application. All required information must be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).

I met with Greg Strosnider this morning to review the latest VDOT design
and walk the site to demonstrate how it will/will not impact the
existing operation. There are two issues that we discussed that have
not been brought up before (to my knowledge) that I think you and I
should sit down and figure out. One regards the food service access
area and the second involves the relocation of a fire hydrant.
I will be out of the office tomorrow through September 13 on a much
needed break (first one since Christmas!); however, I would like to get
together with you the week of 9/13-917 if you will be in the area.
Please let me know what works for you.

I have reviewed your proposed relocated parking lot design with our Zoning Administrator. Please find attached a
mark-up of this plan which has been modified slightly, although it is not to scale. The following modifications are
suggested:
1) Utilize 90 degree angle parking for all new spaces. Although one additional space is lost, this design is preferred,
as the 60-75 degree angle parking creates difficulty for vehicles that will back out into the travel aisle.
2) Provide raised concrete islands as indicated to ensure that vehicle maneuvering patterns are delineated and do not
create a conflict with parking areas.
3) Ensure that the dumpster pad area is of a sufficient width and is designed to allow for truck maneuvering should
parking spaces be occupied. Also ensure that the dumpster pad area is concrete instead of asphalt to prevent pavement
failure.
4) Ensure that all parking lot landscaping that is impacted will be replaced. This includes parking lot trees which can
be located around the perimeter or within raised islands and shrubs.
I will need to review a detailed design for this area which addresses the above issues before I can advise Greg

On Wednesday, October 1, 1997, I performed an erosion and sediment control inspection
at the subject site. At the time of the inspection, your crew was erecting the new building and
performing other site work. I talked with your foreman and informed him that a lot of the silt
fence had been destroyed and that additional silt fence would be needed to be placed adjacent to
Valley Mill Road. Site grading work had been performed along the road and there is no erosion
control measures. All of the silt fence needs to be maintained on a daily basis. Also, continue to
maintain the construction entrance with fresh stone to keep mud from being tracked onto Valley
Mi11 Road.
I will perform an inspection within two (2) weeks from the date of this inspection to
ensure compliance with the approved erosion and sediment control plan. If you have any
questions, do not hesitate to contact me.

On Tuesday, August 19, 1997, I performed an erosion and sediment control inspection at
the subject site. At the time of the inspection, your crew was performing work on the inside of the
existing house. I talked with your foreman and informed him to continue to maintain the silt fence
that surrounds the perimeter of the site. Several sections of silt fence have fallen into disrepair.
Also, I informed him to enlarge the construction entrance and to maintain fresh stone on the
entrance to keep mud from being tracked onto Valley Mill Road.
I informed the foreman that when work begins along Valley Mill Road and the new,
roadside ditch is excavated, careful installation of erosion and sediment control will be required
and that daily maintenance will be needed to contain any silt from draining to Abrams Creek.
I will perform an inspection within two (2) weeks from the date of this inspection to
ensure compliance with the approved erosion and sediment control plan. If you have any

Dear Ms. D'Arcangelis:
In response to your letter dated July 19, 1997, our staff evaluated the potential impact
of the development of the Rose Foundation -Community Center for Living building as well as
Hill Valley Subdivision on your property in Carlisle Estates. The following discussion
addresses each one of these concerns as a separate issue.
The Rose Foundation site plan was designed to ensure that the proposed facility
complied with all applicable county ordinances and that all stormwater and erosion and
sediment control concerns where addressed. A majority of the stormwater drainage from the
Rose Foundation Facility will be directed to the drainage ravine to the north of the site. As
part of this site development, a new drainage ditch will also be constructed along Valley Mill
Road to intercept and contain the stormwater on the north side of the road. This runoff will be
diverted to the bottom of the hill and into Abrams Creek. The stormwater drainage should not
impact Carlisle Estates in any way. Also, erosion and sediment control devices have been
installed as part of the project to prevent all silt from leaving the site. Bi-weekly inspections
are performed by Frederick County to ensure that these devices are maintained.
You expressed concern about the development of Hill Valley Subdivision which is to
the north of Rose Foundation. This subdivision was reviewed and approved by Frederick
County. We have been monitoring its development since the commencement of construction.
All of the stormwater from this subdivision drains directly to a tributary to Abrams Creek and
does not impact Carlisle Estates. The contractor has been maintaining the erosion and
sediment control devices. Frederick County as well as other local agencies continue to inspect
the site to ensure compliance.

