ALL WORK ON THIS PROJECT SHALL CONFORM TO THE LATEST EDITIONS OF THE VIRGINIA DEPARTMENT OF TRANSPORTATION (VDOT) ROAD AND BRIDGE SPECIFICATIONS AND STANDARDS, THE VIRGINIA EROSION AND SEDIMENT CONTROL REGULATIONS AND ANY OTHER STATE, FEDERAL OR LOCAL REGULATIONS APPLICABLE. IN THE EVENT OF CONFLICT BETWEEN ANY OF THESE STANDARDS, SPECIFICATIONS OR PLANS, THE MOST STRINGENT SHALL GOVERN. 2. ALL CONSTRUCTION SHALL COMPLY WITH THE LATEST U.S. DEPARTMENT OF LABOR, OCCUPf'kTIONAL SAFETY AND HEALTH ADMINISTRATION AND VOSH RULES AND REGULATIONS. 3. THE CONTRACTOR SHALL BE RESPONSIBLE FOR RELOCATING. AT HIS EXPENSE ANY AND ALL UTILITIES. INCLUDING TRAFFIC SIGNAL POLES, JUNCTION BOXES, CONTROLLERS, ETC. OWNED BY VDOT OI PRIVATE UTILITY COMPANY. IT IS THE SOLE RESPONSIBILITY OF THE CONTRACTOR TO LOCATE AND IDENTIFY UTILITY FACILITIES OR ITEMS WHICH MAY BE IN CONFLICT WITH THE PROPOSED CONSTRUCTION ACTIVITY. 4. THE CONTRACTOR WILL BE RESPONSIBLE FOR MAINTAINING ADEQUATE ACCESS TO THE PROJECT FROM THE ADJACENT PUBLIC ROADWAY THROUGH CONSTRUCTION AND MAINTENANCE OF A CONSTRUCTION ENTRANCE IN ACCORDANCE WITH THE VIRGINIA EROSION AND SEDIMENT CONTROL HANDBOOK, SECTION 3.02. FURTHERMORE, ACCESS TO OTHER PROPERTIES AFFECTED BY THIS PROJECT SHALL BE MAINTAINED THROUGH CONSTRUCTION. THE CONTRACTOR SHALL HAVE, WITHIN THE LIMITS OF THE PROJECT, AN EMPLOYEE CERTIFIE'i1 BY THE DEPARTMENT OF CONSERVATION AND RECREATION IN EROSION AND SEDIMENT CONTROL WHO SHALL INSPECT EROSION AND SILTATION CONTROL DEVICES AND MEASURES FOR PROPER INSTALLATION AND I)EFICIENCIES IMMEDIATELY AFTER EACH RAINFALL, AT LEAST DAILY DURING PROLONGED RAINFALL, AND WEEKLY WHEN NO RAINFALL EVENT OCCURS. 5. CONTRACTOR SHALL ENSURE ADEQUATE DRAINAGE IS ACHIEVED AND MAINTAINED ON THE SITE DURING AND AT THE END OF CONSTRUCTION. 6. ANY UNUSUAL SUBSURFACE CONDITIONS ENCOUNTERED DURING THE COURSE OF CONSTRUCTION SHALL BE IMMEDIATELY BROUGHT TO THE ATTENTION OF THE DEVELOPER AND ENGINEER. WORK SHALL CEASE IN THAT VICINITY UNTIL AN ADEQUATE DESIGN CAN BE DETERMINED BY THE ENGINEER. 7. ALL ROADWAY FILL, BASE, SUBSURFACE MATERIAL AND BACKFILL OR UTILITY SEWER TRENCHES SHALL BE COMPACTED IN 6" LIFTS TO 95 % OF THEORETICAL MAXIMUM DENSITY AS DETERMINED BY AASHTO T -99 METHOD A, WITHIN PLUS OR MINUS 2% OF OPTIMUM MOISTURE FOR THE FULL WIDTH OF ANY DEDICATED -TREET RIGHT -OF -WAY. AT THE DIRECTION OF INSPECTOR, DENSITY TEST PERFORMED BY A QUALIFIED INDEPENDENT AGENCY SHALL BE CONDUCTED AS REQUIRED IN THE VDOT ROAD AND BRIDGE SPECIFICATIONS. 8. AGGREGATE BASES AND SUB -BASES SHALL BE PLACED ON THE SUB -GRADE BY MEANS OF A C.tIECHANICAL SPREADER. DENSITY WILL BE DETERMINED USING THE DENSITY CONTROL STRIP AS SPECIFIED IN VDOT'S "ROAD AND BRIDGE SPECIFICATIONS 304) AND VTM-10. A CERTIFIED COMPACTION TECHNICIAN SHALL PERFORM THESE TESTS. 9. ASPHALT CONCRETE PAVEMENT SHALL BE PLACED IN ACCORDANCE WITH THE LATEST EDITION VDOT'S "ROAD AND BRIDGE SPECIFICATIONS" (SECTION 315). DENSITY WILL BE DETERMINED USING THE DENSITY CONTROL STRIP AS SPECIFIED IN SECTION 315 AND VTM- 76. A CERTIFIED COMPACTION TECHNICIAN SHALL PERT'ORM THESE TESTS. 10. TOPOGRAPHIC INFORMATION WAS DERIVED BASED UPON FIELD SURVEY PERFORMED BY URBAN ENGINEERING & ASSOCIATES, INC., AND AERIAL PHOTOGRAPHY. 11. THE ROAD DESIGN HAS BEEN BASED UPON AN AVERAGE DAILY TRAFFIC VOLUME OF 430 VPD (43 LOTS X 10 TPO). 12. DAMAGE TO ANY EXISTING ENTRANCES, PAVEMENTS OF, OR OTHER EXISTING STRUCTURES .NOT PROPOSED TO BE DISTURBED WITH THIS DEVELOPMENT, WILL BE THE RESPONSIBILITY OF THE CONTRACTOR AND MUST BE REPAIRED TO SATISFACTION OF THE INDIVIDUAL PROPERTY OWNER. 13. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ESTABLISHING A SMOOTH TRANSITION FRCJI EXISTING CENTERLINE TO THE PROPOSED EDGE OF PAVEMENT. T 14. THE CONTRACTOR SHALL FURNISH ALL MATERIALS, LABOR AND EQUIPMENT AND PERFORM ALL WORK INCLUDING RESTORATION FOR THE COMPLETED INSTALLATION OF THE UTILITY FACILITY READI' FOR USE. UNLESS OTHERWISE NOTED, SPECIFICATIONS FOR ALL THE WORK TO BE IN ACCORDANCE WITH APPLICABLE DEPARTMENT OF HEALTH, LAKE HOLIDAY ESTATES UTILITY CO., FREDERICK COUNTY AND THE CONTRACT DOCUMENTS. 