SITE PLM CHECKLIST
The checklist below indicates all the information=that needs to be -submitted as part of the site plan
application. All required information must.be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site- plan application will not be accepted and returned
to the applicant(s).
Site Plan Package
1. One set of approved comment sheets are required from the following review .
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

On April 6, 2001, Michael Artz, of Artz and Associates, informed me that MCI Worldcom
wishes to withdraw their application of Site Plan # 65-00. This site plan was for a proposed pole
building at MCI Worldcom's existing facility, located off of Middle Road on the above
referenced lot.

Dear Mr. Artz:
Thank you for your recent submission of the Site Development Plan for the MCI Worldcom Lease
Area. Unfortunately, at this time there are 15 issues that need to be addressed before we can consider
site plan approval. I have enclosed a marked copy of the site plan for you. The red markings on this
plan should coordinate with the various issues listed below.
Please label and show the location and total area of the administrative wing and all other uses
in the junction building.
2. Please provide the required parking calculations on the site plan.
Please show and label the location of the required parking, including lines and dimensions of
each space.
4. Please provide a statement to the effect that the facility will be in compliance with Federal
Communication Commissions established ANSI/EEE standards for electromagnetic field and
radio frequency radiation.
Please label the height of all structures at the site.
Please provide a statement to the effect that the proposed pole building will be painted blue
or gray in color or will be constructed with materials of a galvanized finish.
7. Please reference the previous site plan #02-83.
Please label and show the location of all utility lines, with the size of lines, mains, and laterals.
Also, provide the location of all utility easements.

9. Please label the junction building and communication tower as existing buildings.
10. Please provide the location of all outdoor lighting fixtures. If no outdoor lighting exists or is proposed,
please provide a statement to that effect. Furthermore, for existing or proposed lighting, please provide
a statement to the effect that all lighting will meet the requirements of the Frederick County Zoning
Ordinance.
11. Please provide the total floor area and ground floor area for the proposed pole building and the
junction building.
12. Please provide pavement specifications for the entrance and all other areas that are paved or will be
paved.
13. Please provide the location of all signs proposed on the property. If no signs are proposed, please
provide a statement to that effect.
14. Please provide the location of all outdoor storage areas. If no such storage areas are proposed, please
provide a statement to that effect.
15. Please provide the location of all outdoor trash receptacles. Ifno such receptacles are proposed, please
provide a statement to that effect.
After you have revised the site plan, please resubmit one copy so that I may verify the information contained
on the plan. I will need all approved review agency comment sheets and at least five copies of the final plan
for approval. Comment sheets are required from the following agencies: the Inspections Department, the
Frederick County Engineer (Public Works), the Frederick County Fire Marshal, and the Frederick County
Health Department.
Do not hesitate if you have any questions or concerns regarding this letter.

A VDOT review has been completed of the concept plan dated 12/11/00 for the
referenced project.
A site plan should be prepared for the proposed access entrance developed along the
geometrics shown in the Minimum Standards of Entrances to State Highways (Page 15,
copy attached).
The radial curb & gutter shown on Page 15 may be waived. All other dimensions and
geometrics should be provided as indicated to meet minimum requirements.
Please provide one copy of the plan to this office for VDOT review whenever they may
be available.