The stormwater drainage structures associated with each project are designed to
accommodate 10-year design storms only. Hurricane Fran and the Flood of 1996 were both
approximate 100-year design storms. In the case of these events, ditches and streams will
overtop their banks and could cause some flooding. Frederick County nor any other agency
can impose more stringent design parameters than what is outlined in the codes and ordinances
of Frederick County and the State of Virginia.

As you know we have had a severe stormwater drainage
problem off of Valley Mill Road into our yard and our
development for years. After eight years there has been
no action to alleviate the problem. Now that
construction has begun on the Center for Community Living
located at the top of the hill on valley Mill Road, we
sincerely hope that provisions for stormwater run-off are
being made to protect the citizens who live below this
hill from damage to their private property. As you know,
the removal of grass, trees, bushes, and other vegetation
in order to build the parking lot, driveways, etc. will
cause more stormwater run-off. Mr. Harrington Smith has
seen how severe the problem was eight years ago, and it
remains the same. As you can imagine, we are extremely
worried about the effects of this construction.
In addition, what is being done about run-off from the
new development at the intersection of Greenwood Road and
valley Mill Road? With the removal of all that
vegetation, what will stop the flow of the water and soil
in the event of a heavy rain or an occurrence such as
Fran or the snowmelt and rain of January of 1996? We
sincerely hope that Frederick County is making plans for
the protection of its citizens who live in this area. We
are REALLY worried.
Please contact us to let us know what is happening on
this matter of serious concern.

The Frederick County Planning Department administratively approved revisions to Site Plan #004-
97 for the referenced project on December 2, 1997. The original site plan was revised to relocate
the dumpster, provide for a new concrete pad, enclose the dumpster with a six-foot high wooden
fence that has a latched gate, and provide landscaping around the fenced area as demonstrated.
The revised plan also calls for the provision of a slab on grade for trash storage on the south side
of the building that will be screened by a three-foot wooden fence. All other improvements
specified in my letter to your firm, dated July 28, 1997, still apply to this project and must be
complete prior to the issuance of an occupancy permit for this project.
Please find enclosed one copy of approved revisions to sheet C3 and C4 from the original site
plan. This copy is to be provided to Mr. Max Clatterbuck of Lantz Construction Company.
Please provide our department with additional plans for signature and distribution as needed.
Please contact this office to schedule an on -site inspection when all improvements have been
completed.

The Frederick County Planning Department administratively approved Site Plan #004-97 for the
referenced project on July 25, 1997. The approved site plan calls for the development of an adult
care facility that includes the renovation of the existing 4,077 square feet structure and the
construction of a 7,928 square feet addition that will be 25 feet in height, the provision of a paved
parking lot that will include curb and gutter, seven regular parking spaces, one disabled parking
space with an appropriate ramp and signage, and perimeter parking lot landscaping to include six
willow oaks and seven sugar maples with a minimum caliper of two inches at time of planting,
the planting of 82 yews as indicated on the site plan, the provision of an access drive that will
contain a concrete dumpster pad which will be completely enclosed by a board -on -board wooden
fence with a latched gate, the relocation of an existing fire hydrant and the installation of a new
fire hydrant, the relocation of overhead utility poles and lines, the construction of a commercial
entrance onto Route 659 that will meet all requirements of the Virginia Department of
Transportation, and all appropriate erosion and sedimentation control measures.
As requested, I have provided Mr. Tim Painter with eight copies of the approved site plan for
distribution. Please ensure that an approved copy is maintained on the project site. Please contact

Please provide VDOT with a copy of plans signed on Page 1 by Rose Foundation. Once
we have the signed set you may apply for the appropriate VDOT land use permit to
perform work within the VDOT right-of-way. The permit is issued by this office and
will require a minimum processing fee, surety bond coverage, and the salary &
expenses of a State assigned Inspector.
Once satisfactory application has been made, a permit will normally take approximately
thirty days to process and issue.

