EROSION AND SEDIMENT CONTROL
NARRATIVE
Description: This project proposes construct a turn lane on Rte 522, relocate
approximately 2,500 feet of a private access way within a ingress/egress easement and
construct a 4400 square feet office building. The total area to be disturbed by this project
Is approximately 5.4 acres.
Date of construction: Gonstruction will begin following plan approval and will take -
approximately one (1) year to complete with the exception of the turn lane In Rte 522
which will take place in conjunction of the second phase of the expansion of OTAN's
facilitle5..
Existing site conditions: The area to be disturbed under this plan is currently Improved by
a gravel driveway, church, dwelling and a office building.
Adjacent areas: This project Is bounded on by Route 522 on the east and by RA zoned
property owned by Herbert Davis, John Warzln5kl, Otis Goodwin Bethany HIII Baptist
Ghurch, Murrell Renner and Outreach to Asia Nationals.
Soils information: The existing soil conditions were obtained from the USDA Soils
Survey. The predominant soil types for this site are 41G and 41E ( Welkert-Berke
chonnery silt loam). This soil has a moderate erosion hazard.
Griticol areas: Gritical areas on this site include steep slopes and woodlands. The steep
slopes are to be stabilized with standard EG-2 and the minimum area of woodlands are to
be disturbed to construct the driveway.
Erosion and sediment control measures: Gontrol of erosion and sedimentation for this site
shall be achieved by installing the following.
I. Gonstruction entrance.
2. Slit fence.
3. Gheck dams.
4. Outlet protection.
5. Gulvert Inlet protection.
6. Soil 5tabilizatlon matting.
Permanent stabilization: Once construction is complete, all disturbed areas not built upon
or paved shall be covered with a minimum of four (4) Inches of topsoil. Fertilizer, seeding
and a blanket of strov'i shall be applied to the topsoil.
5tormwater management: No stormwater management is proposed for this phase of
OTAN,S expansion due to the neglegible Increase in runoff due to development. Future
expansion may require stormwater management.
Maintenance Program: The erosion and sediment control measures Installed on the site -
shall be maintained by performing the following.
I. The site superintendent shall Inspect all erosion and sediment control devices after each
rainfall and at least dolly during prolonged rainfall. Damaged or deficient controls shall be -
repaired or improved immediately.
2 The site superintendent shall Inspect dolly the areas Gonstruction activlty to insure
erosion control effectiveness. Additional measures shall be installed Immediately where
deficiencies are found.
3. Permanent erosion control measures Include the installation of rip rap at storm sewer
outfolls and the establishment of grass on exposed surfaces disturbed In the construction
process.
Erosion and sediment .ontrol program
I. Hold pre construction meeting on site.
2. Install construction entrance.
3. Glear and grub the areas necessary for installation of the silt fence.
4. Grade the site, install the culverts and culvert inlet and outlet protection.
5. Fine grade and install the surface gravel. Temporary soil stabilization shall be applied to
all disturbed areas within 14 days if the area is to remain dormant for more than 21 days.
Apply loll stabilization matting immediately upon completion of grading.
6. After approval by the Inspector, remove the temporary control devices and stabilize the
areas disturbed during removal.
Note: Frederick County and VOOT retains the right to add and/or modify these erosion
and sediment control measures during the construction process, within reason, to ensure
adequate protection to the public and the environment.
Seeding schedule (soil stabilization):
1. Spread topsoil at a minimum depth of four (4) inches.
2. Incorporate pulverized agricultural limestone into the topsoil at a rate of 12 lbs. per
1,000 square feet (2 tons per acre).
3. Fertilize with 10-10-10 fertilizer at a rate of 23 Ibs per 1,000 square feet (1,000 Ibs per
acre).
4. Seed all areas with a seed mix consisting of 6-7% Kentucky 31 Tali Fescue and 33%
Red Top Glover.
5. Mulch all seeded areas with straw mulch applied at a rate of 5,500 Ibs per acre,
anchored with cutback or emulsified asphalt applied at a rate of 200 gallons per acre.

