Furthermore, all traffic control flaggers must be certified in accordance with Section 104.04(c) of the VDOT Road and Bridges Specifications. V4. The developer shall be responsible for relocating, at his expence, any and all utilities, including traffic signal poles, junction boxes, controllers, etc., owned by VDOT or private / public utility companies. It is the sole responsibility of the developer to locate and identify utility facilities or items that may be in conflict with the proposed construction activity. VDOT approval of these plans does not indemnify the developer from this responsibility. V5. Design features relating to field construction, regulations, and control or safety of traffic may be subject to change as deemed necessary by VDOT Any addi tional expense incurred as a result of any field revision shall be the responsibility of the developer. V6. Prior to initiation of work, the contractor shall be responsible for acquireing all necessary VDOT land use permits for any work within VDOT right-of-way. V7. If required by the local VDOT Residency Office, a pre -construction conference shall be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. V8. The contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT requires and shall receive 48 hours advance notice prior to any required or requested inspection. V9. The contractor shall be responsible for maintaining adequate access to the project from the adjacent public roadway via a construction entrance that is constructed and maintained in accordance with Section 3.02 of the Virginia Erosion and Sediment Control Handbook. Furthermore, access to other properties affected by this project shall be maintained through construction. The developer shall have, within the limits of the project, and employee certified by the Virginia Department of Conservation and Recreation (VDCR) in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures on a continuous basis for proper installation and operation. Deficiencies shall be promptly rectified. V10. The contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. V11. All water and sewer lines within existing or proposed VDOT right-of-way shall have a minimum thirty-six (36) inches cover and when possible shall be installed under roadway drainage facilities at conflict points. V12. Any unusual subsurface conditions (e.g., unsuitable soils, springs, sinkholes, voids, caves, etc.) encountered during the course of construction shall be immediately brought to the attention of the engineer and VDOT. Work shall cease in that vicinity until an adequate design can be determined by the engineer and approved by VDOT. V13. All fill areas, borrow material and undercut areas shall be inspected and approved by a VDOT representative prior to placement and fill. Where CBR testing is required, a VDOT representative shall be present to insure the sample obtained is representative of the location. When soil samples are submitted to private laboratories for testing, the samples shall be clearly identified and labled as belonging to a project to be accepted by VDOT and that testing shall be performed in accordance wtih all applicable VDOT standards and procedures. V14. All roadway fill, base, subgrade material, and bockfill in utility/storm sewer trenches shall be compacted in six (6) inch lifts to 95% of theoretical maximum density as determined by AASHTO T-99 Method A, within plus or minus 2% of optimum moisture for the full width of any dedicated street right-of-way. At the direction of VDOT, density tests shall be performed by a qualified independent agency in accordance with VDOT Road and Bridge Specifications. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. V15. VDOT Standard CD and UD underdrains shall be installed where indicated on these plans and/or as specified by VDOT. V16. The installation of any entrances and mailboxes within any dedicated street right-of-way shall meet VDOT minimum design standards and is the responsibility of the developer. V17. Prior to VDOT acceptance of any streets, all required street signage and/or pavement markings shall be installed by the developer or, at VDOT's discretion, by VDOT on an account receivable basis following the Manual On Uniform Traffic Control Devices. V18. The developer shall provide the VDOT Residency Office with a list of all material sources prior to the start of construction. Copies of all invoices for materials utilized within any dedicated street right-of-way must be provided to the local VDOT Residency Office prior to acceptance of the work. Unit and total prices may by obscured. V19. Aggregate base and subbase materials shall be placed on subgrade by means of a mechanical spreader. Density will be determined using the density control strip in accordance with Section 304 of the VDOT Road and Bridge Specifications and VTM-10. A certified compaction technitian shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. In addition to checking stone depths, a VDOT representative shall be notified and given the opportunity to be present during the construction and testing of the density control strip. V20. Asphalt concrete pavements shall be placed in accordance with Section 315 of the VDOT Road and Bridge Specifications. Density shall be determined using the density control strip as specified in Section 315 and VTM-76. A certified compaction technition shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. A VDOT representative shall be notified and given the opportunity to be present during the construction and testing of the control