SITE PLAN APPLICATION CHECKLIST
The checklist shown below specifies the information which is required to be submitted as part of the site plan
application. The Department of Planning & Development will review the application to ensure that it is
complete prior to accepting it. If any portion of the application is not complete, it will be returned to the
applicant(s).
(1) One (1) set of approved comment sheets are required from each relevant review
agency prior to final approval of a site plan. It is recommended that applicants
contact the Department of Planning & Development to determine which review
agencies are relevant to their site plan application. A list of potentially relevant

Dear Mr. Lellock:
The Frederick County Department of Planning and Development administratively approved the above -
referenced site plan on September 1, 2004. Approval is granted for the paving of the parking lot by the
existing tennis court, along with improvements to the existing clubhouse parking lot. This site plan also
includes the addition of five (5) handicapped parking spaces and a concrete sidewalk leading to the
building and fourteen (14) parking lot trees. All requirements of the Frederick County Zoning
Ordinance have been adequately addressed in the site plan, including approvals from all relevant review
agencies.
I am providing you with three (3) copies of the approved site plan. Please forward these copies to the
appropriate representative(s). Fuithermore, advise the owner(s) that a copy should be kept for future
reference, and an approved copy must be kept on the construction site throughout the development
process. Once site development is complete, the owner(s) should contact this office to schedule an on -
site inspection. Do not hesitate to contact me should you have any questions or concerns.

group A (Assembly) of the International Building Code/2000.Other Code that applies is CABO
A117.1-98 Accessible and Usable Buildings and Facilities. Structural plans submitted for permit
application shall be sealed by a Virginia Licensed Design Professional.
Please note the requirements in Chapter 17 of BOCA for special inspection requirements on the
type of structure.( Soils, concrete ect..)
All required egress from the structure shall be HC Accessible to min. exterior landing.
HC parking shall not exceed 2% across the unloading area and min. 8' with 8' stripped Van
Accessible space required. Please confirm max. 5% slope on accessible route from parking to
building or submit a detailed accessible ramp plan at the time of permit application.

We have received your revised site plan dated June, 2004 for the referenced project. It appears
all of our earlier review comments have been satisfactorily addressed. Please advise the owner
of our approval. Also please provide six (6) sets of approved construction plans Nvith signed
seal for VDOT distribution.
The appropriate land use permit may now be applied for to cover work within the Route 701
right-of-way. The permit is issued by this office and will require a minimum processing fee,
surety bond coverage, and the salary & expenses of a State assigned Inspector. A copy of any/all
recorded plats of dedication for drainage easements or additional right-of-way required for
implementation of this proposed project should be provided to VDOT prior to issuance of any
land use permits. Dedication should be made to the Commonwealth of Virginia.
Once satisfactory application has been made, a permit will normally take approximately thirty
days to process and issue. Should you have any questions, do not hesitate to call.

Dear Mr. Lellock:
Planning Staff has reviewed the above -referenced site plan to determine if administrative approval can be
granted. At this time, administrative approval cannot be granted. This site plan is denied until the issues in
this letter, as well as all issues of the other review agencies, have been adequately addressed. Please review
Staff's comments listed below and then prepare a revised site plan which adequately addresses each concern.
Review Comments:
1. Handicapped Spaces. A sidewalk running from the handicap ramp needs to be provided; please
show this sidewalk on the plan. Please provide a detail for the handicap ramp.
2. Pavement to be Removed. The areas marked for new pavement have been removed from Sheet 3.
Please put these areas back on the plan.
3. Special Limited Power of Attorney (SLPA). A SLPA needs to be signed by the property owner,
notarized and submitted, see attachment.
After you have revised the site plan, please resubmit one copy so that I may verify the information contained
on the plan. I will need all approved review agency comment sheets and at least five copies of the final plan
for final administrative approval. Comment sheets are required from the following agencies: the Frederick
County Inspections Department, the Frederick County Department of Public Works (County Engineer), the
Frederick Countv Fire Marshal, and the Health Department.

Dear Ms. Mills,
In response to your comment letter dated May 7, 2004, regarding the above referenced project, we
offer the following:
1. Comment: ✓ Lighting. There seems to be no outdoor lighting associated
with this project; outdoor lighting is encouraged in the
parking areas and near the building for safety aspects. If any
lighting is proposed, it needs to be shown on the plan along
with a detail.
Response: Outdoor lighting shall be installed in the parking areas. Lighting
design is being coordinated by the building architect as all
outdoor lighting shall be incorporated into one electrical system.
C Uniment: ✓Signature Line. A signature line for the Zoning
Administrator with the statement "Approval of these plans
shall expire five (5) years from the date of planning
approval", needs to be on the cover sheet.
Response: Signature line added to the cover sheet.
3. Comment: Handicapped Spaces. All handicapped spaces need to have a
three foot wide curb ramp that is adjacent to the parking
space to provide access to the sidewalk. Sidewalks should
also be placed from the handicap ramps to the building.
Response: Two (2) VDOT standard CG-12 curb ramps added to the parking
area adjacent to the handicapped spaces. Sidewalks connect these
ramps to the building at a slope no greater than 5%.

4. Comment: ✓Adjoiners. All adjoining properties need to be shown and
labeled with owners, uses, zoning and Property Identification
Numbers (PIN). A map could be placed on Sheet 2 of 6 at a
larger scale showing the subject property and the adjoining
properties.
Response: Adjoining property owners added. See Sheet 2.
5. Comment: Access Roads. Label the access roads that go on the docks, as
indicated on the attached plan.
Response: Access roads labeled. See Sheet 3.
6. Comment: ,Stop Signs. A stop sign should be placed on the primary
access point for Lakeview Drive.
Response: A stop sign was added at the intersection of the parking entrance
and Lakeview Drive. See Sheet 3.
7. Comment: Wheel Stops. Concrete wheel stops need to be placed at the
end of each parking space.
Response: Wheel stops have been added to each parking space. See Sheet 3.
S. Comment: ✓ Pavement to be Removed. The areas that pavement is being
removed from need to be replaced with topsoil and seeded.
Response: Note added to legend that pavement to be removed is to be
replaced with topsoil and seeded.
10. Comment: Special Limited Power of Attorney (SLPA). A SLPA needs to
be signed be the property owner, notarized and submitted, see
attachment.
Response: As soon as I have the SLPA completed I will mail it separately.

Planning Staff has reviewed the above -referenced site plan to determine if administrative approval can be
granted. At this time, administrative approval cannot be granted. This site plan is denied until the issues in
this letter, as well as all issues of the other review agencies, have been adequately addressed. Please review
Staff s comments listed below and then prepare a revised site plan which adequately addresses each concern.

9. Landscaping. The plant list legend shows that only five trees are being planted on the site;
however, the plan shows 13 trees. Please revise the list to reflect what is drawn on the plan.

After you have revised the site plan, please resubmit one copy so that I may verify the information contained
on the plan. I will need all approved review agency comment sheets and at least five copies of the final plan

Frederick County Fire Marshal, and the Health Department.
