The checklist shown below specifies the information which is required to be subnutted as part of the site plan
application. The Department of Planning & Development will review the application to ensure that it is
complete prior to accepting it. If any portion of the application is not complete, it will be returned to the
applicant(s).
(1) One (1) set of approved comment sheets are required from each relevant review
agency prior to final approval of a site plan- It is recommended that applicants

The Frederick County Department of Planning and Development adnlinistrativel}, approved the above -referenced
site plan on September 12, 2005. A pproval is granted for the construction of a parking lot addition for the existing
Triad Office on Aviation Drive. This expansion includes a ne\v truck turn -around, 16 new parking spaces and
landscaping. All requirements of the Frederick County Zoning Ordinance have been adequately addressed in the
site plan, including approvals front all relevant review agencies. Please note that this approval is for the Phase 11
parking area only. The building expansion and phase I parking were approved with Site Plan 458-00.
I alll providing you with three (3) copies of the approved site plan. Please forward these copies to the appropriate
representat ive(s). Furthermore, advise the owners) that a copy should be kept for future reference, and an
approved copy must be kept on the construction site throughout the development process. Once site development

Dear Ms. Perkins:
Attached herewith are one set of revised plans, one copy of the original approved plan set,
and the Triad responses to your letter dated June 9, 2005, copy attached. We are responding on a
point by point basis to each of the above mentioned comments.
Title sheet has been revised to remove the existing Zoning Administrator approval
signature.
2. We believe that we have now clarified the existing, proposed and relocated tree issue as
shown on plan sheets C-4 and C-6.
OTHER
As stated in our previous letter dated 5/31/05, and at the specific direction of Mr.
Beniamino at the 3/15/05 meeting, we have sent plans and comment sheets to only Frederick
County Planning and Frederick County Dept. of Public Works. As stated previously in our letter
of 5/31/05, we are only making a modification to an existing approved site plan package. There
is no need to request comments from Inspections Department, Fire Marshal, Sanitary Authority,
or VDOT. We are confused as to what benefit can be derived from supplying plan sets to these
departments for changes to an approved set of plans that does not increase building size beyond
what was approved on 3/05/02, does not request approval of an additional sewer lateral or
waterline connection, and does not request an additional driveway access. We are not clear as to
why is it necessary to take up their respective valuable time to review something that either does
not effect them or for which they have already approved? If you desire us to submit again to
these agencies, please further explain the purpose of submittals given the current plan conditions.

Dear Ms. Perkins:
Attached herewith are one set of revised plans, one copy of the original approved plan
set, and the Triad responses to your letter dated April 14, 2005, copy attached. We are
responding on a point by point basis to each of the above mentioned comments.
1. Since this is a minor alteration to an existing approved site plan, we believe it would be
inappropriate to completely remove information on any approved plan sheet, most especially, the
title sheet. If we were to remove the existing signed signature block, we would be in fact
submitting the package as a new submittal. This would be contrary to our intent of simply
altering an existing approved plan package that is still valid until 3/05/07. It would also be
contrary to the County recommendation and our concurrence at the 3/15/05 meeting with David
Beniamino and yourself where we agreed that we would only show the affected areas on the
appropriate sheets.
2. Again, it was our intent to alter the existing approved plan set as little as possible. In this
regard we would prefer to not alter sheet C-2 in any way. We feel that the "Business"
designation is adequate.
3. The Phase II (building addition) leader actually refers to the building addition area not
specifically the building addition, and we are (were) hesitant to alter anything on the plan sheet
other than what was required for the additional parking and the truck turn -around. However, in
the interest of clarity, we now show a revised leader to the building addition. You will also note
that we have altered the building addition footprint to reflect the actual proposed design. As per
your request, we have striped the future building addition. You are correct, the proposed
addition is 5740 square feet. However, only 4,000 square feet will be used for office space. The
remaining 1740 square feet is file storage and hallways.
4. The plan has been revised to show an additional handicap parking spot and the total
parking spaces has been revised to 63.

