application. All required information must.be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).

Enclosed is the Certificate to Operate (CTO) for the sewage treatment works constructed at
God's Glory Land.
A Statement of Completion was received from Austin Brockenbrough & Associates, LLP, on
August 9, 2005. The project was inspected by the writer on August 11, 2005, at which time two minor
issues — an inoperative alarm hom and a broken pipe connection — were identified. On August 16, 2005,
the writer received an e-mail from Mathew Keefer, Operations Manager, stating that both minor issues
had been corrected.

Description of Facility/System: sue: This project consisted of the installation of a 10,000 gpd extended
aeration activated sludge package plant including flow equalization,
alkalinity supplementation, dual aeration basins, clarifier, tablet
chlorination, chlorine contact, tablet dechlorination, post aeration,
flow measurement, and aerated sludge digestion. Plans and
specifications for this project were conditionally approved by the
Department of Environmental Quality on January 24, 2000.
Authorization to Operate: The owner is authorized to operate these facilities in accordance with
VPDES Permit No. VA0088871 and the approved Operations and
Maintenance Manual.

The Frederick County Planning Department administratively approved the above referenced site plan,
#061-98, on June 7, 1999. This revised site plan meets all County requirements for development within
the RA (Rural Areas) Zoning District and is in conformance with the approved Conditional Use Permit
for this facility.
I have provided you with several copies of the approved site plan. Please forward two copies to the
owner and advise them to keep an approved copy for their files and maintain an approved copy on the
construction site. Once the site development has been completed, the owner should contact this office to
schedule an on -site inspection.

I have had the opportunity to review the above referenced site plan resubmitted to our department. Thank you
for addressing the majority of my comments. Prior to resubmitting five copies of the site plan and all approved
review agency comment sheets for final approval, please provide the sidewalk connection from the lodge to the
chapel area. In light of the fact that access to the pool is through the chapel, a sidewalk from the lodge directly
to the pool would be unnecessary. However, a sidewalk, similar to the one depicted on the attached copy of the
plan, would be required.

A review of the above referenced site plans has been completed by this office. It is the
understanding of this office that construction of the facilities as shown on the site plan will
involve different phases over a period of several years and that construction of the facilities has
begun. Please be advised that these facilities must comply with the following Health
Department regulations and that this office must review and approve all plans prior to
construction: Transient Lodging and Hotel Sanitation in Virginia 1965, Virginia State
Department of Health; Rules and Regulations Governing Campgrounds, 1973; Rules and
Regulations of the Board of Health Commonwealth of Virginia, Governing Restaurants,
November, 1984; and Regulations of the Board of Health Commonwealth of Virginia,
Governing Tourist Establishment Swimming Pools And Other Public Pools 1962. Plan review
and approval for the proposed sewage disposal system and the public non -community water
supply system is being handled by the Virginia Department of' Health's Office of Water

In regards to plan review and approval, this office has previously reviewed a set of plans
for the lodging, food service area, and the campground areas of the proposed facilities. The
Health Department is awaiting for plan revisions prior to final approval. The site plan and
blueprints as submitted for review do not contain the necessary information in order for this
office to perform an adequate review of the campground areas of the proposed facilities. Based
on the plans as submitted, the following items need to be provided and/or corrected:
1. A sanitary dump station for the disposal of sewage and other liquid wastes from self-
contained camping units shall be provided and is to comply with 12 VAC 5-450-90 E
& F of the campground regulations.
2. An adequate, safe, and sanitary supply of potable water must be provided that is

capable of supplying at least 100 gallons per campsite per day (35 campsites X 100
gallons per site = 3500 gallons per day minimum). Provide this office with plans and
specifications for all proposed water storage reservoirs.
Will individual water and sewer connections be provided at those campsites labeled
"Camper Access" on the site plan? If so, the water and sewer connections must be
separated by a minimum horizontal distance of five feet and shall be located and
installed so that they will not be damaged by the parking of camping vehicles.
Provide this office with a set of plans and specifications showing each individual
water and sewer connection along with grade elevations. Each water supply
connection must be protected against the hazards of backflow and back siphonage.
Refer to sections 12 VAC 5-450-80 "Water Supplies" and 12 VAC 5-450-90 "Sewage
Disposal" for specifications.
4. The "Retreat Cottages" are shown as being supplied with a water supply line;
however, no sewage disposal method is shown on the plans. Please provide this
office with a set of plans and/or written documentation as to the type of sewage
disposal proposed for the cottages.
Provide the Health Department with a set of plans, specifications, and an interior
finish schedule for each proposed service building (bath house) along with the
proposed number of toilets, urinals, lavatories, and showers that will be provided. A
service building must be located within 500 feet of each campsite , but can be no
closer than 30 feet to a campsite. Refer to sections 12 VAC 5-450-100 "Service
Building" and 12 VAC 5-450-110 "Structural Requirements for Service Buildings"
for specifications.
6. Each campsite (including the parking space) must provide a minimum of 1600 square
feet of space and shall not be less than 25 feet at its narrowest point. The campsites
as shown in the "Camper Access" area of the site plans do not comply with the 25
foot minimum size requirement.

The plans also indicate that a swimming pool will be constructed at this facility. This
office must review and approve of the plans for the swimming pool and filter room prior to
construction. Please provide the necessary plans and specifications when they are available.

