THE SANITATION AUTHORITY MAINTENANCE DEPARTMENT IS RESPONSIBLE FOR THE INSTALLATION OF WATER METERS, BEFORE A PERMANENT METER IS SET, THEY REQUIRE; 1) THE FRAME AND COVER TO BE PROPERLY ALIGNED WITH ITS METER BOX AND YORE BAR, 2) THE FRAME AND COVER TO BE SET TO THE FINAL GRADE OF THE LOT; 3) THE DISTANCE BETWEEN THE TOP OF THE COVER AND THE YOKE BAR BE BETWEEN 18 AND 21 INCHES; 4) ALL COMPONENTS OF THE METER BOX ASSEMBLY TO BE IN PROPER WORKING ORDER. IT IS THE DEVELOPER'S/ CONTRACTOR'S RESPONSIBILITY TO HAVE THE METER BOX ASSEMBLY INSTALLED CORRECTLY. 4. THE CONTRACTOR SHALL TIE-IN A NEW LINE TO AN EXISTING MANHOLE BE CORE DRILLING THE MANHOLE, EROSION AND SILTATION CONTROL NARRATIVE Project Description The owner proposes to build a church on a 22.6 acre parcel. The total area of disturbance on the parcel is approximately 2.0 acres. Storm rioter runoff from the improved areas will be collected in a storm sewer system and discharged to the proposed storm grater detention pond. Site grading will include the use of on -site materials to bring the site up to the proposed grades. Existing Site Conditions The site is currently occupied by a church. Slopes range from 2.0 53 to approximately 40.OX Adjacent Areas The site is bordered on the east by the Route 656 right-of-way. The remaining sides are adjacent to residential properties. Off -site Areas No off site disturbance or construction activities are anticipated. Critical Areas None ore identified. Erosion and Sediment Control Measures 3.02 A 12'w by 70'1 temporary stone construction entrance with o concrete woshrock will be constructed from Route 656. The entrance shall be maintained in o condition which will prevent tracking or flow of mud onto the public rights-of-woy. This may require periodic top dressing with additional stone or the washing and reworking of existing stone as conditions demand and repair and/or cleanout of any structures used to trap sediment. All materials spilled, dropped, washed, or tracked from vehicles onto roadways or into storm drains must be removed immediately. The use of water trucks to remove materials dropped, washed, or tracked onto roadways will not be permitted under any circumstances. 3.05 Silt fence will be installed in selected locations downstream from the construction area as a first measure of construction. Silt fence will be placed at the proposed toe of slope along the entrance where the fill slope will be extended for the turn lone. Silt fence will be installed around the downstream side of the topsoil stockpile. Silt fences shall be inspected after each rainfall and at least doily during prolonged rainfall. Any required repairs shall be mode immediately. Damaged, decomposed or otherwise ineffective sift fence shall be replaced. Sediment deposits should be removed after each storm event. They must be removed when deposits reach opproximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.07 A bole and gravel drop inlet sediment filter will be installed around each of the proposed storm voter drop inlets immediately after installation. The structure shall be inspected after each rain and repairs mode as needed. Sediment shall be removed and the trap restored to its original dimensions when the sediment has accumulated to one half the design depth of the trap. Removed sediment shall be deposited in a suitable area and in such a manner that it will not erode. Structures shall be removed and the area stabilized when the remaining drainage area has been properly stabilized. 3.08 A stone berm will be installed as culvert inlet protection at the upstream end of the detention pond outlet culvert. The berm shall be inspected after each rainfall and at least doily during prolonged rainfall. Any required repairs shall be mode immediately. Damaged otherwise ineffective portions of the stone shall be replaced. Sediment deposits should be removed after reaching approximately one-half the height of the barrier. Any sediment deposits remaining in place after the berm is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.09 Temporary diversion dikes will be constructed along the downstream limits of grading. The dikes will be constructed with on -site materials and will be seeded with the temporary seeding specification immediately after construction. The dikes shall be inspected after each rain and repairs made to the dike, flow channel, or outlet as necessary. 3.13 A temporary sediment trap will be installed as the outlet control structure for the diversion dikes along the north side of the construction site. The trap will be constructed in accordance with the typical detail and dimensions given on the plans. The trap will be constructed with on -site materials and will be seeded with the temporary seeding specification immediately after installation. The trap shall be inspected after each rain storm and repairs mode as necessary. 3.20 Rock check dams shall be installed as depicted on the plans. The check dams will serve as sediment control for the diversion berms as indicated. Sediment shall be removed from behind the check dams when it has accumulated to one half the original height of the dom. The dams shall be checked for sediment accumulation and functionality after each storm event and repairs mode as required. 3.31 Temporary seeding in accordance with the E & S Std & Spec 3.31 will be applied to the topsoil stock pile and oil areas which will not be brought to final grade within 30 days. Embankment or excavated slopes denuded for a period of greater than 30 days shall be temporar6ly seeded and mulched. To coincide with the anticipated construction dotes, the seeding will consist of 50 pounds per acre of German Millet. All temporary seeding areas will be mulched in accordance with the schedule included herein. 3.32 All disturbed areas will be stabilized by permanent seeding in accordance with the schedule included herein.