We have received your preliminary site plan dated July 3, 1997 for the referenced
project. A review has been completed and our comments are marked in red on the plan
sheets as follows:
Sheet C2/C5
1. The VDOT General Notes should be shown on Sheet 1 or 2. Also trip generation
traffic volumes from the I.T.E. Trip Generation Manual, 501 Edition should be shown.
Sheet C3/C5
2. The driveway (Robert W. Koon) should be reconstructed to VDOT PE-1 Standard.
3. The two existing 15" C.M.P. should be removed at the Koon Entrance and the site
plan entrance.
4. The proposed 8" Yard Drain "E" should end at the Route 659 right-of-way. Riprap
should be placed from the end of pipe to riprap in road ditch.
5. Dimensions should be shown on the pavement tapers on each side of proposed site
entrance, the width and length of taper, also a typical section of pavement design
should include 8 21-B aggregate with a sealer between the aggregate and surface
course on the VDOT right-of-way. Included in this area is a right-of-way dedication
which includes a 25' radial line connecting the right-of-way on the north side of the
existing Route 659 to the proposed southern right-of-way line of relocation of
Route 659, as shown checkered on the plan sheet. Please identify all other rights -of -
way and easements on the plan sheets.

6. Can the section of proposed watermain which falls within the proposed relocation of
the Route 659 right-of-way on the north turnaround be shifted to private property to
prevent relocation at future VDOT project expense?
7. A typical section should be provided to show the new shoulder dimension in the
regraded area along existing Route 659.
8. The depth of the 21-B aggregate should be 8" within VDOT right-of-way.
All miscellaneous comments and/or notes should be addressed and/or added to the
plan notes as appropriate. Any changes in design items should meet or exceed the
standard.
When all the above comments have been addressed, please submit five sets of revised
plans for final review and approval.

After our meeting two weeks ago concerning the proposed alignment of Valley Mill
Road, our designers modified the design in accordance with that meeting.
I am attaching plans which indicate the new proposed alignment (Alternative Q. The
Department estimates an additional $50,000.00 above our original alignment (Alternative A).
It appears we will be taking approximately 0.60 +/- acres. Our Right -of -Way section will
be making an approximate appraisal of this and will furnishing this information to me within the
next week. I will forward this along to you once it is received.

We have received your preliminary site plan dated October 15, 1996 (stamped February 7, 1997) for the
referenced project.
A preliminary plan is being developed by VDOT Designers in our Staunton District Office which is in
the immediate vicinity of your proposed site plan. That plan has a basic goal to improve the intersection
condition at Route 659 and 656. The project number is 0656-034-1 18, C501.
The meeting held January 17, 1997 between Frederick County Planners and VDOT Designers which you
attended brought forth some alternative ideas concerning revising alignment at the Rose site. The
alignment adjustments yielded cost estimates in the vicinity of $200,000+ and introduced additional
impacts on adjacent property owners. The County is considering these costs and impacts on funding
ability.
Considering these facts, we have reviewed your site plans and have several action plans with proposals to
consider for a temporary entrance to serve the site pending resolution of the roadway studies. These
action plans are shown on the attachments to this letter.
We will be happy to review any plan you may develop with the understanding these plans would be

Based on the existing and proposed contours associated with the proposed "future"
easternmost entrance, it appears that additional grading will be required to provide
a smooth transition with the future Route 659 realignment. In all probability, a
portion of this entrance including the drop inlet will be redesigned when the
Virginia Department of Transportation completes the new road alignment.
It will not be necessary to resubmit these drawings for our review. However, we would
appreciate any revised drawings for our records.

The site plan submittal is not recommended for approval subject to the following
items, as discussed at the TRC Meeting held on February 13, 1997.
1. The existing hydrant is to be re -located onto the island on the South side of the
entrance.
2. The siamese connection must be located within 100' of the nearest hydrant.
This measurement is to be taken in the centerline of the street.
3. The section of 6" water main supplying the site must be increased
to 8", in order to ensure adequate fire flow rates. This involves the replacement of
approximately 600' of pipe along Valley Mill Road.
4. The section of Fire Lane to the Southwest side of the building must be widened
to 20'. It is currently only 12' in width.