Mr. Beeman:
A review of the above referenced site plans has been completed by this office. It is the
understanding of this office that construction of the facilities as shown on the site plan will
involve different phases over a period of several years and that construction of the facilities has
begun. Please be advised that these facilities must comply with the following Health
Department regulations and that this office must review and approve all plans prior to
construction: Transient Lodging and Hotel Sanitation in Virginia 1965 Virginia State
Department of Health; Rules and Regulations Governing Campgrounds 1973, Rules and
Regulations of the Board of Health. Commonwealth of Virginia Governing Restaurants
November. 1984; and Regulations of the Board of Health Commonwealth of Virginia,
Governing Tourist Establishment Swimming Pools And Other Public Pools 1962. Plan review
and approval for the proposed sewage disposal system and the public non -community water
supply system is being handled by the Virginia Department of Health's Office of Water
Programs, Lexington Field Office. Please contact Tom Eberly or Chuck Connors at (540)463-
7136 for comments in reference to this project.
Based on a review of the above referenced plans as submitted, the following items need
to be provided and/or corrected:
The site plan shows 13 parking spots for RV's. A sanitary dump station meeting the
requirements of the Campground Regulations must be provided unless all RV's
and/or self-contained camping units are unoccupied while parked on -site at the
establishment.
2. The "Retreat Cottages" are shown as being supplied with a water supply line;
however. no sewage disposal method is shown on the plans. Please provide this
office with a set of plans and/or written documentation as to the type of sewage
disposal proposed for the cottages or remove the water supply line from each cottage.

I have had the opportunity to review the above referenced site plan resubmitted to our department. Thank you
for addressing the majority of my comments. Prior to resubmitting five copies of the site plan and all approved
review agency comment sheets for final approval, please provide the sidewalk connection from the lodge to the
chapel area. In light of the fact that access to the pool is through the chapel, a sidewalk from the lodge directly
to the pool would be unnecessary. However, a sidewalk, similar to the one depicted on the attached copy of the
plan, would be required.

I have had the opportunity to review the above referenced site plan. The following comirients should be
addressed during the revision of this site plan.
1) Please provide a parking summary for the revised site plan. This should be located on sheet 2A of 3 and
reflect the changes from the previous plan to the revised plan. In addition, please show the handicap
parking spaces, handicap access to and from the various uses, and provide the dimensions for both the
handicap and standard parking spaces.
2) Please provide sidewalks connecting all of the parking spaces, the chapel, the pool, and lodge number
two. The sidewalk surrounding lodge number two appears to have disappeared. Please ensure that they
are included on the plan.
3) Please revise the area surrounding the RV parking area. Currently, the width of the driving isle is
insufficient to accommodate larger vehicles. In particular, as they attempt to access the parking spaces.
4) Please include landscape plantings consistent with the previous site plan which was reviewed and
approved by the Planning Commission.
Please revise your site plan to reflect the comments specified in this letter and resubmit one copy for my review.
Once I have verified the information contained on the revised site plan, I will need all approved review agency
comment sheets and five copies of the final site plan.

Regulations of the Board of Health Commonwealth of Virzinia Governing Restaurants,
November. 1984; and Regulations of the Board of Health Commonwealth of Vir.inia,
Governing Tourist Establishment Swimming Pools And Other Public Pools, 1962. Plan review
and approval for the proposed sewage disposal system and the public non -community water
supply system is being handled by the Virginia Department of Health's Office of Water
Programs, Lexington Field Office. Please contact Tom Eberly or Chuck Connors at (540)463-
7136 for comments in reference to this project.
In regards to plan review and approval, this office has previously reviewed a set of plans
for the lodging, food service area, and the campground areas of the proposed facilities. The
Health Department is awaiting for plan revisions prior to final approval. The site plan and
blueprints as submitted for review do not contain the necessary information in order for this
office to perform an adequate review of the campground areas of the proposed facilities. Based
on the plans as submitted, the following items need to be provided and/or corrected:
A sanitary dump station for the disposal of sewage and other liquid wastes from self-
contained camping units shall be provided and is to comply with 12 VAC 5-450-90 E
& F of the campground regulations.
2. An adequate, safe, and sanitary supply of potable water must be provided that is

capable of supplying at least 100 gallons per campsite per day (35 campsites X 100
gallons per site = 3500 gallons per day minimum). Provide this office with plans and
specifications for all proposed water storage reservoirs.
Will individual water and sewer connections be provided at those campsites labeled
"Camper Access" on the site plan? If so, the water and sewer connections must be
separated by a minimum horizontal distance of five feet and shall be located and
installed so that they will not be damaged by the parking of camping vehicles.
Provide this office with a set of plans and specifications showing each individual
water and sewer connection along with grade elevations. Each water supply
connection must be protected against the hazards of backflow and back siphonage.
Refer to sections 12 VAC 5-450-80 "Water Supplies" and 12 VAC 5-450-90 "Sewage
Disposal" for specifications.
4. The "Retreat Cottages" are shown as being supplied with a water supply line;
however, no sewage disposal method is shown on the plans. Please provide this
office with a set of plans and/or written documentation as to the type of sewage
disposal proposed for the cottages.
Provide the Health Department with a set of plans, specifications, and an interior
finish schedule for each proposed service building (bath house) along with the
proposed number of toilets, urinals, lavatories, and showers that will be provided. A
service building must be located within 500 feet of each campsite , but can be no
closer than 30 feet to a campsite. Refer to sections 12 VAC 5-450-100 "Service
Building" and 12 VAC 5-450-110 "Structural Requirements for Service Buildings"
for specifications.
6. Each campsite (including the parking space) must provide a minimum of 1600 square
feet of space and shall not be less than 25 feet at its narrowest point. The campsites
as shown in the "Camper Access" area of the site plans do not comply with the 25
foot minimum size requirement.

The plans also indicate that a swimming pool will be constricted at this facility. This
office must review and approve of the plans for the swimming pool and filter room prior to
constriction. Please provide the necessary plans and specifications when they are available.