15. THE ENGINEER HAS ATTEMPTED TO LOCATE EXISTING SUBSURFACE UTILITIES; HOWEVER, S`UCH MAY EXIST THAT ARE NOT SHOWN. THE CONTRACTOR SHALL EXERCISE CARE IN THIS WORK SO AS TO AVOID DAMAGE TO ANY UTILITIES. THE DAMAGE SHALL BE THE CONTRACTOR'S RESPONSIBILITY. The site is located in the Gainesboro District of Frederick County, Virginia. Approximately 2.90 acres will be disturbed for this project. EXISTING SITE CONDITIONS The proposed site consists of slopes ranging from 2 to 33%. The site consists of an existing community building including a gravel parking lot. OFFSITE AREAS No disturbance of any off -site areas will be required for this project. SOILS (See Sheet 2) EROSION AND SEDIMENT CONTROL MEASURES Unless otherwise indicated, all vegetative and structural erosion and sediment control practices shall' be constructed and maintained according to minimum standards and specification of the handbook. The minimum standards of the VESCR shall be adhered to unless otherwise waived or approved by a variance. No disturbed area shall be denuded for more than seven (7) days, except for that portion of the site in which work will be continuous beyond seven (7) days. All Temporary Diversions, Silt Trapping Structures, Silt Fence and other perimeter controls as indicated on the plans, shall be placed in conjunction with clearing and prior to rough grading. CONSTRUCTION SEQUENCING 1. Construction shall be sequenced so that grading operations can begin and end as quickly as possible. 2. Install permieter controls including silt fence and check dams. 3. Begin grading and removal of existing pavement where necessary. 4. Provide temporary seeding and mulching to all earthen perimeter controls immediately following grading. 5. Install inlet and outlet protection as storm structures are constructed. 6. Start re -vegetation as areas are brought to grade or stand idle for more than fourteen (14) days. 7. Stabilize the remainder of the disturbed area with permanent seeding. 8. When the site work is completed, remove all structural practices and apply permanent seeding to all disturbed areas. PERMANENT STABILIZATION All areas disturbed by construction shall be stabilized with permanent seeding immediately following finished grading. Seeding shall be done with Kentucky 31 Tall Fescue according to Std. & Spec. 3.32, Permanent Seeding, of the handbook. Permanent seeded areas shall be protected during establishment with straw mulch. MAINTENANCE All measures are to be inspected daily by the site superintendent. Any damaged controls shall be repaired and/or replaced by the close of the day. No controls are to be removed without the approval of the site inspector. The contractor shall be responsible for any damage or penalties caused by his failure to comply with the erosion and sediment control program or the direction of the site inspector. The design engineer shall be notified of any changes to this program. 1. The check dams will be checked regularly for sediment and cleaned when necessary. 2. The silt fence barrier will be checked regularly for undermining or deterioration of the fabric. Sediment shall be removed when the level of sediment deposition reaches half way to the top of the barrier. 3. The seeded area will be checked regularly to ensure that a good stand is maintained. Area should be fertilized and reseeded as needed. STRUCTURAL PRACTICES 1. Outlet Protection - 3.18 The installation of pave and/or rip -rap channel sections and/or stilling basins below storm drain outlets to reduce erosion from scouring at outlets and to reduce flow velocities before stormwater enters receiving channels below these outlets. 