The checklist below indicates all the information -that needs to be -submitted as part of the site plan
application. All required information must.be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).
Site Plan Package
1. One set of approved comment sheets are required from the following review
agencies prior to final site plan approval. It is recommended that applicants contact
the Department of Planning and Development to determine which review agencies
are relevant to their site plan application.

This letter is to inform you that the Frederick County Planning Department administratively approved
site plan 4040-97 on November 17, 1997.
This site plan consists of a 2,046-square-foot addition to the existing 2,590-square-foot building
(4,636 square feet total). In addition to the proposed addition, the parking lot will be improved to
comply with ordinance standards. Parking lot standards include curb and gutter around the lot's
perimeter, and landscaping.
Enclosed are two copies of the approved plan. This will enable you to provide your contractor with
an approved copy and maintain a copy for your records. Please have the contractor contact this
office to schedule an on -site inspection once all improvements are complete.

Dear Mr. RePine:
In accordance with your request dated November 11, 1997 you are given permission to install a temporary
construction entrance at the referenced location. This letter is to be considered your permit to do so. The following
provisions will be required:
• Work area protection signage is to be in accordance with the current Work Area Protection Manual while
working on the right-of-way.
• All work on the right-of-way is to be confined to the temporary entrance location only.
• A minimum 6" depth of VDOT # 1 (2-3 inch stone) course aggregate by 30' in width will be required. It is to
be placed from the edge of pavement and extended back onto private property far enough to prevent the
carrying of mud onto the roadway.
• Positive drainage is to be maintained within the right-of-way at all times.
• A wash rack will be required.
• A minimum 15" x 30' culvert will be required.
• Any disturbed areas of the right-of-way or damages to the pavement structure adjacent to this entrance will be
repaired by you to our satisfaction.
• This permit is being issued for 180 days.
• All work is to be completed in accordance with the enclosed Virginia Erosion & Sediment Control Handbook,
Specification 3.02.
The Department of Transportation will not be held liable for suit should such result from this operation nor is it
granting permission to grade on the property of others or disturb underground utility lines. If the above listed items
are not followed this permit may be revoked.

I have reviewed the above referenced site plan received by this office October 29, 1997. The
following comments will need to be addressed in order to proceed with approval of this site plan.
Please provide:
Site Plan Checklist
1) A space labeled "Approved by the Zoning Administrator" for the approval signature
and date of approval. Also include the statement that "This site plan is valid for a
period of five years from date of approval."
2) Location map (scale 1:2000).
3) The location and dimensions of all required setbacks. State both the required and the
provided setbacks. Required are: front - 35'; rear and side - 0'
4) Location of sidewalks and pedestrian way. A sidewalk should be provided along the
eastern edge of the parking lot.
5) Location, height, and specifications of outdoor lighting fixtures.
6) Building heights, maximum and provided.
Please revise your site plan to address the above comments, and the comments provided to you by
the review agencies.

You will also need to provide this office with the approved comment sheets from all required review
agencies, as discussed during the Technical Review Committee (TRC). The required review agencies

review. If the revised site plan satisfies all agency comments, we will request five copies of the final
site plan and recommend the site plan for. approval.

Background
Children's Services of Virginia, Inc. is located on Airport Road in the Shawnee District of
Frederick County. There is an approved site plan dated October 20, 1994 on file with
Frederick County. Our client has developed the site according to the site plan design and
now wishes to place a 2,066 sf addition onto eastern side of their existing 2,730 sf
building. Our client has met with County agencies and VDOT and has told us that the
Planning Department's only concern is the issue of stormwater management.
Method
This stormwater mangement plan will account for the existing, proposed, and future
structures shown on the site plan. We have approached the site as if it were still totally
undeveloped and reduced the ten year post -developed runoff to the two year predeveloped
conditions.