5. We have designed the truck turn -around using the program "Autoturn". The location and
configuration of the turnaround provides for the least amount of excavation and embankment.
We can provide a copy of the "Autoturn" printout should you so require. The plan now calls for
the placement of bollards to protect the employee parking. You will note this addition on sheets
C-4 and C-6.
6. The proposed parking lot expansion is now shown as Phase I1. The previous parking lot
expansion is now shown as Phase 1.
The deed book and page number for the sanitary and water easements are now shown.
8. The existing parking lot landscaping analysis (cross out) indicates the number of trees
required during the original plan submission and approval dated 3/05/02. The new (clouded)
parking lot landscaping analysis indicates the number of trees and landscaping required for the
total site based on today's criteria. (Zoning Chapter 165-27 (11) a and b dated 6-10-2003).
9. Since there is an existing 50+ foot wooded area between the existing and proposed
parking and Aviation Drive and since the 3 foot hedge was not required at the time of approval
3-05-02, we respectfully ask that this requirement not be enforced. You may also note that a 3
foot hedge does not exist between the parking lot and Aviation Drive on the Datalux Corporation
property directly across the street and we have never found this to be a problem.
10. Note 3 now indicates existing building and proposed building addition heights (25'-4").
Note 6 now states that all utilities are to be placed underground.
11. The landscape schedule on sheet C-6 has been revised to show tree caliper and
appropriate quantities for existing, proposed and re -located trees.
12. The landscape schedule on sheet C-6 has now been revised to show the correct number of
trees. Also see item I 1 above.
13. The word customer is now spelled correctly and the misplaced notation has been
removed.
14. A graphic scale is now shown on all appropriate plan sheets.

OTHER
At the direction of Mr. Beniamino at the 3/15/05 meeting, we have sent plans and
comment sheets to only Frederick County Planning and Frederick County Dept of Public Works.
We are only make a modification to an existing approved site plan package. There is no need to
request comments from Fire Marshal, Sanitary Authority, or VDOT. We are not increasing the
size of the approved addition. We are not requesting approval of an addditional sewer lateral.
We are not asking for an additional driveway access. You may remember that one of the
purposes of our meeting with Mr. Beniamino and you on 3/15/05 was to request a second access
for our trucks and your conclusion was that it could not be granted since it would be too close to
the Lantz driveway - hence my request for a truck turnaround in the same area which Mr.
Beniamino and you agreed would be granted and there would be no need to get VDOT involved.

around should be moved over so that trucks do not have to back up around the employee parking
spaces on the eastern side. Also, bollards need to be placed adjacent to the last employee parking
space on the eastern side so that trucks do not back into parked vehicles.
6) ParkinI4 Lot Phases. The parking lot additions are shown in phases. The first section is labeled
"proposed parking expansion" and the next section is labeled "Phase I". The first part of the
parking lot should be phase I and the second should be phase 11.

7) Legal References. Provide the legal references for all existing easements.
8) Parking Lot Landscaping Analysis. Under the calculations for this section on Sheet C-4,
provide the existing calculations and the proposed additions.
9) Perimeter Landscaping. The perimeter landscaping sections also require a three foot high
opaque hedge row (wall or berm) to keep headlights from shining onto adjoining properties and
road right-of-ways. The opaque screening needs to be placed along the parking lots that face
Aviation Drive.
10) Notes. On Sheet C-4, note 3 needs to include the actual height of the office addition. Also,
under the notes include a statement that all utilities will be placed underground.
11) Tree Size. On Sheet C-6 under the landscaping schedule, the Littleleaf Linden Trees need to be
planted at two inch caliper, not 36-48" tall. Also, it would be beneficial to include in this table
the total number of trees on this site (new and existing), as some of the existing trees are being
relocated.

Upon review of the subject site plan, we offer the following comments:
I) Provide a copy of the geotechnical engineering report for the subject site. Because
a 2:1 slope has been proposed adjacent to the building addition as well as the
parking lot, inclucle a slope stability analysis in the report.
2) The grading plan for the truck turn -around shall rcllect the tie-in of the proposed
contours with the existing contours. None are shown on the plan. Provide top of'
curb spot elevations for the proposed parking lot.
3) The detail for the silt fence shall inclucle wire reinforcement.
Once these comments have been addressed, submit one ( I ) copy for fuftllel" review.

As per our meeting of 3/15/05 with you and Ms. Perkins we are submitting the attached Site Plan
Application Package for the additional employee parking and a turnaround for our drilling rigs. You will
note that these changes are being added to our Building and Parking Expansion Plan which was approved
on 3/05/02.
Also as per our discussion of 3/15/05, we are submitting Comment Review Forms to only the
Planning Department (enclosed) and the Public Works Department (copy of cover letter enclosed).
We are in the final stages of negotiations with design/build contractors and are considering their
request to alter our building addition footprint slightly from what is shown. Should this occur we will not
increase the gross square footage beyond the previously approved 5740 square foot limit. Nor will we
allow the accompanying grading to enter the environmental corridor.