strip. V21. In accordance with Section 302.03, the foundations for pipe culverts thirty-six (36) inches and larger shall be explored below the bottom of the excavation to determine the type and condition of the foundation. The contractor shall report findings of foundation exploration to the engineer and VDOT for approval prior to placing pipe. Foundation designs shall comply with VDOT Road and Bridges Standard PB-1. Where soft, yielding, or otherwise unsuitable foundation is encountered, the foundation design and/or need for foundation stabilization shall be determined by the engineer and approved by VDOT. V22. Approval of these plans shall expire three (3) years from the date of the approval letter. V23. VDOT Standard CG-12 Curb Ramps shall be installed where indicated on these plans and/or as specified by VDOT. V24. VDOT Standard Guardrail shall be installed where warranted and/or as proposed on these plans in accordance with VDOT's installation criteria. Grading operations will generate fill and cut material which will be utilized on -site. Existing Site Conditions The site is an open field which is now vacant. The ground slopes down from the eastern property corner to the Valley Pike R-O-W and the proposed storm water detention facility that is to be located on the proposed 1 acre out parcel. The out parcel was subdivided from the original 2.72 acres lot. Adjacent Areas The proposed 1.73 acre site is at the corner of Volley Pike and Musket Drive which make up the northwestern and northeastern property boundaries. The southeastern property r and the southwestern property boundary is adjacent to an existing commercial property a p p y boundary is adjacent to the proposed 1 acre out parcel. Off -site Areas During clearing and grubbing, the site contractor may transport any residue off -site as required. During earthwork operations, the contractor may import materials as required to achieve final grades and specifications. Erosion and Sediment Control Measures 3.02 Initially, a 12'w by 70'1 temporary stone construction entrance will be constructed at the location of the proposed main site entrance off of Musket Drive. The entrance shall be maintained in a condition which will prevent tracking or flow of mud onto the public rights -of -way. This may require periodic top dressing with additional stone or the washing repair and/or clean out of an and reworkingof existing stone as conditions demand and e a a P / Y 9 structures used to trap sediment. All materials spilled, dropped, washed, or tracked from vehicles onto roadways or into storm drains must be removed immediately. The use of water trucks to remove materials dropped, washed, or tracked onto roadways will not be permitted under any circumstances. 3.05 Silt fence will be installed downstream from the construction area and maintained until permanent seeding has been established. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shall be made immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment shall be removed and the trap restored to its original dimensions when the sediment has accumulated to one half the design depth of the trap. Removed sediment shall be deposited in a suitable area and in such a manner that it will not erode. Structures shall be reimoved and the area stabilized when the remaining drainage area has been properly stabilized. 3.08 Silt fence shall be installed as culvert inlet protection at the upstream end of the existing cmp located in the right-of-way of Musket drive just north of the proposed project entrance. Silt fence shall also be installed at the upstream end of the existing cmp culvert under Mill Lane located near the outlet structure for the proposed storm water detention facility. The silt fence shall be maintained inaccordance with 3.05 above. 3.09 Temporary diversion dikes shall be installed along the downstream perimeter of the work area. The dikes will be constructed of with on -site materials and shall be seeded with the temporary seeding mix immediately after installation. The dikes shall be inspected after each rain storm and repairs made as needed. 3.10 One temporary sediment trap shall be installed as the outlet control structure for the temporary diversion dikes. The trap shall be constructed in accordance with the typical details and dimension given on these plans. The trap will be constructed with on site materials and seeded with the temporary seeding mix immediately after installation. The trap shall be inspected after each rain storm and repairs made as needed. 3.18 Outlet protection will be provided in the proposed stormwoter management pond at the outlet of the 15" RCP. Outlet protection will also be installed from between the storm water detention pond outlet structure outfall and the existing 15" cmp. The stone protection will be installed in accordance with the specifications and details on the plans. 3.30 Replacement of topsoil will generally occur throughout the work site where vegetation is to be reestablished. Topsoil will be uniformly spread and rolled to a minimum depth of 6" or as called for on the plans. Topsoil shall not be placed while in a frozen of wet condition. 3.31 Temporary seeding in accordance with the E do S Std do Sec 3.31 will be applied all P rY 9 P PP areas which will not be brought to final grade within 30 days. Embankment or excavated slopes denuded for a period of greater than 30 days shall be temporarily seeded and mulched. Permanent Stabilization The contractor shall stabilize all denuded land within 7 days after the end of construction.
