SITE PLAN APPLICATION CHECKLIST
The checklist shown below specifies the information which is required to be submitted as part of the site plan
application. The Department of Planning & Development will review the application to ensure that it is
complete prior to accepting it. If any portion of the application is not complete, it will be returned to the
applicant(s).
(1) One (1) set of approved comment sheets are required from each relevant review
agency prior to final approval of a site plan. It is recommended that applicants
contact the Department of Planning & Development to determine which review
agencies' are relevant to their site plan application. A list of potentially relevant
review agencies is shown below:

Thank you for forwarding a copy of the Federal Aviation Administration's Aeronautical Study on the referenced
tower. As per our letter of June 24, 2004, the Winchester Regional Airport Authority offers the following final
comment on the above referenced tower:
It appears that the proposed tower does not penetrate any FAR Part 77 surfaces however when the
proposed tower was reviewed by the Airport in June of 2004 for a conditional use permit, the proposal
was for a monopole in height of 124'. The tower height has now changed to 130' according to the
papers filed with the Federal Aviation Administration adding an extra five feet. The Airport's preliminary
review was also prior to a medevac helicopter company establishing operations at the Winchester
Regional Airport.
2. Based upon this information, the Airport Authority is requesting that this tower be lit in accordance to
Federal Aviation Administration Advisory Circular 70/7460-1 K Change 1, Obstruction Marking and
lighting, Chapters 4 and 8 (M-Dual).
We are greatly concerned about the safety of medevac helicopter operations out of and into the Winchester
Regional Airport. Given the close proximity of the tower site to the airport, we feel lighting is necessary in order
to enhance continued safe helicopter operations.
If the above referenced conditions along with any outlined in the FAA study are met, the Winchester Regional
Airport Authority has no objections to these site plans.

Clough, Harbour & Associates LLP (CHA) is pleased to present revised drawings to
address the Planning Department's review of site plan 04-05-Cingular
Wireless/Shenandoah University. A copy of your review letter dated January 24, 2005 is
attached. The specific comments have been addressed as follows:
1. Location map: The location map has been drawn to a scale of 1 inch = 2000 feet
depicting all parcels of land involved in the project as well as those in the near
surrounding area on sheet TO I.
2. Magisterial District: The magisterial district has been added to sheet TO1 under
Site Information.
3. Site Plan Approval: A signature box has been provided on sheet TO stating what
was requested.
4. Contact Information: The representative for which Cingular wireless has been
communicating with at Shenandoah University has been added to sheet TO under
Site Information — Owner.
5. Proposed & Existing Uses: An additional page, sheet C20, has been added
showing all existing and proposed structures within a minimum of 100 feet of the
proposed compound location. The land area, use, description and height of each
structure are listed in tabular format on this page.
6. Conditional Use Permit: A copy of the conditional use permit has been included
on sheet C20 of the drawings.
7. Utility Providers: The name and contact information for the tower owner,
electrical and telephone company have been added to sheet TO

Staff is in receipt of the revised submission of the site plan for the above -referenced
project, accepted on January 5, 2004. At this time, the site plan is denied until the
following comments are adequately addressed.
Review Comments
1) Location Map- Individual parcels need to be placed on the location map as well
as a scale that represents 1'=2000ft.
2) Magisterial District- Please identify and label the proper magisterial district
where this site plan is located.
3) Site Plan Approval- A box should be placed on the title page that states the
following: Approved by the Frederick County Zoning Administrator — Site plan
approval valid for 5 years from date shown hence.
4) Contact Information- Please include the phone number and name for a contact
person who represents the owner at Shenandoah University.
5) Proposed & Existing Uses- A detailed description needs to be shown on the plan
for all existing as well as proposed uses on this site.
6) Conditional Use Permit- A reference to the approved conditional use permit
number and the corresponding list of conditions approved by the Board of
Supervisors should be listed on the site plan.
7) Utility Providers- Please list the names and contact information of all utility
providers associated Nvith this site plan.

8) Topographic Contours- Either the existing condition or site plan map should
accurately represent the topographic contours of this site.
9) Wi-itten Scale- Along with the graphic scale you have provided, a written
representation of the scale should accompany it.
10) Site Plan Map (Sheet S02) — Please indicate the height, use, and land area of all
existing structures located on site.
Once you have adequately addressed all of these comments, please resubmit a copy,
which includes the seal of a licensed engineer, for my re -review. I will need all approved
review agency comment sheets and at least five copies of the final plan for approval.
Comment sheets are required from the following agencies: the Frederick County
Department of Building Inspections, the Frederick County Fire Marshal, the Frederick
County Engineer, the Frederick County Health Depailinent, and the Virginia Department
of Transportation. These comment sheets need to be obtained and submitted by the
applicant. You may obtain a copy of the building inspector's comments by sending a self-
addressed, stamped envelope to their department. Peel free to contact me if you have any
