CONSTRUCTION NOTES GENERAL 1. ,The contractor shall furnish all materials, Ilabor and equipment and perform all work including restoration for the completed installation of the utility systems anrd all other improvements shown or'implied as necessary for the completed facility ready for use, Unless otherrwise noted, specifications for all work to be in accordance with applicable Standards and Construction Specificcotions of The Virginia Department of Transportation, Virginia Department of' Health, Frederick County Sanitatiion Authority and the contract documents. 2. The engineer has attempted to„ locate existting subsurface utilities; however, such may exist 'that are not shown. The contra( or sholl exercise care in this work( so as to avoid damage to any utilities. Any damage shall be thp, contractor's respponsibility. The contractor shaill notify all utility companies 72 hours prior to construction. Call Miss Utility" 1-800-257-7777. 3. The contractor shall be responsible for all construction stakeout. All dimensions, elevations and locations shall be verified in the field by each contractor prior to beginning the work. The Engineer will approve all cut sheets. 4. An unusual subsurface conditions encounttered during the course of the work shall be immediately brought to 5.e Contractor tothe obt nn,natrh s ex ense, all npermits required en gand/or p p eq y dhere, to the permit ' ed of this work. and shall faithful{ a conditions imposed. 6. All grading for improvements to be confined to the project area. Any work performed offsite is the Contractor's responsibility. 7. Cutsheets shall be submitted' to -the Engineer for approval prior to construction. All cutsheets should show existing ground elevations at centerline of utilitty, structure or road, and offset hub elevation: EROSION CONTROL 1. A Land Disturbance Permit ,will be requiredt prior to initiating any site work. This permit can be obtained from the Frederick County Public Works Department'' 2. Temporary and permanent erosion control structures and methods to be in accordance with the Virginia - Erosion Control Guidelines and local ordinances, and to be utilized during the construction period. 3. All erosion control structures shown or as required during construction shall be continuously, maintained throughout the period in, which areas are denwded.' Areas shown to be ,surfaced are to,havellstone base installed' at the earliest possible date after establishing grade.; No area is to remain denuded for more than 7 calendar days when construction is not in progress. 4. 'Contractor to clean up and restore disturbed areas immediately upon completion of pipe -laying in the affected. area. 5. All unpaved areas of the site which. ore dlisturbed by this construction shall be provided with 4" topsoil, fertilized; seeded, compacted and mulched. Seed bed to be watered and cared for as necessary to provide a good lawn quality grass cover. Areas not graded shall be disced, leveled and seeded to achieve lawn conditions. FIRE & RESCUE 1. - Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited. 2. Access for emergency vehicles must be rmaintained at all times duringconstruction. Temporary street signs must be posted as construction begins. Contiractor must ensure that parked vehicles do not impede access to fire hydrants. Fire Hydrants are not to be obstruccted at any time. FREDERICK COUNTY SANITATION AUTHORITY 1. Water and sewer lines are to be constructed in accordance with these plans and specifications and shall additionally meet all requirements of the Frederick Count Sanitation Authority, The contractor shall coordinate with this agency and arrange for inspection byr the Sanitation Authority as necessary. 2. Exact locations for sewer and water services to be directed by the, Frederick County Sanitation Authority inspector in the field. Contractor to provide (assistance as the Inspector may require in this effort. 3. Temporary termination of sewer line shall be plugged with a watertight plug, The ends of the laterals shall be marked with a, treated 2x4 post extending from the plug eighteen (18) inches above the ground. The top six (6) inches of the post shall be painted green. Sewer lateral elevations to be compatible with gravity service to proposed buildings as shown on the Architectural drawings. 4. The FCSA is responsible for the installation of., water meters. Before a permanent meter is set they will require: 1) the frame and over be properly aligned with its meter box and yoke bar, 2) 'the frame and cover be set to final grade of the lot, 3� the distance between the top of the cover and the yoke bar be between 18 and 21 inches, and 4) all components of he meter box assembly to bein proper working order. It is the contractors responsibility to have the meter box installed correctly. 5. The Frederick County Sanitation Authority shall install all water and sewer services that connect to existing lines. The location of the tap and the termination point of the lateral. are to be coordinated with the Authority's assistant engineer. 6. The Frederick County Sanitation Authority shall tap all existing waterlines. They will furnish the materiels for and make the tap. The contractor must make application for and poy the cost of t is work. 7. Frederick County Sanitation Authority apprroval of this site plan expires two (2) years from. date of approval. VIRGINIA DEPARTMENT OF TRANSPOR'ITATION 1. All work on this project shall conform to the latest editions of the Virginia Department of Transportation (VDOT). When working on VDOT right, -of -way, all ttraffic control, whether permanent or temporary, shall be in accordance with the current edition of VDOT' work area {protection manual; Furthermore, all traffic control floggers must be certified in accordance with VDOTs January, 11994 (or latest edition of) Road & Bridge Specifications, Section 104.04- C. 4. 'Design features relating .to construction err to regulation, control and safety of traffic may .be, subject to change as deemed necessary by VOOT. 5. Prior to init iation of work, Contractor shall] be responsible for acquiringr all necessary VDOT land use permits for any it, on, VDOT right-of-wa . 6. If require by the local VDOT residency 0)ffice, a preconstruction conference must 'be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. /. Contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT will also require 48 hoiurs notice for any inspection. • 8. The Contractor will be responsible for maiintaining adequate access to the project from the adjacent public road- way through construction and maintenance. of .a construction entrance in accordance with .the Vir inia Erosion & Sediment Control Handbook, Sec 3.02. Furtheermore, access to other properties affected by this -project shall be maintained through construction. 9. Contractor shall ensure adequate drainages is achieved and maintained` on the site during and at the end of construction. 10. In all cuts stone is to be placed in the trench daily up to a maximum length of 500 feet, it which time the pavement shall be covered with a temporary or permanent asphalt patch. If the application of the bituminous laver is delayed for adverse weather conditions, the contractor shall provide and maintain a base course that is acceptable to the VDOT until such time as the appropriate pavement patch can be applied and completion of the installation of the gas, sewer, and water lines, contractor shall restore pavement in the manner prescribed on the VDOT permit within (10) days. 12.
