The checklist below indicates all the information that needs to be submitted as part of the site plan
application. All required information must be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s).

As discussed, attached is a sketch showinci setbacks from the property lines to the possible future canopy. Quarles
they
Petroleum. Inc. does not intend to apply fol . r permit to install a canopy. However, the current permit application does
include installation of footers for the possible future canopy. It has been a standard practice by Quarles Petroleum, Inc. to
install the footers during initial site develop-enl to avoid a significant disruption to customer traffic should the canopy be
needed in the future.
The intent is to have an inspection on recon, to document the Tooter dimensions and installation. A future application for the
canopy installation would refer to the foote, inspection. In regard to the setback from Park Center Drive right of way, at the
time of the future canopy permit application. Quarles Petroleum, Inc. will make the decision whether to apply for a variance
(to reduce the required setback to match the setback allowed for retail fuel station canopies) or to increase the setback (by
revising the future canopy dimensions).

Dear Mr. Artz:
The Frederick County Department of Planning and Development administratively approved the above
referenced plan on January 16, 2002. The property is approved to be developed as a fuel station, which is
shown in detail on the approved plans. The fuel station is only intended to be used by trucks owned by Quarles
Petroleum. Furthermore, the fiiel station is not approved as a retail gas station. Improvements associated with
the project include the construction of an 8x8 shed, an entrance off. of Park Centre Drive (Route 1323), four
(4) fuel stations, and asphalt pavement with curb and gutter. Landscaping is being provided for the proposed
fuel station, as well as, for the remaining portion of Ronnie Sinkohovitch's land. Development of the
remaining land will require that a site plan (s) be submitted, reviewed, and approved by the Frederick County
Planning Department and all relevant review agencies.
I have already provided you fifteen (15) copies of the approved site plan. Please forward these to the
appropriate persons. Furthennore, please advise the owner that a copy should be kept for future reference and
an approved copy must be kept on the construction site throughout the development process. Once site
development is complete, the owner should contact this office to schedule an on -site inspection.

We have received your revised site plan dated December 17, 2001 for the referenced
project. It appears all of our earlier review comments have been satisfactorily
addressed. Please advise the owner of our approval. Also please provide 7 sets of
approved construction plans with signed seal for VDOT distribution.
The appropriate land use permit may now be applied for to cover work within the
Route 11 & 1323 right-of-way. The permit is issued by this office and will require a
minimum processing fee, surety bond coverage, and the salary & expenses of a State
assigned Inspector.
Once satisfactory application has been made, a permit will normally take approximately
thirty days to process and issue. Should you have any questions, do not hesitate to call.

A VDOT review has been completed on the preliminary site plan dated September 7,
2001 for the referenced project. Our comments follow and are shown in red on the
plan sheets.
Plan Sheet 1 of 3
1. The professional seal should be signed on the final plan sets.
Plan Sheet 2 of 3
2. Elevations at a minimum interval of 50' should be shown along existing edge of
pavements for Routes 11 and 1323.
3. An earthen berm should be constructed along the storm water management
pond to be 2' above roadways on Route 11 and Route 1323. An alternate could
be standard guardrail along the same location.
4. The AADT volumes should be shown for VDOT facilities along the site area. The
values shown should be projected at 4% compounded annually to the site build
out year. The final projection should be used to determine pavement structures.
Structure for 4,400* AADT is: 1 1/2" SM-12.5E; 2" IM-19.0; 4" BM-25.0; 6" 21-B
Type I Aggregate.
*Please note whatever year AADT is used the pavement structure should have these elements.

Upon review of the revised site plan dated November 9, 2001, we offer the following _
comments:
1. Prior to approving the plan, the site plan shall be stamped by a professional engineer or a class
III-B land surveyor.
2. The stormwater management calculations appear to adequately address all stormwater issues.
3. The note concerning the installation of the french drain shall indicate that is shall be inspected
by Frederick County Public Works.
4. Address comment (5) from the original letter dated October 15, 2001.
5. Comment (9) of the original letter indicated that the silt fence detail must include wire support.
Once these comments are addressed, submit two (2) copies of the site plan for further v
review. If you have any questions, do not hesitate to contact me.