EROSION AND 5EDIMENT GONTROL
NARRATIVE -
Description: This project proposes construct a turn lane on Rte. 522, relocate
approximately 2,500 feet of a private access way within a ingress/egress easement and
construct a 4400 square feet office building. The total area to be disturbed by this project
is approximately 5.4 acres.
Date of construction: Gonstruction will begin following plan approval and will take
approximately one (1) year to complete with the exception of the turn lane in Rte 522
which will take place in conjunction of the second phase of the expansion of OTAN's
fac111tles..
Existing site conditions: The area to be disturbed under this plan is currently Improved by
a gravel driveway, church, dwelling and a office building.
Adjacent areas: This project is bounded on by Route 522 on the east and by RA zoned
property owned by Herbert Davis, John Warzlnski, Otis Goodwin Bethany HIII Baptist
Church, Murrell Renner and Outreach to Asia Nationals.
Solis information: The existing soil conditions were obtained from the U50A Soils
Survey. The predominant loll types for this site are 41G and 41E ( Welkert-Berks
channery silt loam). This soil has a moderate erosion hazard.
Gritical areas: Critical areas on this site Include steep slopes and woodlands. The steep
slopes are to be stabilized with standard EG-2 and the minimum area of woodlands are to
be disturbed to construct the driveway.
Erosion and sediment control measures: Control of erosion and sedimentation for this site
shall be achieved by installing the following.
I. Gonstruction entrance.
2. Slit fence.
3. Gheck dams.
4. Outlet protection.
5. Gulvert inlet protection.
b. Soil stabilization matting.
Permanent stabilization: Once construction Is complete, all disturbed areas not built upon
or paved shall be covered with a minimum of four (4) Inches of topsoil. Fertilizer, seeding
and a blanket of straw shall be applied to the topsoil.
stormwater management: No stormwater management Is proposed for this phase of
OTAN,5 expansion due to the neglegible Increase in runoff due to development. Future
expansion may require stormwater management.
Maintenance Program: The erosion and sediment control measures Installed on the site
shall be maintained by performing the following.
I. The site superintendent shall Inspect all erosion and sediment control devices after each
rainfall and at least daily during prolonged rainfall. Damaged or deficient controls shall be
repaired or Improved immediately.
2 The site superintendent shall inspect dolly the areas construction activity to insure
erosion control effectiveness. Additional measures shall be installed Immediately where
deficiencies are found.
3. Permanent erosion control measures Include the installation of rip rap at storm sewer
outfolls and the establishment of grass on exposed surfaces disturbed in the construction
process.
Erosion and sediment control program
I. Hold pre construction meeting on site.
2. Install construction entrance.
3. Glear and grub the: areas necessary for installation of the silt fence.
4. Grade the site, install the culverts and culvert inlet and outlet protection.
5. Fine grade and install the surface gravel. Temporary loll stabilization shall be applied to
all disturbed areas within 14 days if the area Is to remain dormant for more than 21 days.
Apply soil stabilization matting Immediately upon completion of grading.
6. After approval by the inspector, remove the temporary control devices and stabilize the
areas disturbed during removal.
Note: Frederick County and VDOT retains the right to add and/or modify these erosion
and sediment control measures, during the construction process, within reason, to ensure
adequate protection to the public and the environment.
Seeding schedule (soil stabilization):
I. Spread topsoil at a minimum depth of four (4) Inches.
2. Incorporate pulverized agricultural limestone into the topsoil at a rate of c12 lbs. per
1,000 square feet (2 tons per acre).
3. Fertilize with 10-10-10 fertilizer at a rate of 23 Ibs per 1,000 square feet (1,000 Ibs per
acre).
4. Seed all areas with a seed mix consisting of 67% Kentucky 31 Tall Fescue and 55%
Red Top Glover.
5. Mulch all seeded areas with straw mulch applied at a rate of 5,500 ibs per acre,
anchored with cutback or emulsified asphalt applied at a rate of 200 gallons per acre.