3. Riprap - 3.19 A permanent, erosion -resistant ground cover of large loose, angular stone with filter fabric or granular underlining. VEGETATIVE PRACTICES 1. The site is located in the Gainesboro District of Frederick County, Virginia. Approximately 2.90 acres will be disturbed for this project. EXISTING SITE CONDITIONS The proposed site consists of slopes ranging from 2 to 33%. The site consists of an existing community building including a gravel parking lot. OFFSITE AREAS No disturbance of any off -site areas will be required for this project. SOILS (See Sheet 2) EROSION AND SEDIMENT CONTROL MEASURES Unless otherwise indicated, all vegetative and structural erosion and sediment control practices shall be constructed and maintained according to minimum standards and specification of the handbook. The minimum standards of the VESCR shall be adhered to unless otherwise waived or approved by a variance. No disturbed area shall be denuded for more than seven (7) days, except for that portion of the site in which work will be continuous beyond seven (7) days. All Temporary Diversions, Silt Trapping Structures, Silt Fence and other perimeter controls as indicated on the plans, shall be placed in conjunction with clearing and prior to rough grading. CONSTRUCTION SEQUENCING 1. Construction shall be sequenced so that grading operations can begin and end as quickly as possible. 2. Install permieter controls including silt fence and check dams. 3. Begin grading and removal of existing pavement where necessary. 4. Provide temporary seeding and mulching to all earthen perimeter controls immediately following grading. 5. Install inlet and outlet protection as storm structures are constructed. 6. Start re -vegetation as areas are brought to grade or stand idle for more than fourteen (14) days. 7. Stabilize the remainder of the disturbed area with permanent seeding. 8. When the site work is completed, remove all structural practices and apply permanent seeding to all disturbed areas. PERMANENT STABILIZATION All areas disturbed by construction shall be stabilized with permanent seeding immediately following finished grading. Seeding shall be done with Kentucky 31 Tall Fescue according to Std. & Spec. 3.32, Permanent Seeding, of the handbook. Permanent seeded areas shall be protected during establishment with straw mulch. All measures are to be inspected daily by the site superintendent. Any damaged controls shall be repaired and/or replaced by the close of the day. No controls are to be removed without the approval of the site inspector. The contractor shall be responsible for any damage or penalties caused by his failure to comply with the erosion and sediment control program or the direction of the site inspector. The design engineer shall be notified of any changes to this program. 1. The check dams will be checked regularly for sediment and cleaned when necessary. 2. The silt fence barrier will be checked regularly for undermining or deterioration of the fabric. Sediment shall be removed when the level of sediment deposition reaches half way to the top of the barrier. 3. The seeded area will be checked regularly to ensure that a good stand is maintained. Area should be fertilized and reseeded as needed. STRUCTURAL PRACTICES 1. Outlet Protection - 3.18 The installation of pavedand/or rip -rap channel sections and/or stilling basins below storm drain outlets to reduce erosion from scouring at outlets and to reduce flow velocities before stormwater enters receiving channels below these outlets. 3. Riprap - 3.19 A permanent, erosion -resistant ground cover of large loose, angular stone with filter fabric or granular underlining. VEGETATIVE PRACTICES 1. Permanent Seedingq__3.3�2 Establishment of perennial cover by planting seed on rough grading areas that will not be brought to final grade for a year or more or where permanent, long lived vegetative cover is needed on fine grading areas. Seed mixtures are as follows: 200 Ibs Tall Fescue per ac. 12 lb. Annual Rye per ac. 1000 Ibs.10-20-20 Fertilizer per ac. 2 tons agricultural limestone per ac 2 tons straw mulch per ac.
