This letter is a request for an application waiver fee for the minor site plan revision for the New life
Christian Church located in Frederick County, Virginia.
The footprint of the original approved site plan has changed. 1lle building is now expanded 9.5' further to
the rear. The front and side setbacks have not changed. We have also added a 10' landscape island to each
side of the building and adjusted the parking spaces. The new site plan now has two less spaces than the
original plan, but still is above the required number of spaces.

Dear Scott:
The Frederick County Department ofPlanning and Development administratively approved the above
referenced site plan on May 14, 2001. The site plan meets all requirements for development of a
public building/church in the (RA) Rural Areas Zoning District. Preliminary approval has been given
by all relevant review departments and agencies. As future issues arise with these departments and
agencies, please adhere to their concerns promptly.
The site plan is approved for the construction of a 7,125-square-foot church. The subject property
is located southwest of the City of Winchester, on Middle Road. The following lists of improvements
are the primary features associated with this development:
•_ Landscaping. A single row of Arborvitae is required to be planted along the perimeter of the
parking area that is closest to Middle Road.
• Pavement and 88 Parking Spaces. The site plan is approved to have 88 parking spaces,
including four disability spaces. Striping of all spaces and fire lanes is required. A minimum
of double prime and seal shall be provided.
• Concrete Sidewalks. A five-foot concrete sidewalk will be provided around the building to
provide safer pedestrian access to and from the parking lot.
• VDOT Entrance. A 30-foot entrance will be constructed with curb and gutter off of Middle
Road, as required by the Virginia Department of Transportation.
• Church Sinn. A sign with unspecified dimensions is shown on the site plan. A sign permit
is required prior to construction of any sign.

• Class IIB Well. The Frederick County Health Department and the Lord Fairfax
Environmental Health District have required a Class IIB well.
I am providing you with three copies of the approved site plan. Please forward these to the
appropriate representative for New Life Christian Church. Furthermore, please advise the owner that
a copy should be kept for fixture reference and an approved copy must be kept on the construction
site throughout the development process. Once site development is complete, the owner needs to
contact this office to schedule an on -site inspection.

We have received your revised site plan dated 02/21/01 for the
referenced
project. It appears all of our earlier review comments have been
satisfactorily addressed. Please advise the owner of our approval.
Also
please provide seven (7) sets of approved construction plans with signed
seal for VDOT distribution.
The appropriate land use permit may now be applied for to cover work
within
the Route 628 right-of-way. The permit is issued by this office and
will
require a minimum processing fee, surety bond coverage, and the salary &
expenses of a State assigned Inspector. A copy of any/all recorded
plats of
dedication for drainage easements or additional right-of-way required
for
implementation of this proposed project should be provided to VDOT prior
to
issuance of any land use permits. Dedication should be made to the
Commonwealth of Virginia.
Once satisfactory application has been made, a permit will normally take
approximately thirty days to process and issue. Should you have any

Dear Scott:
Upon review of the subject site plan, we offer the following comments:
Provide a copy of the drainage study performed for the subject site for our review.
Provide an evaluation of the existing 24 inch CMP under Middle Road to ensure that the
backwater condition from a 100 - year storm will not inundate the proposed church. Also,
the proposed finished floor elevation for the church should be 810.5.
2. Determine the drainage area into the proposed pond to determine if a sediment trap will
provide enough control for the proposed run-off. Provide a permanent concrete riser
structure within the stormwater pond to minimize the potential for clogging. Provide a
detail of the proposed structure. Also, provide dimensions of all outlet protection.
The proposed diversion berm along the western portion of the site will divert additional
stormwater onto the adjacent property at that location. Determine if an adequate channel
exists along the property and also if a drainage easement may be required if the berm is to
remain.
Once these comments are addressed, submit two (2) copies of the site plan for further
review. If you have any questions, do not hesitate to contact me

RE: New Life Christian Church Well
Dear Scott,
This letter is in reference to our evaluation of the proposed well site for the New Life
Christian Church public water supply. In accordance with 12VAC 5-590-280 of the
Commonwealth of Virginia Waterworks Regulations, this letter is to advise the proposed
well site as approximately located on tax map 463-A-4 (Lot 21, Clay Hill subdivision) is
tentatively approved by the Health Department for construction of a class II13 well. This
approval is valid for a period of 12 months. If the construction of the well has not
commenced by 4/3/02, a re-evaluation of the well site will be required.
The following items must be submitted to this office for final approval:
The well must be located at least 50 feet inside the boundaries of a surveyed, platted,
and dedicated well lot. The well lot must be graded to divert surface runoff from the
well and prevent standing water around well head. An all weather access road, public
or private, must be provided to the well lot. The well plat must be recorded in
Frederick County with the well lot dedication document. It is recommended that
surveying and platting of the well lot be delayed until testing verifies acceptable
water quality and quantity.
2. The well must be constructed to meet Class IIB requirements and cased with
approved casing to a depth of at least 50 feet. The bottom of the well casing must
terminate in solid bedrock or other impervious formation. In addition, the well must
be grouted to at least 50 feet. A signed copy of the water well completion report must
be provided to this office verifying these minimum construction requirements.
3. Please notify the health department of the date and approximate time the well is to be
grouted so an environmental health specialist can be present.
4. Upon completion of the well, a yield and drawdown test of a minimum duration of 12
hours is required to determine reliable well production. A copy of the completed test
results must be submitted to this office.
5. Near the end of the yield test, a water sample must be collected for nitrate & nitrite
analyses. You are requested to notify this office for assistance in the proper

6. A series of nine bacteriological test samples must be collected from the discharge of
the well and submitted to an approved laboratory to determine the bacteriological
quality of the raw groundwater prior to the issuance of an operation permit. One
sample must be collected each hour during the last nine hours of the pumping test.
These samples must be analyzed by the Most Probable Number ( MPN) test .

Should any of the initial nine samples indicate contamination, an additional eleven
samples for MPN analysis will be required.
7. Upon completion of all sampling requirements, plans and specifications for
development of the well head and installation of water system must be submitted to
this office for review. A professional engineer, licensed in Virginia, must prepare the
plans and specifications. A completed well application must be included with the
plans.
Upon receipt of all required information and approval of plans, the State Health
Commissioner will issue a construction permit. Construction of the facilities shall not
commence until receipt of the constriction permit.

The error appears to be the result of an incorrectly marked parking indicator, shown in the front of the lot.
The indicator should be 13 spaces instead of 16.
