SITE PLAN CHECKLIST
The checklist below indicates all the information ..that needs to be submitted as part of the site plan
application. All required information must -be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).
Site Plan Package
1. One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

Dear Mr. & Mrs Dousa-.
The Frederick County Planning Department administratively approved the above referenced site plan,
#066-98, on November 18, 1998. This site plan, for the construction of a nineteen hole miniature golf
course and the renovation of the existing structure to accommodate an office for the facility, meets all
County requirements for development within the RA (Rural Areas) Zoning District and is consistent with
the approved Conditional Use Permit (CUP #004-98). The site plan also calls for the provision of thirty
(30) parking spaces and buffering from the adjoining residential properties.
I previously provided you with three copies of the approved site plan. Please keep an approved copy for
your files and maintain an approved copy on the construction site. The third copy should be forwarded
to Mr. Gary Oates of Greenway Engineering for his records. Once the site development has been
completed, you should contact this office to schedule a final site inspection. Upon completion of all site
improvements, a Certificate of Occupancy may be issued for the business.

Dear Mr. and Mrs. Dousa:
Upon review of the revised site plan dated November 5, 1998, we offer the following
comments:
1. Stormwater management will not be required for this development at this time.
However, if any expansion occurs, stormwater management will be required. The
pre -developed run-off coefficient for the entire site will be 0.35 for these future
calculations.
All other comments have been addressed. Therefore, we recommend approval of the
subject site plan dated November 5, 1998. _

Dear Mr. and Mrs. Dousa:
Upon review of the subject site plan, we offer the following comments:
1. The site plan does not indicate the type of surface that will be used for the
miniature golf course. If artificial turf with concrete pads are used, a run-off "C"
value of 0.9 must be used for the site. It will then be necessary to implement
stormwater management on -site to reduce the ten-year run-off rate. Also, a
downstream drainage easement may be required if there is an increase in
stormwater run-off. If natural grass is used, indicate this on the drawings and
revise the calculations accordingly for the entire drainage area.
2. An 18 inch RCP culvert drains onto the property from Route 522. An adequate
channel needs to be constructed or an extension of the culverts will be required in
order to allow the stormwater run-off to drain through the site.
3. The site plan does not indicate any grading for the golf course. Indicate finished
grades for the site. Also, erosion and sediment control measures will be required
for the site. Place a note on the site plan indicating that a land disturbance permit
will be required prior to commencing any site work. Include an erosion and
sediment control narrative as well.
Once these comments have been addressed, submit two (2) copies of the site plan for
further review.

Dear Mr. and Mrs. Dousa:
This letter is to confirm action taken by the Frederick County Board of Supervisors at their meeting of
August 12, 1998. Your Conditional Use Permit, #004-98 for a Commercial Outdoor Recreation
Miniature Golf Facility, was approved with the following conditions:
All review agency comments must be addressed and complied with at all times.
2. A site plan shall be approved by the County.
3. A full screen shall be provided adjacent to the adjoining residential properties (with the exception
of the Fowler's property line).
4. Any on -premise business sign along North Frederick Pike shall be limited to a maximum of fifty
(50) square feet in area. Any on -premise business sign along Gainesboro Road shall be limited
to a maximum of twenty (20) square feet in area and ten (10) feet in height.
5. Commercial entrances shall be located along Gainesboro Road.
6. Any expansion of use or change of use would require a new CUP.
Enclosed is your Pump and Haul application which has been signed by the County Administrator, plus
a copy for your records. If you have any questions regarding the site plan requirements or any other
aspects of your Conditional Use Permit, please feel free to call this office.
