The checklist shown below specifies the information which is required to be submitted as part of the site plan
application. The Department of Planning & Development will review the application to ensure that it is
complete prior to accepting it If any portion of the application is not complete, it will be returned to the
applicant(s).
(1) One (1) set of approved comment sheets are required from each relevant review
agency prior to final approval of a site plan. It is recommended that applicants
contact the Department of Planning & Development to determine which review
agencies' are relevant to their site plan application. A list of potentially relevant

The Frederick County Department of Planning and Development acinlinistratively approved the
above -referenced site plan on August 24, 2006. The site plan is approved for T-Mobile to
collocate on an existing cellular tower on a site located off of Ralph Shockey Drive in the
Shawnee Magisterial District. This construction includes the following:
• Collocation of a T-Mobile antenna
• Equipment shcltcr
The site plan meets all requirements for development in the I-IE (Fligher Education) Zoning
District. All relevant review agencies have approved the project at this lime. I and providing you
with three (3) copies of the approved site plan. Please forward these copies to the appropriate
representat 1ve(s). Furthermore, advise the owners) that a copy should be kept for future
reference, and all approved copy must be kept on the construction site throughout the

an existing telecommunications lightpole located at 1122 Ralph Shockey Drive, Winchester, VA
22601. In response to your letter dated August 1, 2006, T-Mobile has obtained the comment
sheets from the Frederick County Building Inspections, Winchester Airport Awhority, and the
Department of Public Works. Please find the attached continent sheets in the package.

We have reviewed your request for site plan comments to place antennas at 95'
on an existing 125' street light located on University Drive.
Since the existing tower is the immediate controlling obstruction you will not be
required to file with the Federal Aviation Administration or the Virginia
Department of Aviation.
The following comment is offered on behalf of the Winchester Regional Airport
Authority:
1. During the installation of the antennas all cranes will be required
to have an aviation obstruction flag and/or obstruction light
visible during operations and must be lowered at night unless lit.
Intended hours and dates of operation need to be coordinated
with Airport staff no less than twenty-four hours prior to start.

Enclosed herewith please find a minor site plan application with corresponding drawings
for recommendation of a proposed facility for T-Mobile Northeast LLC ("T-Mobile") at
the following location:
1122 Ralph Shockey Drive
Winchester, VA 22601
T-Mobile is proposing to collocate nine (9) antennas onto an existing telecommunications
light pole located in a HE zone. T-Mobile's antennas are to be attached at the 95-foot
level on an existing 125-foot telecommunications light pole. A nine (9) foot by thirteen
(13) foot equipment shelter will be built to house the equipment cabinets and will be
placed near the telecommunications light pole. The equipment shelter will match the
existing shelters within the compound.

Enclosed herewith please find a minor site plan application with corresponding drawings
for recommendation of a proposed facility for T-Mobile Northeast LLC ("T-Mobile") at
the following location:
1122 Ralph Shockey Drive
Winchester, VA 22601
T-Mobile Is proposing to collocate nine (9) antennas onto an existing telecommunications
light pole located in a HE zone. T-Mobile's antennas are to be attached at the 95-foot
level on an existing 125-foot telecommunications light pole. A nine (9) foot by thirteen
(13) foot equipment shelter will be built to house the equipment cabinets and will be
placed near- the telecommunications light pole. The equipment shelter will match the
existing shelters within the compound.