OUTREACH FOR A51A NATIONALS
EROSION AND 5EDIMENT CONTROL
NARRATIVE
Description: This project proposes construct a turn lane on Rte 522, relocate
approximately 2,500 feet of a private access way within a ingress/egress easement and
construct a 4400 square feet office building. The total area to be disturbed by this project
Is approximately 5.4 acres.
Date of construction: Construction will begin following plan approval and will take
approximately one (1) year to complete with the exception of the turn lane in Rte 522
which will take place in conjunction of the second phase of the expansion of OTAN's
facilities..
Existing site conditions: The area to be disturbed under this plan Is currently improved by
a gravel driveway, church, dwelling and a office building.
Adjacent areas: This project Is bounded on by Route 522 on the east and by RA zoned
property owned by Herbert Davis, John Warzinskl, Otis Goodwin Bethany Hill Baptist
Ghurch, Murrell Renner and Outreach to A510 Nationals.
Soils information: The existing soil conditions were obtained from the USDA Soils
Survey. The predominant soil types for this site are 41G and 41E ( Neikert-Berks
chonnery silt loam). This soil has a moderate erosion hazard.
Critical areas: Critical areas on this site include steep slopes and woodlands. The steep
slopes are to be stabilized with standard EG-2 and the minimum area of woodlands are to
be disturbed to construct the driveway.
Erosion and sediment control measures: Control of erosion and sedimentation for this site
shall be achieved by installing the following.
I. Gonstruction entrance.
2. 511t fence.
5. Gheck dams.
4. Outlet protection.
5. Culvert inlet protection.
6. 5oll stabilization matting.
Permanent stabilization: Once construction is complete, all disturbed areas not built upon
or paved shall be covered with a minimum of four (4) inches of topsoil. Fertilizer, seeding
and a blanket of straw shall be applied to the topsoil.
stormwater management: No stormwater management is proposed for this phase of
OTAN,5 expansion due to the neglegibie increase in runoff due to development. Future
expansion may require stormwater management.
MoIntenonce Program: The erosion and sediment control measures installed on the site
shall be maintained by performing the following.
I. The site superintendent shall Inspect all erosion and sediment control devices after each
rainfall and at least daily during prolonged rainfall. Damaged or deficient controls shall be
repaired or improved immediately.
2 The site superintendent shall Inspect daily the areas construction activity to insure
erosion control effectiveness. Additional measures shall be installed immediately where
deficiencies are found.
5. Permanent erosion control measures Include the installation of rip rap at storm sewer
outfolls and the establishment of grass on exposed surfaces disturbed In the construction
process.
Erosion and sediment control program
I. Hold pre construction meeting on site.
2. Install construction entrance.
3. Glear and grub the areas necessary for installation of the silt fence.
4. Grade the site, install the culverts and culvert inlet and outlet protection.
5. Fine grade and install the surface gravel. Temporary soil stabilization shall be applied to
all disturbed areas within 14 days if the area 15 to remain dormant for more than 21 days.
Apply loll stabilization matting Immediately upon completion of grading.
6. After approval by the inspector, remove the temporary control devices and stabilize the
areas disturbed during removal.
Note: Frederick County and \/DOT retains the right to add and/or modify these erosion
and sediment control measures during the construction process, within reason, to ensure
adequate protection to the public and the environment.
Seeding schedule (soil stabilization):
I. Spread topsoil at a minimum depth of four (4) Inches.
2. Incorporate pulverized agricultural limestone into the topsoil at a rate of Q2 lbs. per
1,000 square feet (2 tons per acre).
3. Fertilize with 10-10-10 fertilizer at a rate of 23 Ibs per 1,000 square feet (1,000 ibs per
acre).
4. Seed all areas with a seed mix consisting of 6-7% Kentucky 31 Tall Fescue and 35%
Red Top Glover.
5. Mulch all seeded areas with straw mulch applied at a rate of 5,500 Ibs per acre,
anchored with cutback or emulsified asphalt applied at a rate of 200 gallons per acre.
