from your office when the pipe is needed
to ensure we have the pipe available.
Please give Mr. Steve Melnikoff a call at (540) 722-3460 at least 48 hours in advance of the start
of construction to arrange for any necessary inspections.

Dear Mr. Smith:
As requested, we have reviewed your site plan dated February 22, 1994 for the
referenced project. Our comments may be found on the attached plans marked in red
and as follows:
1. Curb & gutter should be Standard CG-7.
2. The southeast taper should consist of a 12' offset from the edge of pavement and be
36' in length.
3. The culvert under the entrance should be 15" R.C.P. rather than 15" C.M.P.
4. An extended paved ditch is needed to the retaining wall.
5. Due to potential safety hazards, the retaining wall will not be permitted on the
State's right-of-way. Please redesign to reflect the retaining wall off right-of-way.
Please revise and resubmit six copies of the plan for final approval. Should any
changes be deemed necessary, please design them to meet or exceed the above
recommendations.

RE: Rosedale Baptist Church Multi -Purpose Building Site Plan Approval
Dear Mr. Smith:
The Frederick County Planning Department administratively approved Site Plan #023-94 for
Rosedale Baptist Church on October 18, 1994. The approved site plan calls for the construction
of a two story multi -purpose building that will be 11,890 square feet, the expansion of the
asphalt parking lot that will provide 34 regular parking spaces and two disabled parking spaces,
the provision of right and left turn lanes along Northwestern Pike, the widening of the existing
entrance complete with drainage improvements, the relocation of the existing sign, the provision
of a concrete dumpster pad that will be enclosed by a six foot high stockade fence, the creation
of a stormwater management facility that complies with the specifications made by the County
Engineer, and all appropriate erosion and sedimentation control measures.
I have provided you with two copies of the approved site plan. This will allow you to provide
the contractor with an on -site copy and keep a copy for your records. Please have the contractor
notify this office to schedule an on -site inspection once all improvements are complete.

RE: Preliminary Site Plan Review of Rosedale Baptist Church Classroom Addition
Dear Keith:
I have had an opportunity to review the above referenced site plan. The following comments
need to be addressed prior to final approval of this site plan:
1) Please review the adjoining property owners. It appears that there is some
inconsistencies in our mapping information and the information provided on this plan.
2) Please provide the metes and bounds description for the three parcels.
3) Please provide the proposed use for the new facility.
4) Please provide height of all structures located on this property.
5) Please provide the dimensions of the sign that is planned to be relocated. This sign
needs to be placed a minimum of 10 feet from the property line.
6) Please provide the location of any proposed or existing outdoor lighting fixtures, outdoor
storage areas, or outdoor trash receptacles.
7) Please provide the parking calculations that verify the requirements for 120 parking
spaces.
8) Please provide the dimensions for all regular parking spaces, disabled parking spaces,
and parking aisles.
9) Are directional arrows provided within the parking lot as indicated on the site plan? It
would be beneficial to provide parking lot landscaping in this area due to the removal of
existing vegetation.
Please provide our department with the site plan review fee, the approved County Engineer
comments, and five copies of the final site plan which reflect the comments specified in this
letter. Our department has placed this site plan on the Planning Commission's Bimonthly Report
of September 7, 1994. Our department will provide administrative approval for this plan once
all information is received and the Planning Commission has had an opportunity to request or
waive formal review.

This is to acknowledge receipt of your revised site plan dated August 8, 1994 for the referenced
project. It appears all of our earlier comments have been satisillctorily addressed.
Your proposal to insert the 15" R.C.P. storm sewer culvert into the side of the existing median inlet
seems workable. However, to ensure our minimum compaction requirements are met, it will be
necessary to backfill the pipe under the counter levered portion of the structure with Class A-3
Concrete.
Please advise the owner of our approval and that a land use permit may now be applied for to cover
work on the Route 50 right-of-way. The permit will require a minimal processing fee, surety bond
coverage and the salary & expenses of a State assigned Inspector. A permit of this nature normally
takes approximately thirty (30) days to process and issue once satisfactory application has been
made.

We have reviewed the above referenced project's revised site plan dated
July 14, 1994. Our comments may be found on the enclosed plan marked in red and
as follows:
1. The proposed 3001+ of 15" R.C.P. culvert in the Route 50 median will be too
long when considering future maintenance. A standard DI-1 drop inlet will
be required midway between the proposed manhole and existing structure.
2. Additional detail will need to be provide for the modifications to the
existing median inlet. It appears the proposed top elevation is too high
when considering the edge of pavement. Also, why can't the proposed 15"
R.C.P. invert be set at elevation 48.59?
3. Other miscellaneous marked comments will need to be incorporated into the
plan.
Once revised please resubmit six (6) copies for final approval. Should any
changes be deemed necessary, please design them to meet or exceed the above
recommendations.

Dear Bruce:
In response to our telephone conversation on August 19, 1994, I
recommend that the following consideration be given to stormwater
management related to the proposed site development:
1) The increase in the ten (10) year storm post -development
should be determined for the proposed development and the
cumulative on -site development which would include the
previous development. If the construction of the multi-
purpose building and related parking (approximately 16, 000
sf) results in an increase in stormwater flow greater than
one (1) csf, a stormwater management plan will be required.
2) Because the existing and proposed topographic conditions
may prohibit the construction of an on -site stormwater
management facility, consideration may be given to off -site
management and/or easement. For example, we have approved
the discharge of increased stormwater flows onto adjacent
sites if the adjacent landowners agree to accept and manage
these increased flows. A recorded easement would be
required in this instance.
3) The proposed parking and building areas should include
stormwater conveyance channels or piping to minimize
erosion. For example, the proposed steep slopes (steeper
than 3h:ly) should include a paved channel to prevent
erosion and possible stability problems.
4) Foundation drains will be required along the northwest and
northeast sides of the proposed building.

based on the assumption that the above comments will be implemented

on the church property. According to Mr. Steven SLiefel,Environmental Health
Manager,the gravity line from the septic tanks to the distribution box is to be
considered a Lility line which requires it to be a minimum of ten feet f.rorn
any drainiiel•i ditch, proposed or existing.