Dear Mike:
Upon review of the subject site plan, we offer the following comments:
1. Place a note on the plan that an on -site pre -construction meeting is required prior to
commencing any site work. The owner or developer, contractor, representatives from the design
firm and from our office shall be present at the meeting to discuss erosion and sediment (e&s)
control and stormwater management issues.
2. Place the Responsible Land Disturber title block on the plans as shown. A certified erosion and
sediment control individual shall fill out the title block prior to obtaining a land disturbance
permit. This person shall be responsible for carrying out the approved erosion and sediment
control plan.
3. The site plan shall be stamped by a professional engineer or a class III-B land surveyor since a
stormwater analysis is being performed.
4. The stormwater management calculations need to include the following information:
a. Provide a drainage map of the entire watershed. This information shall be included while
performing your storage volume calculations.
b. Since the site has two (2) separate drainage divides, the stormwater draining to the northeast
shall be analyzed separately from the stormwater draining to the northwest. An adequate channel
evaluation shall be performed along Park Center Drive to ensure that the 24 inch culvert and the
ditches are adequate for the increased storm flows.
c. The pond calculations need to include all hydrographs, stage/storage calculations and total
volumes of stormwater held in the pond. Provide supporting data of your assumptions including
your runoff "c"-value and time of concentrations. Time of concentrations between pre and post -
developed conditions should change since you assumed grass cover for pre -developed conditions.

d. Provide a trash rack on top of the outlet structure.
e. In order to avoid standing water in the stormwater pond, we recommend grading the pond with
a minimum slope of 1 % or provide a french drain in the pond.
5. Provide a sediment trap analysis for all the proposed traps in accordance with vesch standard
3.13. Determine if the sediment traps exceed minimum standard six of the Virginia E&S law. If
the drainage area to the traps are three (3) acres or greater, you shall include a variance request
for our approval to exceed a minimum standard.
6. Provide perimeter erosion and sediment control along the downstream area of the pond to
control erosion run-off during construction of the pond and sediment traps.
7. Provide a temporary and permanent seeding schedule with mulch for the project.
8. Provide outlet protection at the outfall of the proposed 18 inch ADS stormsewer pipe. Provide
dimensions in accordance with vesch standard 3.18. We do not recommend the use of the six (6)
inch PVC stormsewer pipe under the entrance road due to clogging. We recommend the minimum
size of stormsewer pipes to be 12 or 15 inch.
9. The detail for the construction entrance shall include the use of a washrack. The detail for the
silt fence shall include the use of wire support. We recommend the use of a diversion berm in lieu
of the silt fence.
Once these comments are addressed, submit two (2) copies of the site plan for further
review. If you have any questions, do not hesitate to contact me.

Dear Mr. Artz:
I have had the opportunity to review the site plan that you submitted to our department for a proposed truck
filling station at the old drive-in theater on Martinsburg Pike. The letter at hand details staffs review
comments so you may prepare a revision. I am enclosing a red -marked copy of the site plan and a zoning
location map to visually assist you. Below is a list of staffs concerns and comments:
Review Comments
(1) Property & Zoning Boundaries. Please show the current property and zoning boundaries of all three
lots on the site plan. If it is the intent to subdivide the property into new lots, please either show the
proposed boundary lines and anticipated uses for each lot on the site plan, another page, or on an inset
map. It should also be noted that the Board of Supervisors must approve any subdivision of the
existing three lots since a Master Development Plan has not been submitted. Therefore, please provide
a statement on the site plan which states that the proposed property lines are contingent on approval
by the Board of Supervisors.
(2) Acreage. Please provide the total acreage of each existing lot on the site plan, as well as the acreage
of all proposed lots.
(3) Future PlanningIssues. ssues. Please breakdown the improvements shown on the site plan into two phases.
Phase One should be the improvements necessary to open the truck filling station. This would be
everything on the proposed lot, the entrance, and all landscaping along Park Center Drive. Phase Two
Should be shown as planned improvements. This should include the full screen and buffers, and the
landscape screen and buffers, along the southern boundary line.
(4) North Ai -row. Please provide a north arrow on the site plan.

(5) Adjoining Lot Information. Please correct the zoning information of the following adjoining lots:
• 54-A-85 (Moore's) - current zoning is B3, not B2
• 43-A-29 (Shade Equipment) - current zoning is M2, not B2
• 43-A-31 (Shade Equipment) - current is both M 1 and M2, not just M 1
(6) Height. Please provide the proposed height of the 8' x 8' shed.
(7) Building Setbacks. The required setbacks are 50 feet from Martinsburg Pike, 35 feet from Park
Center Drive, and 15 feet from all side and rear property lines. Please show the correct setbacks on
the site plan and in the information section of Page One of the site plan.
(8) F.A.R. and Open Space Calculations. Please provide F.A.R. Calculations and Open Space
Calculations (% of land landscaped) on the site plan. These calculations should only consider the lot
of the proposed truck filling station. You currently show that the F.A.R. is zero on the site plan. This
cannot be the case if an 8' x 8' shed building is proposed.
(9) Directional Arrows. Please provide the anticipated direction in which trucks will enter and depart the
filling station. I recommend you show directional arrows on the site plan. I also recommend that you
make these arrows part of the proposed development, so they are actually painted on the pavement.
(10) Parking Setbacks. Please label the five-foot parking setback on the site plan.
(1 1) Electric & Other Utility Connections. Please show and label the location of any proposed electric or
other utility connections. Furthermore, please provide a statement on the site plan that all new utilities
will be placed underground.
After you have revised the site plan, please resubmit one copy so that I may verify the information contained
on the plan. I will need all approved review agency comment sheets and at least five copies of the final plan
for final administrative approval. Comment sheets are required from the following agencies: Frederick County
Inspections Department; Frederick County Engineer; Frederick County Fire Marshal; Frederick County
Sanitation fluthority; and the I,"irginia Department of Transportation.

