The checklist below indicates all the information -that needs to be -submitted as part of the site plan
application. All required information must.be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).
Site Plan Package
1. One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

Application #65152 for the entrance improvements and road widening for the
referenced project, we discovered the developer has not provided us with necessary
right-of-way and casement documents. The needed information is outlined in our
project approval letter dated October 1, 1996, a copy of which I have enclosed for your
use.
In addition four sets of the site plans will also need to be submitted for permit issuance.
I regret any inconvenience this may cause, however, the right-of-way dedications and
easements must be in place in order to complete the permitted right-of-way
improvements.

This is to acknowledge receipt of your revised plans dated September 20, 1996 for the referenced
project. The plans appear satisfactory and are approved. Please advise the developer
accordingly.
I offer the following comments:
• A preconstruction conference be held by you and/or developer with the attendance of the
contractor, various County agencies and VDOT shall be conducted prior to initiation of work.
• Materials used and method of construction shall apply to current observed VDOT Road &
Bridge Specifications applicable during construction of this project.
• Our review and comments are general in nature. Should conditions in the field exist such that
additional measures are warranted, such measures shall be completed to the satisfaction of the
Department.
• All drainage is to be carried within the right-of-way in ditch lines or gutters along the street to
a pipe or drainage easement.
• The contractor shall notify VDOT when work is to begin or cease for any undetermined
length of time. VDOT will also require 48 hours notice for inspections.
• The appropriate land use permits shall be obtained before any work is performed on the
State's right-of-way. The permits will require a minimal processing fee, surety bond
coverage, and the salary & expenses of a State assigned Inspector.

Please provide the developer with a copy of this letter to ensure lie is aware of our requirements.
Additionally, the necessary right-of-way dedications along Routes 658 and 659, drainage/grading
easement on the School Board property, and certified copies of permission letters and/or
easements for grading on the adjacent Hillsdale Estates and Rudolph Properties will need to be
provided by the owner prior to our issuance of any permits to cover work on the State's
right-of-way or access to the property. I would suggest the dedication and easement documents
be submitted to this office for review prior to recordation.

Dear Mr. Smith:
This letter is to inform you that the Frederick County Planning Department administratively approved site
plan 4002-97 on April 3, 1997.
This site plan consists of a 16,000-square-foot office building to be built northeast of the existing 6,400-
square-foot office building. There will be 65 parking spaces built adjacent to the proposed office building,
curb and gutter to be installed along the perimeter of the bituminous concrete parking lot. Access to this
site will be gained solely by way of the proposed entrance, located at the intersection of Dicks Hollow and
Indian Hollow Roads. The existing entrance onto Indian Hollow Road will be removed.
Attached are two copies of the approved plan. This will enable you to provide your contractor with an
approved copy, and maintain a copy for your records. Please have the contractor contact this office to
schedule an on -site inspection once all improvements are complete.

appears all of our earlier review comments have been satisfactorily addressed. Please
advise the owner of our approval and our appreciation for the elimination of the
deration for the safety of the traveling public is the rational

way. The permit is issued by this office and will require a minimal processing fee and

As requested, we have reviewed your site plan dated 1 1-27-96 for the referenced project. Our
comments may be found on the attached set of plans marked in red and as follows:
1) The elimination of the existing entrance onto Route 679 shall also include the removal of the
existing entrance pipe. The 12' offset for the proposed entrance will require the shoulder and
ditch line be regraded to conform to the existing typical shoulder and ditch line; all of which
should relocate the proposed entrance pipe to a more desirable distance from the pavement
edge.
2) Pipe end to be mitered in lieu of standard end sections.
3) Tapers to be 36' in length.
4) Entrance widths to be a minimum of 14' from gutter pan faces.
5) The curb and/or gutter is to be CG-3 or CG-7.
6) The existing 12" R.C.P. should be ungraded to a 18" R.C.P.
7) The existing downstream 29"x 17" C.M.P. under Route 679 should have inlet protective
erosion control measures.
8) Directional flow arrows are to be shown for all culverts.
Please revise and resubmit six (6) copies of the plan for final approval. Should any changes be
deemed necessary, please design them to meet or exceed the above recommendations.

Gentlemen:
Recently we observed the results of work within the Route 679 right-of-way. This
grading and fill placement over a make shift culvert connected to an existing concrete
culvert crossing under Route 679 started without first obtaining a land use permit.
VDOT is very concerned with the quality of materials and methods of construction used
to install this pipe. Therefore, we must insist this metal pipe be removed and all soil
placed on the right-of-way be regraded to its original condition.
If a private entrance is desired at this location please contact Mr. Greg Hoffman, our
Maintenance Superintendent at the Cross Junction Area Headquarters, at
540-888-3524. If a commercial entrance is planned you will need to submit engineered
site plans to this office for approval prior to issuance of a land use permit.

The construction entrance detail indicates that the washrack is optional.
Due to the potential of mud being tracked on Route 679, the washrack will
be required.

Add silt fence screening as indicated on the plan because of the up -slope
grading work to be performed.

Surface roughening shall be included in all slope grading to comply with
Section 3.29 of the Virginia Erosion and Sediment Control Handbook.

Install a sediment trap in the northwest corner of the site. The sediment

Indicate the size of all check dams to be placed on site. During on -site
erosion and sediment (e & s) control inspections, it has become apparent
that the e & s control measures need to be clearly delineated on the plans
for the benefit of the contractor performing the work. Provide an e & s
control measure detail in accordance with Section 3.20 of the Virginia
Erosion and Sediment Control Handbook.
6) Sheet 4 of 6:
Indicate the location and provide a cross-section detail of the emergency
spillway.
7) Sheet 4 of 6:
A rip -rap channel is needed at the outlet of the 18 inch RCP storm pad
outlet to the existing 29 inch by 17 inch CMP culvert under Route 679.
8) Land Disturbance Permit:
Place a note on the plan indicating that a land disturbance permit will be
required by the Frederick County Public Works Department prior to the
commencement of any site grading work.

I have discussed the owners' request for a waiver to allow for more than 25 percent of the steep
slopes to be disturbed. The Zoning Administrator feels that the request is valid, and will concur
to disturbances of 61 percent of the steep slope, as requested.
Please revise your site plan to address the above comments, and the comments provided to you
by the review agencies.
You will need to provide this office with the approved comment sheets from all required review
agencies. The required review agencies include: Virginia Department of Transportation,
Inspections Department, County Engineer, Fire Marshal, County Health Department, and the
Planning Department. Once all comments have been addressed, please resubmit one copy of the
revised site plan to this department for final review. If the revised site plan satisfies all agency
comments, we will request five copies of the final site plan and recommend the site plan for
approval.
The fee for this site plan review will be based on the proposed 6 acres of disturbance. Hence,
the fee will be $1,530.00 ($1,000.00 base fee +$100.00 per acre for the first five acres disturbed
+ $30.00 per acre for the sixth acre disturbed).

for Valley Proteins. I anticipate the rezoning of the 1.62 acres to be approved on December I I at
the Board meeting. I have sent requests for comments to VDOT, Engineering, Inspections and
the Fire Marshal. Our initial proposal for sanitary sewerage has been favorably received by DEQ
and we are proceeding with final design.
