Enclosed you will find Commonwealth of Virginia, Department of Health, Waterworks Operation Permit No.
2069116 with an effective date of December 17, 2004. This permit is your authorization from the State
Health Commissioner to operate the public waterworks serving the Candy Hill Campground, located in Frederick
County, Virginia, in accordance with Part II of the Waterworks Regulations. You will note that the permit
indicates that this water supply has a design capacity of service to 100 campsites and 4 employees. This
permitted design capacity is the maximum system capacity based on Part III of the Regulations and shall not be
exceeded.
Please note that the permit has been issued with a variance allowing you to operate this waterworks without
metering of total water production as long as the total water production does not exceed 10,000 gallons per
day. This variance will be automatically revoked at such time the total water production exceeds 10,000
gallons per day. This variance is nontransferable and must be attached to the operation permit. This variance
will be revoked or suspended when the operation permit is revoked or suspended, or at the discretion of the
State Health Commissioner.

intersection of Ward Avenue and Route 50. This intersection is approximately '/z
mile west of the intersection of U.S. Route 50 and Route 37 on the western edge
of Winchester.

DESIGN BASIS: Service to 100 Campsites and 4 Employees
DESCRIPTION OF SYSTEM:
The water system consists of a single drilled well which discharges through a pitless adapter to a utility
room at the pool building where the water passes through a UV light disinfection system and into a 100-
gallon bladder -type pressure tank before flowing into the distribution system.
WELL: The well is located on a grassy knoll in the campground approximately 350 feet from the owner's
residence in line with the garage doors. The well is believed to be approximately 25 years old and is
reportedly drilled to a depth of 150 feet. No information is available concerning casing depth, grout depth,
or well yield. The well is equipped with a 3 hp submersible pump. The 6-inch casing extends approximately
12 inches above grade and is equipped with a water -tight cap and a pitless adapter.
STORAGE: Storage is provided by a 100-gallon bladder -type pressure tank located in the pool building's
utility room.
TREATMENT: Treatment is provided by an ultraviolet light disinfection system mounted on the wall across
from the pressure tank.
WELL HOUSE: A utility room, which is part of the pool control building, is utilized to store all the water
appurtenances. The facility is of frame construction and is well marked with a sign bearing the words
"Water Control Room - Keep Out". The dimensions are approximately 10 feet by 8 feet by 6 feet.
APPURTENANCES: The system is provided with a pressure gauge and sampling taps.