Upon review of the subject site plan, we offer the following comments:

1. Place a note on the plans indicating that a land disturbance permit shall be obtained from
the Frederick County Public Works Department prior to commencing site grading work.
2. Provide a detail of the proposed construction entrance in accordance with the Virginia
Erosion and Sediment Control Handbook (vesch) section 3.02. The construction entrance
shall include installing a wash rack. Also, a copy of the Virginia Department of
Transportation (VDOT) temporary entrance permit shall be submitted at the time of
application for a land disturbance permit.
3. Provide details from the vesch handbook for all proposed erosion and sediment control
measures specified on the plans, including but not limited to: silt fence, sediment trap, inlet
protection, etc.
4. Provide a temporary and permanent seeding schedule in accordance with vesch
specification 3.31 and 3.32.
5. Place a note indicating that the installation of the stormwater pond and sediment trap shall
be the first steps in the grading operation. Also, the stormwater pond needs to be placed
within a dedicated stormwater management easement.
6. The stormwater management calculations prepared by Painter -Lewis, P.L.C., appear to
adequately address the stormwater management requirements for the site.

7. Place a note on the plans indicating that a pre -construction meeting on -site shall be
scheduled with the Frederick County Public Works Department and the contractor prior
to commencing site grading work to discuss erosion and sediment control issues for the
site.

Dear Mike:
I have had the opportunity to review the above referenced site plan submitted to our department on March
30, 2000. The following comments should be addressed and included in a revision of this site plan. The
submitted plan is also marked with these comments.
Please provide a note under the approval box reading "Site plan shall be valid for five years from
date of approval." In addition, please state that approval is granted "by the Zoning
Administrator."
2. Please delineate the boundaries of all parcels involved in this site plan (i.e. 54A- I -A and 54A-1-13).
In addition, please state the individual acreage of each parcel.
3. Number nine of the General Notes states that the site consists of three separate parcels; however,
Real Estate records do not show any record of parcel 54A-1-C.
4. Please note that Parcel 43-A-29 does not appear to be an adjoining property.
5. Please state the current use for all adjoining properties.
6. Please note that parcel 54-A-85 is zoned B3. In addition, parcel 43-A-29 is zoned M-2. Please
ensure that the zoning of all adjoining parcels is correctly stated.
7. Please identify the 5' setback as a "5' parking setback."
8. Please show the location, height, and dimensions of all proposed signs. If this information is
unavailable, please provide a note specifying that all signs will conform to Section 165-30 of the
Frederick County Zoning Ordinance.
9. Please state the route numbers associated with Martinsburg Pike and Park Center Drive (I I and
1323, respectively) and provide the posted speed limit of Martinsburg Pike.
10. Please provide the locations of all easements including access, utility, and drainage easements.

11. Please specify the location and nature of all fire lanes, fire hydrants, and other facilities necessary
to meet Fire Code requirements. Please provide information regarding the location of all streams
and drainage ways. If no such elements exist, please provide a statement to that effect.
12. Please ensure that note number two under the VDOT notes reads as intended.
13. Parcel 54-A-85 appears to be zoned B3. Therefore, no buffer is required on this portion of the
site.
14. A category C buffer is required where the subject parcel adjoins property which is zoned RP. If
a full screen is to be utilized, please show the location of the six -foot -high hedge, fence, mound,
or berm element. A full screen also consists of a totally landscaped easement at least 10' in depth.
Please show the location of the landscaped easement and the layout of the plants to be utilized.
Please refer to the mark up plan for details regarding the location of a full screen buffer.
15. Phase I appears to allow for 59 spaces rather than 63 as stated in the narrative on Sheet 2. Please
resolve this discrepancy.
16. Please delineate the areas where the on -site pavement detail will be utilized and identify the edge
of pavement.
17. Although existing trees are present on site, perimeter landscaping must be provided along the
southwest property line. If mature trees already exist in this area, please show them on the site
plan. If not, please provide for additional shade trees in the landscape tabulation.
After you have revised the site plan, please resubmit one copy so that I may verify the information
contained on the plan. I will then need all approved review agency comment sheets and five copies of the
final site plan for approval. Comment sheets are required from the following agencies: the Virginia
Department of Transportation, the Frederick County Sanitation Authority, the Frederick County
Inspections Department, the Frederick County Engineer, and the Frederick County Fire Marshal.
