This letter is to inform you that the Frederick County Department of Planning and Development
administratively approved Site Plan #014-98 on April 13, 1998. This site plan consists of a 1,120
square foot building addition to be utilized for bathrooms and storage.
Enclosed are two copies of the approved plan. You may provide your contractor with one copy, and
you may keep the other copy for the church's records. Please have the contractor contact our
department to schedule an on -site inspection once all improvements have been completed.

22, 1997, with the Frederick -Winchester Environmental Health Department has been evaluated
in accordance with the requirements contained in the Code of Virginia, Section 32.1-163, the
Sewage Handling And Disposal RegLilations and current agency policy and procedures for
processing on -site disposal systems.
Based on the information filed with your application, and the site and soil evaluations
conducted by Mr. Robert Holmes, Soil Consultant, and the Department's representative, your
application for a Sewage Disposal System Construction Permit for the above referenced location

In accordance with the Virginia Administrative Process Act, Title 9, Chapter 1.1:1 and
Section 32.1-164.1 of the Code of Virginia, this letter is to further inform you of your right of
appeal from the decision of the Department, which is specified in this letter.
If you desire to pursue your right of appeal, you may at your discretion be represented by
counsel. The first step in the appeal process is to submit to Dr. Mohamed Khodr, District
Director, C/O Mr. Kelly Vanover, Manager, 107 N. Kent St., Suite 201, Winchester, VA 22601,
(540)722-3480, a written request detailing and outlining all the facts and such other data or
information which will form the basis for your appeal.
In accordance with GMP #92, dated August 26, 1997, your right to appeal the
Department's decision concerning the previously noted Application for a Sewage Disposal
Construction Permit will expire thirty (30) days fi•om the receipt of this notice. If you desire to
pursue your right of an appeal after this allotted time period has expired, you will be required to
submit a new Application along with the appropriate filing fee.
Enclosed with this letter is an application for a permanent pump and haul system. If you
wish to proceed with a pump and haul system, please complete the enclosed application and
return it to the Health Department. Please note that approval of a permanent pump and haul
system is that of the County Administrator and not the Health Department. Also, an easement or
a written agreement granting permission to install the building sewer will be needed from the
utility company and the adjacent land owner with the 50' right of way in order to locate the
building sewer across the utility easement and the 50' right of way.
